Özet

Omurilik yaralanmalarının mortalite ve morbidite sonuçlarının kötü olması ve tedavi imkanlarının kısıtlı olması omurilik yaralanmalarının fizyopatolojisini açıklamak, yeni tedavi yöntemleri geliştirmek araştırmacıların ilgisini çekmektedir. Omurilik yaralanmasında birincil hasar koruyucu sağlık yöntemleri ile engellense de deneysel araştırmalar olay anından sonraki ikincil hasarın fizyopatolojisinin anlaşılması ve önlenmesine yönelik olmaktadır. Bu nedenle çeşitli deneysel omurilik yaralanma modelleri oluşturulmuştur. Çalışmamızda tavşanlarda oluşturduğumuz deneysel kısmi omurilik yaralanma modelinde TNF-alfa blokeri (Etanersept) ve IL-12/IL-23 monoklonal antikorunun (Ustekinumab) kullanımı ile ikincil hasarın azaltılıp azaltılamadığının klinik, elekrofizyolojik ve histopatolojik olarak gösterilmesi amaçlanmıştır. Çalışmamızda 40 adet erkek Yeni Zelanda tavşanı 5 gruba ayrıldı: grup 1 [Omurilik hasarı + 2-4 saat sonra 2ml serum fizyolojik subkutan (s.c.), n=8], grup 2 [Omurilik hasarı + 2-4 saat sonra 2mg/kg etanersept s.c. n=8], grup 3 [Omurilik hasarı + 6-8 saat sonra 2mg/kg etanersept s.c. n=8], grup 4 [Omurilik hasarı + 2-4 saat sonra 2mg/kg ustekinumab s.c. n=8], grup 5 [Omurilik hasarı + 6-8 saat sonra 2mg/kg ustekinumab s.c. n=8]. Tavşanlarda deneysel omurilik yaralanması oluşturulması amacıyla epidural balon basısı yöntemi kullanıldı ve sonrasında basının neden olduğu omurilik hasarı yaralanma öncesi, yaralanmadan hemen sonra, yaralanma sonrası 1. gün, yaralanma sonrası 1. hafta ve yaralanma sonrası ikinci hafta modifiye tarlov puanı ile değerlendirildi, elektrofizyolojik olarak MEP kaydı alındı ve sakrifiye edilip histopatolojik açıdan inceleme yapıldı. Uyguladığımız deneysel modelde etanersept ile ustekinumabın omurilik hasarı üzerine iyileşmeye yardımcı olduğu motor muayene ile ortaya konulmuştur fakat elektrofizyolojik ve histopatolojik olarak etkinliği desteklenememiştir.

Researchers are compelled to investigate the physiopathology of spinal injuries and to develop new treatment method due to their critical morbidity and mortalities. In the case of a spinal injury, the primary damage can be prevented with preventive healthcare measures. Indeed, most of the associated research concerns the understanding and prevention of the secondary damages that occur after the incident. Therefore, there are various experimental spinal injury models. This study concerns an experimental study designed for the investigation of spinal injuries using rabbit models. The study aims to demonstrate (using clinical, electrophysiological and histopathological evidence) whether the administration of TNF-alpha blockers (Etanercept) and IL-12/IL-23 monoclonal antibodies (Ustekinumab) can improve spinal damage. The subjects of the study were 40 male New Zealand rabbits that were divided into five groups: Group 1 [Spinal damage + after 2-4 hours 2 ml subcutaneous (s.c.) physiological saline solution, n=8], Group 2 [Spinal damage + after 2-4 hours 2 mg/kg etanercept s.c., n=8], Group 3 [Spinal damage + after 6-8 hours 2 mg/km etanercept s.c., n=8], Group 4 [Spinal damage + after 2-4 hours 2 mg/kg ustekinumab s.c., n=8], Group 5 [Spinal damage + after 6-8 hours 2 mg/kg ustekinumab s.c., n=8]. Epidural balloon compression was used for the generation of the experimental spinal damage in the rabbits. The spinal damage was tested using the modified Tarlov scale and the motor evoked potentials (MEP) were electrophysiologically recorded. The evaluations were recorded at the following times: before the injury, right after the injury, 1st day after the injury, 1st week after the injury and 2nd week after the injury. Subsequently, the subjects were sacrificed and histopathologically evaluated. Through the motor examination, the experimental model has revealed that both etanercept and ustekinumab helped improve spinal damage. However, the significance was not supported with the electrophysiologic and histopathologic data.
