Özet
Bu çalışmanın amacı altı yaş dişlerine direkt ve indirekt yöntemle uygulanan farklı kompozit sistemlerin uzun dönem klinik başarısının retrospektif olarak incelenmesidir. Bu tez çalışmasında Selçuk Üniversitesi Diş Hekimliği Fakültesi Pedodonti Anabilim Dalında 2012-2013 yılları arasında toplamda 100 dişe uygulanan direkt ve indirekt kompozit restorasyonlar arasından ulaşılabilen 60'ı retrospektif olarak değerlendirildi. Direkt yöntemde Aelite LS Posterior ve Clearfil Photo Posterior, indirekt yöntemde Estenia ve Tescera ATL kompozit sistemleri incelendi. Klinik değerlendirme; diş hassasiyeti, marjinal adaptasyon, marjinal renklenme, renk uyumu, retansiyon, yüzey görünümü, anatomik form ve sekonder çürük kriterlerinin incelenebildiği modifiye USPHS kriterlerine göre yapıldı. Kompozit rezinler tek tek ve direkt ve indirekt kompozit sistemi olarak bu kriterlere göre değerlendirildi. İstatistiksel analiz için SPSS 24.0 paket programı kullanıldı. Elde edilen veriler Ki-kare testi, Cochran's Q testi ve MC Nemar testi ile değerlendirildi. En az 7 yıllık takibi yapılan kompozit rezin restorasyonların değerlendirildiği bu çalışmada, değerlendirmeye alınan kompozit rezin sistemlerin tümünün klinik olarak tatmin edici başarı seviyelerinde olduğu görüldü. Değerlendirilen restorasyonlar arasında istatistiksel fark olmadığı görüldü. Direkt ve indirekt kompozit rezin sistemlerin benzer klinik başarı gösterdiği ve modifiye USPHS kriterleri arasında istatistiksel anlamlı farklılık olmadığı tespit edildi. Teknik hassasiyet gerektiren ve daha maliyetli olan indirekt teknikle uygulanan kompozit rezinlerin direkt kompozit rezin sistemlere göre uzun dönem klinik başarı açısından avantaj sağlamadığı ve bu sebepten tercih üstünlüğü olmadığı söylenebilir. The aim of this study was to investigate the long-term clinical success of different composite systems applied direct and indirect technics to first permanent molars. In this study, 60 of the direct and indirect composite restorations applied to a total of 100 teeth between 2012-2013 in Selcuk University Faculty of Dentistry Department of Pediatric Dentistry were evaluated retrospectively. Aelite LS Posterior and Clearfil Photo Posterior for direct method and Estenia and Tescera ATL composite systems for indirect method were examined. Clinical evaluation; tooth sensitivity, marginal adaptation, marginal discoloration, color matching, retention, surface appearance, anatomical form and secondary caries were examined according to modified USPHS criteria. Composite resins were evaluated individually and as direct and indirect composite system according to these criteria. SPSS 24.0 software was used for statistical analysis. Data were evaluated by Chi-square test, Cochran's Q test and MC Nemar test. In this study where composite resin restorations were followed for at least 7 years, all of the composite resin systems evaluated were clinically satisfactory. There was no statistical difference between the evaluated restorations. Direct and indirect composite resin systems showed similar clinical success and there was no statistically significant difference between the modified USPHS criteria. It can be said that composite resins applied with indirect technique, which require technical sensitivity and are more costly, do not provide advantage in terms of long term clinical success compared to direct composite resin systems and therefore, there is no preference advantage.
