Özet

Giriş ve Amaç: Tıpta kötü haber, hastanın veya hasta yakınlarının gelecek planlarını olumsuz etkileyen her türlü haber olarak tanımlanır. Kötü haber verme hekimler için en zor sorumluklardan biridir. Bir hekimin profesyonel şekilde kötü haber verebilmesi için öncelikle iyi iletişim becerilerine sahip olması gerekmektedir. Yapılan araştırmalar hekimlerin gerek mezuniyet öncesi dönemde gerekse mezuniyet sonrası dönemde kötü haber verme ile ilgili yeteri kadar eğitim almadıklarını göstermiştir. Üstelik hekimlerin büyük kısmı kendilerini kötü haber verme konusunda deneyimsiz ve yetersiz görmektedir. Uygun olmayan bir biçimde verilen kötü haber hastaların doktoruyla ilişkisini kesmesine, tedavide uyumsuzluğa, anksiyete depresyon gibi psikiyatrik problemlere neden olabilmektedir. Bu çalışmada Atatürk Üniversitesi Tıp Fakültesi Hastanesi'ndeki araştırma görevlilerinin kötü haber verme deneyimlerinin ve bunu etkileyen faktörlerin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Çalışma Eylül-Aralık 2017 tarihleri arasında Atatürk Üniversitesi Tıp Fakültesi'nde yapıldı. Araştırmanın yapıldığı dönemde Atatürk Üniversitesi Tıp Fakültesi'nde toplam 318 araştırma görevlisi bulunmaktaydı. Hasta ile direk görüşmenin olmadığı mikrobiyoloji, radyoloji, patoloji gibi branşlardaki araştırma görevlileri çalışma dışı bırakıldı. Diğer araştırma görevlilerinden çalışmaya katılmaya gönüllü olan 232 kişi katıldı. Araştırma görevlilerinin bir kötü haber verme modeli olarak kabul edilen SPIKES (Setting up, Perception, Invitation, Knowledge, Emotions, Strategy and Summary) modeline göre ne kadar uygun kötü haber verdikleri değerlendirildi. Bu amaçla SPIKES modelindeki 6 başlık (ortam, algı, davet, bilgi, duygulanım, plan-özet) hakkında 20 soru hazırlandı ve beşli likert şeklinde cevap alındı. Ayrıca araştırma görevlilerinde kötü haber verme yöntemlerini etkileyebilecek faktörler sorgulandı. Veriler SPSS 20.0.0 paket programı ile analiz edildi. Tanımlayıcı istatistiklerin yanı sıra analizlerde Ki-kare ve Student T testi kullanıldı. İstatistiksel anlamlılık düzeyi p<0,05 kabul edildi. Bulgular: Katılımcıların %57,3'ü (n=133) erkek, %57,3'ü (n=133) evli ve %70,3'ü (n=163) dahili bilimlerde çalışıyordu. Yaş ortalaması 29±3,5 yıl, meslekte toplam çalışma süresi ortalama 4,5±3,3 yıldı. Katılımcıların %64,7'si (n=150) mezuniyet öncesi dönemde kötü haber verme ile ilgili eğitim almamıştı. %90,9'u (n=211) ise mezuniyet sonrası dönemde kötü haber verme ile ilgili herhangi bir eğitim almamıştı. Kötü haber vermede kendilerini iyi görenlerin oranı %38 iken; %35,8'i zorlandıklarını bildirdiler. Araştırma görevlilerinin %24,1'i görüşme için sessiz bir oda ayarlarken, %60,8'i görüşmeye yeterince vakit ayırdıklarını ifade etti. Araştırma görevlilerinin %75,9'u hastalarıyla empati yaptığını ifade ederken; %69'u görüşme sırasında hastaların duygulanımına izin veriyordu. %84,9'u göz teması kurduğunu ifade etti. %43,1'i bilgi verirken üzgünüm/size daha iyi haber vermek isterdim gibi cümleler kuruyordu. Araştırma görevlilerinin %89,7'si hastaların soru sormasına izin verdiklerini, %88,4'ü hastalıkla ilgili net bilgi verdiklerini ifade etti. Araştırma görevlilerinin %40,1'i görüşme sırasında bilgi vermeden önce hastanın hastalığı hakkındaki bilgisini sorguladığını, %41,4'ü hasta ve yakınları ile takip görüşmesi ayarladıklarını ifade etti. Erkek araştırma görevlilerinin kadınlara göre daha fazla oranda (%66,7, %33,3 sırasıyla) her görüşme öncesinde kendilerini tanıttıkları görülmüştür (p<0,05) Sonuç: Kötü haber verme, hasta-hekim iletişiminde önemli bir yere sahiptir. Bu çalışmada araştırma görevlilerinin hem mezuniyet öncesi hem de mezuniyet sonrası dönemde kötü haber verme konusunda yeterince eğitim almadıkları tespit edildi. Araştırma görevlilerinin kendilerini kötü haber vermede yeterli görmedikleri anlaşıldı. Profesyonel kötü haber verme konusunda ise SPIKES protokolüne göre duygulanım başlığında daha iyiyken; ortamın ayarlanması, davet ve bilgi gibi başlıklarda yetersiz oldukları söylenebilir. Hekim için en zor sorumluluklardan biri olan bu iletişim becerisini kazandırmak için gerek mezuniyet öncesi gerekse mezuniyet sonrası dönemde eğitim ihtiyacı karşılanmalı ve araştırma görevlileri desteklenmelidir.

