Özet

Amaç: Tıbbi ve cerrahi tedavide faydalanılan ajanların oluşturabileceği ototoksisitenin erken tespiti işitmenin korunmasında önemlidir. Saf ses odyometrisinde tespit edilemeyen erken koklea hasarı otoakustik emisyon (OAE) ölçümleriyle saptanabilir. Bu çalışmada genel anestezik ajan sevofluran ile kullanımı giderek yaygınlaşan ve ototoksik potansiyeli bilinmeyen sedatif bir ajan olan deksmedetomidinin koklea üzerine etkisinin OAE ölçümleriyle gösterilmesi amaçlandı. Hastalar ve Yöntem: Elektif bir Kulak Burun Boğaz-Baş Boyun ameliyatı yapılacak olan ASA I, 18–50 yaş arası, işitmesi ve orta kulak basıncı normal sınırlar içinde olan 60 hastanın sağlıklı olan bir kulağı çalışmaya alındı. Bütün hastalara preoperatif Transient (TOAE) ve Distorsiyon ürünü OAE (DPOAE) yapıldı ve Sinyal/Gürültü (S/N) oranı kaydedildi. Hastalar rastgele iki gruba ayrıldı. Tüm hastalara anestezi idamesinde sevofluran % 1,5–2, O2-N2O % 50–50 uygulandı. S Grubundaki hastalar i.v. SF infüzyonu aldı. SD Grubundaki hastalara da ilaveten i.v. deksmedetomidin infüzyonu uygulandı. Hastalara intraoperatif indüksiyondan sonra 30. dakikada ve postoperatif 24. saatte TOAE ve DPOAE ölçümleri yapıldı. Hastaların her OAE ölçümü sırasındaki vücut sıcaklığı (VS) ve ortalama arter basıncı (OAB) değerleri ile intraoperatif 15 dakikada bir ölçülen OAB değeri kaydedildi. Bulgular: Demografik veriler ve operasyon özellikleri bakımından gruplar arasında anlamlı fark yoktu. Đki grup arasında VS açısından preoperatif, intraoperatif ve postoperatif olarak anlamlı fark bulunmadı (P>0,05). SD grubundaki hastaların intraoperatif ortalama arter basıncı 30. dakikadan sonra operasyon sonuna kadar S grubundaki hastalara göre anlamlı şekilde düşüktü (P<0,001). TOAE'de S ve SD grubundaki hastaların 1000, 1500, 2000 Hz'de S/N oranının intraoperatif olarak anlamlı şekilde düştüğü, SD grubunda ek olarak 3000 Hz' de anlamlı düşüş olduğu bulundu. S grubunda tüm frekanslarda postoperatif değerin preoperatif seviyeye dönmesine karşın, SD grubunda 1000, 1500 ve 2000 Hz'de postoperatif S/N oranı ortalamasının preoperatif ortalamayı yakalayamadığı aralarındaki farkın istatistiksel olarak anlamlı olduğu tespit edildi (P<0,001). Ayrıca 3000 Hz'de postoperatif S/N oranının SD grubunda S grubuna göre anlamı şekilde düşük olduğu görüldü. DpGram ölçümünde ise S grubunda 6000 Hz, SD Grubunda 6000 ve 8000 Hz hariç tüm frekanslarda her iki grupta intraoperatif OAE'lerde preoperatif değere göre anlamlı düşüş olduğu belirlendi. Öte yandan postoperatif değerlerin S grubunda bütün frekanslarda preoperatif seviyeyi yakaladığı, SD grubunda ise preoperatif seviyeden düşük kaldığı bu farkın 1500 Hz'de anlamlı ölçüde olduğu görüldü (P<0,05). Bununla beraber 500, 750, 1000 ve 2000 Hz'de SD grubunun postoperatif S/N oranı ortalama değerinin S grubunun postoperatif değerinden istatistiksel olarak anlamlı ölçüde düşük olduğu (P<0,05) anlaşıldı. Sonuç: Her iki grupta OAE'lerin intraoperatif düşük bulunmasının anestezi idamesinde kullanılan N2O'in orta kulak basıncını arttırmasına bağlı olduğu düşünüldü. Postoperatif OAE ölçüm değerleri göz önüne alındığında sevofluran anestezisinin ototoksisite açısından güvenilir olduğu, dekmedetodimidinin ise hipotansif etkisine ve muhtemelen koklea kan akımını azaltmasına bağlı olarak özellikle alçak frekanslarda kokleada kalıcı hasar yapabileceği sonucuna varıldı.

