Özet
Çalışmamızın amacı Multipl Sklerozlu (MS) bireylerde üst ekstremiteyi çok yönlü değerlendirmek, sağlıklı bireylerle karşılaştırarak problemleri belirlemek ve problemlerin aktivite ve katılım ile ilişkisini incelemekti. Çalışmaya 40 MS'li birey ve MS'li bireylerle benzer demografik özelliklere sahip 40 sağlıklı birey dahil edildi. Tüm katılımcıların demografik bilgileri ve MS'li bireylerin klinik özellikleri sorgulandıktan sonra üst ekstremite kas kuvveti Hand Held Dinamometre ile; kaba kavrama kuvveti el dinamometresi ile; çimdikleyici kavrama kuvveti pinçmetre ile; duyu esteziyometre ve diyapozon ile; koordinasyon Parmak-Burun Testi ile; yorgunluk Yorgunluk Şiddeti Ölçeği ile; kognitif düzey Sembol Rakam Modaliteleri Testi ile; anksiyete ve depresyon Hastane Anksiyete ve Depresyon Ölçeği ile; aktiviteye yönelik kapasite Dokuz Delikli Peg Testi ve Minnesota El Beceri Testi ile; aktiviteye yönelik performans Motor Aktivite Günlüğü-28 ve ABILHAND ile; katılım ise Toplumsal Bütünleşme Anketi ile değerlendirildi. Değerlendirmelerin sonucunda MS'li bireylerde benzer özelliklere sahip sağlıklı bireylere kıyasla omuz abdüksiyon, dirsek fleksiyon, el bileği fleksiyon ve el bileği ekstansiyon kas kuvvetinde, tip, lateral ve üçlü kavrama kuvvetinde, vibrasyon ve iki nokta ayrımı duyusunda, üst ekstremite fonksiyonunda ve kognitif düzeylerinde etkilenim olduğu tespit edildi (p<0,05). MS'li bireylerde kas kuvvetinde, duyuda, kognitif düzeyde, yorgunlukta ve ruh halinde görülen etkilenimlerin aktivite ve katılım ile ilişkili olduğu bulundu (p<0,05). Çalışmamız literatürde göz ardı edilen üst ekstremite problemlerinin MS'de yaygın olarak görüldüğünü ve bu problemlerin hastaların aktivite ve katılım düzeyleri ile ilişkili olduğunu göstermiştir. Bu sonuçlar ışığında, MS'li bireylerin aktivite ve katılım düzeylerini geliştirmek için alt ekstremite problemleri kadar duyu, koordinasyon, kavrama kuvveti gibi üst ekstremite problemlerine yönelik yaklaşımlara da rehabilitasyonda yer verilmesinin önemi ortaya konmuştur.

The aim of our study is to evaluate the upper extremity comprehensively in people with multiple sclerosis (PwMS), identifying problems by comparing them with healthy individuals, and examining the relationship of problems to activity and participation. 40 PwMS and 40 healthy individuals were included in the study. Demographic information of all participants and clinical characteristics of PwMS were questioned. PwMS were evaluated with Hand Held Dynamometer for upper extremity muscle strength; hand dynamometer for grip strength; pinchmeter for pinch strength; estesiometer and tuning fork for sensory; Finger-Nose Test for coordination; Fatigue Severity Scale for fatigue; Symbol Digit Modalities Test for cognitive level; Hospital Anxiety and Depression Scale for anxiety and depression; Nine Hole Peg Test and Minnesota Hand Dexterity Test for capacity of the activity; Motor Activity Log-28 and ABILHAND for performance of the activity and Community Integration Questionnaire for participation. As a result of the assessments, it was determined that PwMS were affected at shoulder abduction, elbow flexion, wrist flexion and wrist extension muscle strength, tip, lateral and triple grip strength, sense of vibration and two-point separation,upper extremity function and cognitive levels compared to healthy individuals with similar characteristics (p<0,05). It was found that the effects of muscle strength, sense, cognitive level, fatigue and mood in PwMS were related to their activity and participation (p<0,05). Our study showed that upper extremity problems, which were ignored in the literature, are common in MS and these problems are related to the activity and participation levels of the patients. In the lights of these results, in order to improve the activity and participation levels PwMS, as well as lower extremity problems, upper extremity problems such as sensation, coordination and grip strength should be included in rehabilitation.
