Özet
Amaç: Çalışmamızda kliniğimizde septal deviasyon ve septonazal deformite ön tanıları ile açık teknik septorinoplasti operasyonu geçiren hastalarda koku ve tat fonksiyonları ile cerrahi sonrası sonuçların değerlendirilmesi amaçlanmıştır. Yöntem ve Gereç:Çalışmaya kliniğimizde ameliyat edilen 109 açık teknik septorinoplasti hastası alınmıştır. Hastalara operasyon öncesi, operasyon sonrası 1. ve 3. aylarda koku için n-butanol eşik testi ve sniffin sticks' koku tanımlama testi, tat için sükroz eşik testi ve burun tıkanıklığı sonuçlarını değerlendirmek için SNOT-23 testi uygulanmıştır. Koku testi için azalan derişimlerde hazırlanan 10 adet n-butanol çözeltisi ve 12 adet Sniffin sticks®hazır test çubuğu kullanılmıştır. Tat testi için değişik derişimlerdeki 4 adet sükroz çözeltisi kullanılmıştır. Cerrahi sonuçları değerlendirmek içinde SNOT-23 testi kullanılmıştır. Bulgular: Çalışmamız kliniğimizde ameliyat edilen 109 hasta üzerinde yapılmıştır. Tüm gönüllülere preoperative, postoperatif 1. ve 3. ayda n-butanol,Sniffin sticks®, sükroz ve SNOT-23 testi yapılmıştır. Gönüllülerin preoperatif, postoperatif 1.ay ve postop 3.ay Sniffin' sticks ölçümleri arasında istatistiksel olarak anlamlı farklılık saptanmıştır (p=0.001; p<0.01).Gönüllülerin preoperatif, postoperatif 1.ay ve postoperatif 3.ay n-Butanol eşiklerinin ölçümleri arasında istatistiksel olarak anlamlı farklılık saptanmıştır (p=0.001; p<0.01).Gönüllülerin preoperatif, postoperatif 1.ay ve postoperatif 3.ay tat eşiklerinin ölçümleri arasında istatistiksel olarak anlamlı farklılık saptanmıştır (p=0.001; p<0.01).Gönüllülerin preoperatif, postoperatif 1.ay ve postoperatif 3.ay SNOT 23 ölçümleri arasında istatistikselolarak anlamlı farklılık saptanmıştır (p=0.001; p<0.01). Tartışma ve Sonuç: Daha once yapılan çalışmalardaseptorinoplasti cerrahisinden sonra koku fonksiyonlarında azalma veya düzelme olabildiği gösterilmiştir. Çalışmamızda septorinoplasti sonrasında koku ve tat fonksiyonlarının düzeldiği, burun tıkanıklığının azaldığı saptanmıştır. Çalışmamız tat fonksiyonunun da incelenmesi, deviasyon tipi ve osteotomi ilişkilerinin de ortaya konması açısından mevcut çalışmalardan farklılık göstermektedir. Literatürde septorinoplastinin koku ve tat üzerine etkilerini araştıran pek çok çalışmaya ihtiyaç vardır.

Objective: The aim of this study is to evaluate smell and taste functions and surgery results in patients with septal deviation and septonasal deformationundergoing open approach septorhinoplasty in our clinic. Method and Material: 109 patients who had open approach septorhinoplasty in our clinic is included to the study. In order to comprehend and investigate the smell functions n-butanol threshold test and Sniffin Sticks' smell identification test is applied to the patients. 10 n-butanol solution in descending concentration and 12 Sniffin' Sticks® test bar are used. In order to analyze the taste function sucrose threshold test was applied and 4 sucrose solutions in different concentrations are used. Lastly, for evaluating the surgery results SNOT-23 test is applied.All of the tests are applied preoperative and postoperative in 1st and 3rd months. Results:All of 109 Patients were included in the study and n-butanol, Sniffin Sticks, sucrose and SNOT-23 tests were applied preoperative and postoperative in 1st and 3rd months. There were statistically significant difference (p=0.001; p<0.01) between the n- butanol threshold results in perioperative,postoperative in 1st and 3rd months There were statistically significant difference (p=0.001; p<0.01) between the sucrose threshold results in perioperative, postoperative in 1st and 3rd months. There were statistically significant difference (p=0.001; p<0.01) between the Sniffin Sticks threshold test resultsin perioperative, postoperative in 1st and 3rd months.There were statistically significant difference (p=0.001; p<0.01) between the SNOT-23 threshold resultsin perioperative, postoperative in 1st and 3rd months.. Discussion and Conclusion: The previous studies demonstrated that the olfaction can be improved or affected negativaly after septorhinoplasty surgeries.In our study smell and taste functions were improved and nasal obstruction was decreased..The evaluation of taste function, the effects of osteotomy, deviation typemake our studydifferent fromothers. Further large-scale investigations in which both olfaction and taste are evaluated objectively are required.