Aim: Early detection of the potential ototoxic effects of drugs which are used in medical and surgical treatments is very important to protect hearing. Early damage of cochlea may not be detected by pure tone audiometry, but can be diagnosed by measuring otoacoustic emissions (OAE). The aim of this study is to show the effects of sevoflurane and a sedative drug dexmedetomidine, whose ototoxic potential is unknown, on cochlea by measuring otoacoustic emissions. Patients and method: One side of healthy ears of 60 patients, between 18 to 50 years old, that were going to an elective ENT and head and neck surgery were enrolled to this study. The inclusion criteria for the study were to be ASA I and having normal hearing and middle ear pressure. Transient and Distortion Product otoacoustic emissions (TOAE, DPOAE) of all patients were measured preoperatively, and signal/noise (S/N) ratio were recorded. Patients were randomly divided into two groups. At the maintenance of anesthesia sevoflurane 1–2 % and O2-N2O 50–50 % were applied to all of the patients. Patients that are in the group S were infused normal saline solution, and the patients who are in the group SD were infused dexmedetomidine intravenously. TOAE and DPOAEs were measured intraoperatively 30 minutes after induction and 24 hours after operation. Body temperature and blood pressure levels were measured simultaneously by OAE measurements, also mean arterial pressure levels were recorded at every 15 minutes intraoperatively. Results: There was no statistically significant difference between the groups with respect to demographic data and the operation characteristics (P>0,05). Pre-, intra- and postoperative body temperatures respectively didn't differ significantly between the two groups (P>0,05). After 30th minute intraoperative mean arterial pressure levels were significantly lower in the group SD than the group S until the end of operation (P<0,001). At the TOAE measurements; S/N ratios at 1000, 1500 and 2000 Hz were significatly lower in the intraoperative measurement comparing with preoperative ones in both groups. Also at 3000 Hz, decreasing at S/N ratio intraoperatively was detected in the group SD. In the group S, postoperative TOAEs were similar with preoperative ones at all frequencies. But in the group SD, TOAEs at 1000, 1500 and 2000 Hz were found to be significantly lower than preoperative measurements still in postoperative period (P<0,001). Also at 3000 Hz, S/N ratio was found to be significantly lower in the group SD than the group S in postoperative period. In the evaluation of DpGram measurements, S/N ratios were shown to be decreased at all frequencies in intraoperative period in both groups except at 6000 Hz in the group S, and except 6000 and 8000 Hz in the group SD. But at all frequencies DPOAEs were seen as increased again postoperatively and found as similar with preoperative ones in the group S. However, in the group SD postoperative DPOAEs at all frequencies were lower than preoperative ones, and this difference was statistically significant at 1500 Hz (P<0,05). Also at frequencies 500, 750, 1000 and 2000 Hz, S/N ratios of the group SD were found significantly lower than ratios of the group S in postoperative measurements (P<0,05). Conclusion: The decrease in intraoperative OAEs compared with preoperative OAEs in both groups was thought to be related with the changes of middle ear pressure which was increased by N2O. If postoperative OAE measurements are evaluated, it can be commented as general anesthesia provided with sevoflurane is safe in terms ofototoxicity. On the other hand, dexmedetomidine can commit irreversible cochlear damage especially at low frequencies because of its hypotensive effect and possible effect of attenuating the cochlear blood flow.
