Özet

Bu çalışmanın amacı hormon düzeyinden bağımsız olması sağlanarak, primer dismenoreli (PD) bireyleri aynı menstrüal safhada sağlıklı kontrollerle karşılaştırarak eklem pozisyon hissi, denge ve ikili görev performansında ağrı kaynaklı değişiklikleri incelemektir. PD tanısı olan (n:10) ve olmayan (n:10) bireyler, bir menstrüal siklus boyunca takip edilerek, menstrüasyon ve ovulasyon fazlarında propriyosepsiyon, denge ve ikili görev performansları değerlendirildi. Bireylerin ovulasyon dönemleri, tükürükten ovulasyon testi ile saptandı. Menstrüal ağrı şiddeti Vizüel Analog Skalası (VAS) ile belirlendi. Eklem pozisyon hissi, Biodex System 3 dinamometre (Biodex Inc, Shirley, New York) ile 30°lik hedef açının tekrarlanması şeklinde değerlendirildi. Denge performansı Biodex Balance System ile statik mod (SM) ve dinamik modda (DM) ayakta dururken, gözler açık (GA) ve kapalı (GK) durumda değerlendirildi. İkili görev performansı, denge görevleri sırasında verilen 3'er 3'er geri sayma görevi ile belirlendi. İkili görev sırasında kognitif performansın nasıl etkilendiğini yorumlayabilmek için geri sayma görevi tek başına ve farklı konumlardaki dengeyi korumaya çalışırken ikinci bir görev olarak talep edilerek skorları kaydedildi. Çalışmadan elde edilen bulgulara göre gruplar yaş, VKİ, menarş yaşı, menstrüal döngü süresi ve menstrüasyon süreleri bakımından benzerdi (p>0.05). Menstrüal dönemde; PD'li bireylerin ağrı şiddetinin kontrol grubundaki bireylerin ağrı şiddetinden anlamlı ölçüde yüksek olduğu tespit edildi (p<0.05). Gruplar arasında menstrüasyona karşı tutum benzerlik gösterirken, PD'li grubun menstrüal semptomlardan daha çok etkilendiği görüldü (p<0.05). Kognitif performans ve diz eklem pozisyon hissinde grup içi ve gruplar arasında anlamlı fark olmadığı saptandı. PD grubunda SM-GA koşulunda menstrüasyon döneminde ovulasyon dönemine göre denge performanslarının anlamlı olarak azaldığı tespit edildi (p<0.05). PD'li bireylerin SM-GK koşulunda menstrüasyon dönemindeki postüral salınımlarının kontrol grubundaki bireylerden daha yüksek olduğu ve bu farkın yalnızca mediolateral salınımlar için anlamlılık gösterdiği saptandı (p<0.05). PD grubunun menstrüasyon döneminde DM-GA ve DM-GK koşullarında postüral salınımları kontrol grubundan yüksek bulundu (p<0.05). PD'li bireylerin kognitif görevle beraber yürüttükleri SM-GA, SM-GK ve DM-GA koşullarında denge performansında menstrüasyon döneminde ovulasyon dönemine kıyasla anlamlı ölçüde yüksek salınım olduğu saptandı (p<0.05). PD'li bireylerin, DM-GK koşulunda menstrüal dönem ikili görev performansları sırasında genel ve mediolateral indeks puanlarının ovulasyon dönemindekinden anlamlı ölçüde yüksek olduğu bulundu (p<0.05). Çalışmamızdan elde edilen veriler ışığında PD'nin, eklem pozisyon hissini ve kognitif performansı etkilemediği, ancak denge ve ikili görev performansında azalmaya sebep olduğu yorumuna ulaşıldı.

The aim of this study is to examine pain-induced changes in joint position sense, balance, and dual-task performance by comparing individuals with primary dysmenorrhea (PD) with healthy controls at the same menstrual stage, ensuring that it is independent of hormone level. Individuals with a diagnosis of PD (n:10) and without (n:10) were followed throughout a menstrual cycle and their joint position sense, balance and dual task performances were evaluated during the menstruation and ovulation phases. Ovulation periods of individuals were determined by saliva ovulation test. Menstrual pain intensity was determined by Visual Analogue Scale (VAS). Proprioception was evaluated as repeating the target angle of 30° with the Biodex System 3 dynamometer (Biodex Inc, Shirley, New York). Balance performance with the Biodex Balance System while standing in static mode (SM) and dynamic mode (DM), eyes open (EO) and closed (EC) state. The performance of the dual task was determined by the countdown of 3 tasks given during the balance missions. In order to interpret how cognitive performance was affected during the dual task, the countdown task was requested alone and as a second task while trying to maintain the balance in different positions, and the scores were recorded.According to the findings obtained from the study, the groups were similar in terms of age, BMI, menarche age, menstrual cycle length and menstrual duration (p>0.05). During the menstrual period; It was determined that the pain intensity of the individuals with PD was significantly higher than the pain intensity of the individuals in the control group (p<0.05). While the attitude towards menstruation was similar between the groups, it was observed that the group with PD was affected more by menstrual symptoms (p<0.05). There were no significant differences in cognitive performance and knee joint position sense within and between the groups. In the PD group, it was found that balance performances were significantly decreased in the SM-EO condition during the menstruation period compared to the ovulation period (p<0.05). It was found that individuals with PD had higher postural oscillations during menstruation in the SM-EC condition than those in the control group, and this difference was significant only for mediolateral oscillations (p<0.05). Postural oscillations of the group with PD were higher in DM-EO and DM-EC conditions during menstruation than the control group (p <0.05). It was found that individuals with PD have a significantly higher oscillation in their balance performance during the menstrualperiod compared to the ovulation period in the SM- EO, SM-EC and DM-EO conditions that they carry out with cognitive task (p<0.05). It was found that the general and mediolateral index scores of the individuals with PD during the menstrual period dual task performance in the DM-EC condition were significantly higher than during the ovulation period (p<0.05). In the light of the data obtained from our study, it was concluded that dysmenorrhea did not affect joint position sense and cognitive performance, but caused a decrease in balance and dual task performance.
