Özet
Periferik blok uygulamaları ultrason kullanımının yaygınlaşmasıyla birlikte anesteziyologlar tarafından anestezide öncelikli hale gelmiştir. Periferik sinir bloğu uygulamalarında anestezi sinirin innerve ettiği bölge ile sınırlıdır ve kardiyopulmuner sistem minimal etkilenir. Bunun yanında bilincin açık olması da başka bir avantajıdır. Bu çalışmada üst ekstremite cerrahisi yapılan hastalarda brakiyal pleksus bloğu ile genel anesteziyi hasta ve cerrah memnuniyeti, postoperatif komplikasyonlar açısından karşılaştırmayı ve maliyet analizi yapmayı amaçladık. Ameliyatlardan bir gün önce hastalar servislerinde görülerek uygulanacak anestezi yöntemleri ve çalışma hakkında bilgi verildi, onamları alındı. Olgular rastgele iki gruba ayrıldı. İlk gruba ultrason eşliğinde perifeik sinir bloğu yapıldı (BPB Grubu). Diğer gruba genel anestezi uygulandı (GA grubu). Hastalar standart şekilde monitörize edildi. Her iki anestezi yönteminde de hastalar için perioperatif dönemde kullanılan tüm ilaç ve malzemeler kaydedildi. Preoperatif ve postoperatif komplikasyonlar, ilk analjezik ihtiyaçları, Vizüel Ağrı Skoru, hasta ve cerrah memnuniyet skalaları kaydedildi. Çalışmada kullanılan ilaçlar için sosyal güvenlik kurumunun ödediği fiyatlar, malzemeler için ise hastanemizin ihale usulü satın alış fiyatları temel alındı. İki grup arasındaki demografik veriler, ASA sınıfları, ameliyat süreleri, cerrahi türleri benzerdi. Cerrah memnuniyeti ve hastanın hastanede kalış süreleri arasında anlamlı fark bulunmadı. Brakiayal pleksus bloğu yapılan grupta ortalama maliyet 38.45 TL, genel anestezi yapılan grupta ortalama maliyet 109.9 TL hesaplandı. Postoperatif 0.dk VAS değeri, ilk analjezik uygulanma zamanı, peroperatif komplikasyonlar ve hasta memnuniyeti açısından brakiyal pleksus bloğu genel anesteziye göre daha üstün bulundu. Brakiyal pleksus bloğu uygulanan üst ekstremite cerrahisinde genel anesteziye göre maliyet, VAS değeri, hasta memnuniyeti açısından daha iyi intraoperatif ve postoperatif sonuçlar sunmaktadır. Ultrason eşliğinde brakiyal pleksus bloğunun maliyet ve diğer avantajlarından dolayı uygun hastalarda genel anestezi yerine tercih edilebileceği kanaatine varıldı.

Peripheral nerve block practices are becoming primary choice of anesthesiologist together with the increasing usage of ultrasound. Peripheral nerve blockage is limited with the innervated region of the nerve and so minimally effects cardiopulmonary system. Besides these, vigilance of the patient is another advantage. In this study we aimed to compare general anesthesia and brachial plexus blockage in terms of patient and surgeon satisfactions, side effects and postoperative complications, cost analysis. One day before the operations patients were seen at wards, informed about the anesthesia methods and study then approvals were confirmed. Cases are divided randomly into two groups. Ultrasound guided peripheral nerve blocks were applied to group one (Group BPB). General anesthesia was applied to group two (Group GA). All patients were monitored in standard manner. All medications and supplies used for the patients of both groups were recorded. Preoperative and postoperative complications, first need of analgesic, Visual Pain Score, patient and surgeon satisfactions were recorded. The prices of medications used in the study were based on the price list of Social Security Institution and the supply prices were based on auction purchase prices of our hospital. Demographic data, ASA classes, operation durations, surgery types were similar in both groups. A significant difference was not found between the groups in terms of surgeon satisfaction and hospital stay of the patients. The cost for group of peripheral nerve blockage was 38,45 TL, and the cost for group of general anesthesia was 109,9 TL. Brachial plexus nerve blocking anesthesia is found to be superior to general anesthesia in terms of postoperative minute zero VAS, time to administer analgesic, peroperative complications and patient satisfaction. Brachial plexus nerve blocking anesthesia applied for upper extremity surgery offers better preoperative and postoperative results in terms of VAS, patient satisfaction, compared to general anesthesia. It's confident that ultrasound guided peripheral nerve blocks can be preferred in suitable cases instead of general anesthesia because of cost affectivity and other advantages.
