Özet
Çalışmamızın amacı, bariatrik cerrahi bekleyen morbid obez bireylerde cerrahi öncesi dönemde uygulanan aerobik dans tabanlı egzersiz (ADTE) programı ve fiziksel aktivite danışmanlığının (FAD) cerrahi sonrası alt ekstremite fonksiyonlarına etkisini araştırmak idi. Çalışmaya 34 birey dahil edildi, randomize olarak grup I (ADTE ve FAD) ve grup II (FAD) olarak iki gruba ayrıldı. ADTE programında, alt ekstremite fonksiyonlarına yönelik hazırlanan koreografi ile müzik eşliğinde egzersizler günde 1 saat/haftada 2 gün/8 hafta olarak uygulandı. FAD programında ise fiziksel aktivite düzeylerini arttırmaya yönelik bilgi aktarımı ve motivasyon desteği sağlandı. Olguların alt ekstremite fonksiyonları 6 Dakika Yürüme Testi, Merdiven Çıkma-İnme Testi, Otur-Kalk Testi, Yarıskuat Testi ile; vücut kompozisyonu Inbody Biyoelektrik İmpedans Cihazı ile; alt ekstemite kas kuvveti Biodex İzokinetik Test ve Egzersiz Cihazı® ile; dengesi Biodex Denge Sistemi® ile; fiziksel aktivite düzeyi Uluslararası Fiziksel Aktivite Ölçeği ile; yorgunluk düzeyi Yorgunluk Şiddeti Ölçeği ve yaşam kalitesi ise Kilonun Yaşam Kalitesi Üzerine Etkisi-Kısa Sürüm Ölçeği ile değerlendirildi. Tüm değerlendirmeler tedavi öncesi-sonrası ve cerrahi sonrası 5. ayda yapıldı. Tedavi sonrası her iki grupta da alt ekstremite fonksiyonları, fiziksel aktivite, yorgunluk ve yaşam kalitesinde anlamlı değişim elde edildi (p<0,05). Gruplar arası karşılaştırmada, Grup I'de Grup II'ye kıyasla hem tedavi sonrası hem de cerrahi sonrası dönemde alt ekstremite fonksiyonları, kas kuvveti, fiziksel aktivite ve yorgunluk skorlarında elde edilen değişim istatistiksel olarak üstündü (p<0,05). Sonuç olarak, morbid obez bireylerde cerrahi öncesi uygulanan ADTE ve FAD'ın, FAD'a kıyasla cerrahi sonrası alt ekstremite fonksiyonları, kas kuvveti, fiziksel aktivite ve yorgunluk üzerinde etkili bir tedavi seçeneği olduğu bulunmuştur.

The aim of our study was to investigate the effects of aerobic dance-based exercise (ADBE) program and physical activity counseling (PAC) on post-surgical lower extremity functions in morbidly obese individuals who are awaiting bariatric surgery. 34 individuals were included in the study, randomly divided into two groups as group I (ADBE and PAC) and group II (PAC). In the ADBE program, choreography prepared for lower extremity functions and exercises accompanied by music were applied 1 hour a day/2 days a week/8 weeks. In the PAC program, information transfer and motivation support were provided to increase physical activity levels. Lower extremity functions with 6 Minute Walking Test, Stair Climbing-Stroke Test, Sit-and-Stand Test, and Semiquat Test; body composition with Inbody Bioelectric Impedance Device; lower extremity muscle strength with Biodex Isokinetic Test and Exercise Device®; balance with Biodex Balance System®; physical activity level with the International Physical Activity Questionnaire; fatigue with Fatigue Severity Scale and the quality of life with the Effect of Weight on Quality of Life-Short Version Scale were evaluated. Significant changes were obtained after treatment in lower extremity functions, physical activity, fatigue and quality of life in both groups (p <0.05). In the comparison between the groups, the change in lower extremity functions, muscle strength, physical activity and fatigue scores was statistically superior in Group I compared to Group II, both after treatment and in the postoperative period (p <0.05). As a result, it has been found that ADBE and PAC applied before surgery in morbidly obese individuals are an effective treatment option for improving postoperative lower extremity functions, muscle strength, physical activity level and fatigue, comparing to PAC.
