Özet
Bu çalışmanın temel amacı, toplum ruh sağlığı merkezinden hizmet alan şizofreni hastalarına bakım veren aile bireylerinin duygu dışavurumlarını değerlendirmektir. Araştırma Sakarya Eğitim ve Araştırma Hastanesi Toplum Ruh Sağlığı Merkezi'nde 17.12.2018– 01. 02. 2019 tarihleri arasında, şizofreni (DSM-4 tanı sistemine göre) tanılı hastalara birincil bakım vermekte olan 54' ü kadın, 53' ü erkek toplam 107 kişiyle yüz yüze görüşme yöntemi ile yapılmıştır. Veriler araştırmanın amacı doğrultusunda, yazılı onay alınarak hastaya bakım veren aile üyelerinden toplanmıştır. Veri toplama aracı olarak; hasta ve yakınlarının sosyo-demografik özelliklerini öğrenmek amacıyla Bilgi Toplama Formu ve“Duygu Dışavurum Ölçeği”kullanılmıştır. Çalışmada hastaneye çoklu yatışı olan hastalara bakım veren kişilerin duygu dışavurumu ölçek puanının, hastaneye hiç yatışı olmayan hastalara bakım veren kişilerden anlamlı düzeyde yüksek olduğu tespit edilmiştir. Hastası hastaneye hiç yatmamış olan bakım verenin çoklu yatışı olan hastaya bakım verene göre; TRSM'ye düzenli olarak devam edenlerin de etmeyenlere göre duygu dışavurum ölçeğinden aldıkları puanın istatistiki açıdan anlamlı düzeyde (p<0.005) düşük olduğu saptanmıştır. Çalışmada geliri ve çalışma hayatı olan bakım verenlerin, geliri ve çalışma hayatı olmayan bakım verenlere göre anlamlı düzeyde (p<0.05) duygu dışavurumu ölçek puanının düşük olduğu rapor edilmiştir. Çalışma sonucuna göre bakım verenin hastalıkla ilgili bilgi düzeyi ve hastayla geçirilen sürenin duygu dışavurumu üzerinde etkili bir yordayıcı olmadığı saptanmıştır. Özetle hastanın hastaneye yatış sayısı ve Toplum Ruh Sağlığı Merkezine düzenli olarak devam etmesi gibi klinik bulgularının bakım verenin duygu dışavurum düzeyini yordayan önemli iki değişken olduğu ve bakım verenin gelir ve çalışma hayatının var olması durumunun, bakım verenin duygu dışavurumu üzerinde etkili sosyo-demografik yordayıcılar olduğu bulgularına ulaşılmıştır. Çalışma Toplum Ruh Sağlığı Merkezinin bakım verenin hastasına yönelttiği duygu dışavurum düzeyini nasıl etkilediğinin incelenmesi açısından önemlidir. Toplum Ruh Sağlığı Merkezlerinin sayısının arttırılarak merkezlerdeki mesleki rehabilitasyon çalışmalarının istihdama yönelik olarak yapılandırılması önerilmektedir

This study aims to evaluate the expressed emotions of the family members who care for the schizophrenic patients under treatment in the community mental health centers. The study was conducted between December 17, 2018 and February 2, 2019 in the Community Mental Health Center affiliated to Sakarya Tranining and Research Hospital. Face-to-face interview method was employed in the study. A total of 107 people (54 female, 53 male) who were primary caretaker of patients diagnosed with schizophrenia (according to DSM-4 classification system) were interviewed. The data were collected from the family members giving care for the patients after giving consent in writing.“Expressed Emotion Scale”and Information Collecting Form were employed in the study as the data collection tools in order to gather information about the socio-demographic characteristics of patients and their caretakers. It was found that the points of the caregivers whose patients had no previous hospitalization were significantly lower than the points of the caregivers whose patients had multiple hospitalizations, and the points of the caregivers whose patients regularly attend to the community mental health centers were significantly lower than the caregivers whose patients didn't attend to the centers regularly (p<0.005). However, the expressed emotion scale point of the caregivers with income and the working life is significantly lower than the points of the caregivers with no income and working life (p<0.05). According to the results of the study, it was found that the level of knowledge of the caregiver about the disorder and the time spent with the patient were not effective predictors of expressed emotion of the caregivers. In summary, the patients' number of hospitalizations and the regular attendance to the community mental health center were two variables predicting the expressed emotion level of caregivers, and the income level and the working life of the caregiver were effective socio-demographic predictors on the expressed emotion of the caregivers. The study contributes to the literature by examining how community mental health center affects the expressed emotions of the caregivers towards the patient. It is suggested that the number of community mental health centers should be increased and the vocational rehabilitation studies in these centers should be oriented at increasing the employment of the patients.
