Özet
Ortognatik cerrahi, maksilla ve mandibulada dental ve iskeletsel deformitelerin düzeltilmesi amacıyla uygulanmaktadır. Bu amaçla en sık yapılan cerrahi işlem mandibulada bilateral sagittal split ramus osteotomisi (SSRO) ve maksillada Le Fort I osteotomisidir. İdeal ve stabil okluzyona ortodontik tedavi ve ortognatik cerrahi iş birliği ile ulaşılır. Dentofasiyal deformitesi olan hastalarda yüz asimetrisi insidansı nispeten yüksektir. Yüz asimetrisi olan hastalar, asimetrik mandibular geriletme veya ilerletme gibi karmaşık mandibular hareketlere maruz kalırlar. Bu karmaşık mandibular hareketlerin proksimal segmentin pozisyonunu ve açılanmasını simetrik ilerletme veya geriletmelere göre daha da şiddetlendireceği bilinmektedir. Ameliyat sonrası proksimal segmentteki değişiklikler iskeletsel stabiliteyi etkileyebilir ve bazen temporomandibular eklem bozukluğu semptomlarına neden olabilir. Literatürde proksimal segmentin pozisyonunun korunması amacıyla tanımlanan kabul görmüş bir teknik bulunmamaktadır. Bu çalışmanın amacı asimetrik ilerletme veya geriletme cerrahileri sırasında gerçekleşen kondilin istenmeyen pozisyonel değişimlerini önlemek amacıyla Ellis tarafından 2007 yılında tanımlanan teknik (posterior bending osteotomi) ile konvansiyonel (tıraşlama) yöntemi kondil pozisyonuna etkileri açısından karşılaştırmaktır. Bu klinik çalışma prospektif, randomize ve kontrollü olarak tasarlandı. Çalışmaya 42 hasta dahil edildi. Bütün hastaların ameliyattan 1 hafta önce ve ameliyattan 3-6 ay sonra konik ışınlı bilgisayarlı tomografi (KIBT)'leri alındı ve temporomandibular eklem muayeneleri yapıldı. Bu hastalar rastlantısal olarak 2 gruba ayrıldı. Çalışma grubunda (N=20) SSRO sırasında erken kemik temasların giderilmesi için Ellis'in tanımladığı teknik kullanıldı. Kontrol grubunda (N=22) ise SSRO sırasında erken kemik temasları rond frez ile tıraşlanarak giderildi. Bütün hastaların ameliyat öncesi ve sonrası kondil pozisyonunun açısal ölçümleri MİMİCS (Materialise, Leuven, Belçika) yazılımı üzerinden yapıldı ve karşılaştırıldı. Kontrol grubunda grup içi, aksiyal ve koronal düzlemde istatistiksel olarak anlamlı fark bulundu ve kondilin içe doğru rotasyon yaptığı tespit edildi (p=0,002). Sagittal düzlemde grup içi ve gruplar arası fark bulunmadı (p>0,05). Temporomandibular eklem muayeneleri açısından çalışma grubunda eklem sesinin istatistiksel olarak anlamlı şekilde azaldığı tespit edildi (p=0,004). Ağız açıklığı miktarlarında ise her iki grupta da istatistiksel olarak anlamlı bir azalma tespit edildi (p<0,001). Çalışmanın sonuçlarında kondilin pozisyonunu korumada Ellis'in tanımladığı tekniğin başarılı olduğu, kontrol grubunda proksimal segmentteki açısal değişimlerin eklemde bir patolojiye sebep olmayıp eklemin adaptasyon sınırları içinde kaldığı ve ortognatik cerrahinin temporomandibular eklem semptomlarında genel olarak bir iyileşme sağladığı gözlemlendi.

Orthognathic surgery is performed for the correction of dental and skeletal deformities in the maxilla and mandible. The most common surgical procedure for this purpose is bilateral sagittal split ramus osteotomy (SSRO) in the mandible and Le Fort I osteotomy in the maxilla. Ideal and stable occlusion is achieved with the cooperation of orthodontic treatment and orthognathic surgery. The incidence of facial asymmetry is relatively high in patients with dentofacial deformity. Patients with facial asymmetry are exposed to complex mandibular movements, such as asymmetric mandibular setback or advancement. It is known that this situation will further intensify the position and angulation of the proximal segments compared to symmetrical advancement or setback. Changes in the proximal segment after surgery may affect skeletal stability and sometimes cause symptoms of temporomandibular joint disorders. There is no accepted technique defined in the literature for the purpose of preserving the position of the proximal segment. The aim of this study is to compare the technique modified by Ellis in 2007 and conventional shaving method with the effect of on condyle position in order to prevent undesired positional changes of the condyle during asymmetric advancement or setback surgeries. This clinical study was designed to be prospective, controlled, and randomized. 42 patients were included in the study. CBCT's of all patients were taken 1 week before surgery and 3-6 months after surgery and temporomandibular joint examinations were performed. These patients were randomly divided into two groups. In the study group (N = 20), the modification defined by Ellis was used to eliminate premature bone contacts during SSRO. In the control group (N = 22), premature bone contacts were eliminated by grinding with rond burs during SSRO. Angular measurements of condyle position before and after surgery of all patients were done using MİMİCS (Materialize, Leuven, Belgium) software and compared. In the control group, a statistically significant difference was found in the axial and coronal plane, and the condyle was found to be turned inwards in both planes (p=0,002). There was no statistically difference was found in the groups and between groups on the sagittal plane (p>0,05). In terms of temporomandibular joint examinations, it was determined that joint sound was statistically significantly reduced in the study group (p=0,004). A statistically significant reduction in mouth opening amounts was detected in both groups (p<0,001). In the results of the study, it was observed that the technique described by Ellis was successful in maintaining the position of the condyle, the angular changes in the proximal segment in the control group did not cause a pathology in the joint, remained within the limits of adaptation of the TMJ, and orthognathic surgery generally provided an improvement in the temporomandibular joint symptoms.