Introduction and Aim: Bad news in medicine is defined as any news that adversely affects future plans of the patient or his/her relatives. Breaking bad news is one of the most difficult responsibilities for physicians. Breaking bad news in a professional way for a physician, it is necessary to have good communication skills. The previous researches have shown that doctors have not received as much training as needed about breaking bad news, in the pre-graduation period, or in the post-graduate period. Moreover, most physicians see themselves as inexperienced and incompetent in giving bad news. Inappropriately given bad news can cause patients to break off relationships with their physician, discordance of patient to the treatment, psychiatric problems such as anxiety or depression. In this study, it was aimed to determine the experiences of the residents of Atatürk University Medical Faculty and the factors affecting them. Materials and Methods: The study was carried out at Atatürk University Faculty of Medicine between September-December 2017. At the time of the research, Atatürk University Faculty of Medicine had a total of 318 residents. Residents in the departments such as microbiology, radiology, pathology where there was no direct interview with the patient were excluded from the study. Of the other residents, 232 were volunteered to participate. It is evaluted that how appropriate the residents breaking bad news according to the SPIKES (Setting up, Perception, Invitation, Knowledge, Emotions, Strategy and Summary) model, which was considered a breaking bad news model by researchers. For this purpose, 20 questions about the 6 topics (environment, perception, invitation, knowledge, affection, plan-summary) in the SPIKES model were prepared and a five-point Likert-type answer was obtained. In addition, residents were questioned about the factors that could affect their breaking bad methods. The data was analyzed with the SPSS 20.0.0 package program. In addition to descriptive statistics, Chi-square and Student's T test were used for the analyzes. Statistical significance level was accepted as p <0.05. Results: Of the participants 57.3% (n = 133) were male, 57.3% (n = 133) were married and 70.3% (n = 163) were studying in the internal sciences. The average age was 29 ± 3,5 years and the total duration of work was 4.5 ± 3.3 years. Of the participants 64.7% (n = 150) were not trained in the pre-graduate period. The rate of who did not receive training on bad news in the post graduation period was 90.9% (n = 211). Those who see themselves well when giving bad news was 38%, whereas 35.8% reported that they were compulsory. Of the participants 24.1% said they set up a quiet room for the interview, while 60.8% said they had enough time for the interview. Those who stated that they empathize with their patients were 75.9% of the residents, 69% allowed patients' affection, 84.9% stated that they established eye contact. While giving information, 43.1% of them were using words like I am sorry/I would like to give you better news. Of the residents 89.7% stated that they allow patients to ask questions, and 88.4% stated that they gave clear information about the disease. Of the residents 40.1% stated that they questioned the patient about their knowledge before giving information during the interview and stated that 41.4% had arranged follow-up interviews with patients and their relatives. It was seen that male residents were more likely to introduce themselves from the females (66.7%, 33.3%, respectively) before each interview (p <0.05) Conclusion: Breaking bad news has an important place in patient-physician communication. In this study it was determined that residents were not educated enough about breaking bad news both before and after graduation. Residents did not feel themselves competent for breaking bad news. In terms of professionally breaking bad news, they were better at the affection subheading of the SPIKES protocol; while incompetent at the setting of the environment, invitation and knowledge. In order to gain this communication skill which is one of the most difficult responsibilities for the physician, education needs should be met before and after graduation and residents should be supported.
