Özet
Bu araştırma laparoskopik kolesistektomi sonrası uygulanan akupresürün ameliyat sonrası bağırsak hareketlerine, ilk gaz ve gaita çıkış sürelerine etkisini incelemek amacıyla uygulandı. Ön test/son test kontrol gruplu deneysel tasarımda olan araştırmanın verileri, Eylül 2018-Şubat 2019 tarihleri arasında toplandı. Araştırma İstanbul'da bulunan bir üniversitesi hastanesinin genel cerrahi servislerinde elektif laparoskopik kolesistektomi uygulanan ve araştırma kriterlerine uygun 60 hasta (deney grubu=30, kontrol grubu=30) ile gerçekleştirildi. Deney grubunda yer alan hastaların, sırasıyla CV-12, LIV-3 ve ST-36 noktalarına, 5 saniye süreyle 1-1.5 cm derinliğinde 3-5 kg'lık basınç uygulandı, 2 saniye ara verildi ve akupresür uygulamasına her bir noktada 3 dakika süreyle devam edildi. Akupresür deney grubunda yer alan hastalara ameliyat sonrası 1., 2., 3. ve 4. saatlerde olmak üzere toplam 4 kez uygulandı. Kontrol grubunda yer alan hastalara ise rutin ameliyat sonrası tedavi ve bakım girişimleri yapıldı. Gruplarda yer alan hastaların her akupresür uygulaması öncesi ve ameliyat sonrası 24. saatte bağırsak sesleri dinlendi, gaz ve gaita çıkışları olana kadar da takipleri yapıldı. Verilerin analizinde anlamlılık p<0,05 düzeyinde değerlendirildi. Araştırmaya başlamadan önce etik kurul ve kurum izni, yazılı ve sözlü olarak hasta izinleri alındı. Deney grubunda kontrol grubuna göre ameliyat sonrası bağırsak hareketi sıklık ortalamalarının, 2. saatte istatistiksel olarak anlamlı derecede (p<0,05) daha fazla olduğu, 3., 4. ve 24. saatlerde istatistiksel olarak çok ileri derecede anlamlı (p<0,001) daha fazla olduğu bulundu. Deney grubunda yer alan hastaların ameliyat sonrası ilk gaz çıkarma zamanının kontrol grubuna göre daha kısa ve aralarındaki farkın istatistiksel olarak çok ileri derecede anlamlı olduğu saptandı (p<0,001). Ayrıca, deney grubunda ameliyat sonrası ilk gaita çıkarma zamanı ortalamasının 39,67±15,72 (saat), kontrol grubunda 47,80±18,99 (saat) olduğu; deney grubunda ilk gaita çıkarma zamanının daha kısa olduğu, aralarındaki farkın istatistiksel olarak anlamlı olmadığı belirlendi (p>0,05). Laparoskopik kolesistektomi sonrası uygulanan akupresür bağırsak hareketlerini hızlandırmada, ameliyat sonrası erken dönemde gaz ve gaita çıkışının gerçekleşmesinde güvenli bir hemşirelik uygulamasıdır.

The aim of this study was to investigate the effect of acupressure after laparoscopic cholecystectomy on postoperative bowel movements, first flatus and defecation times. This pre‐test/post‐test control group experimental design study was conducted between September 2018 and February 2019 with 60 patients who underwent elective laparoscopic cholecystectomy at the general surgery wards of a university hospital in Istanbul (experiment group= 30, control group= 30). The CV-12, LIV-3 and ST-36 points of the patients in the experimental group were subjected to a pressure of 3-5 kg at a depth of 1-1.5 cm for 5 seconds, 2 seconds were interrupted and acupressure was applied for 3 minutes at each point it was continued. Acupressure was applied to the patients in the experimental group 4 times, 1st, 2nd, 3rd and 4th hours postoperatively. Routine postoperative treatment and care interventions were performed for the patients in the control group. Bowel sounds were rested before the acupressure application and at 24th postoperatively, and the patients were followed up until flatus and defecation exits. Significance was evaluated at p <0.05 level in the analysis of the data. Prior to the study, the consent of the ethics committee and institution, written and verbal patient consent was obtained. In the experimental group, the mean postoperative bowel sounds frequency was significantly higher (p<0.05) at the 2nd hour, and at the 3rd, 4th and 24th hours, it was found significantly higher (p<0.001). The postoperative first flatus time of the patients in the experimental group was significantly shorter than the control group (p<0.001). In addition, the mean postoperative defecation time in the experimental group was 39.67±15.72 (h) and 47.80±18.99 (h) in the control group; In the experimental group, the first defecation time was shorter and the difference was not statistically significant (p>0.05). Acupressure applied after laparoscopic cholecystectomy is a safe nursing practice for accelerating bowel movements and early postoperative flatus and defecation.
