Köpeklerde kalıcı idrar kataterizasyonunun neden olduğu enfeksiyonlarda Sulfisaxazol'un terapötik ve profilaktik etkinliği

Özet

VI. ÖZET Bu çalışmada 1-3 yaşlı 15 erkek köpeğe 10 gün süreyle kalıcı üretral katater uygulandı. Köpekler üç gruba ayrıldı. Birinci grup 5 köpeğe 10 günlük kataterizasyon süresince 33 mg/kg vücut ağırlığı dozunda 12 saat arayla oral sulfisaxazol verildi. İkinci grup 5 köpeğe aynı dozda ve sıklıkta oral sulfisaxazol katater çıkartıldıktan sonra 10 gün süreyle verildi. Üçüncü grup 5 köpek kontrol grubu olarak bırakıldı ve bunlara hiçbir sağaltım uygulanmadı. Birinci grup köpeklerden araştırmanın 0, 5, ve 10. günlerinde ikinci ve üçüncü grup köpeklerden 0, 5, 10, 17 ve 24. günlerinde sistosentezle alınan idrar örneklerinde fiziksel, kimyasal, mikroskobik ve mikrobiyolojik muayeneler yapıldı. Araştırmanın 10 günlük periyodu süresince ikinci ve üçüncü grup köpeklerde halsizlik, huzursuzluk, iştahsızlık ve hematüri belirlenirken birinci grup köpekler klinik olarak sağlıklı görünmekteydiler. İkinci ve üçüncü gruptaki köpeklerin tamamının ve birinci gruptaki iki köpeğin idrar muayenelerinde idrar pH sının arttığı, proteinüri, hematüri, bol lökosit ve eritrosit, birkaç epitel ve strüvit kristalleri saptandı. Araştırmanın 5. günündeki idrar örneklerinin mikrobiyolojik muayeneleri ikinci grupta 4 köpeğin (% 80) E. coli (3 köpek) ve Staph aureus (1 köpek) ile, üçüncü gruptaki 3 köpeğin (% 60) E. coli (3 köpek) ile enfekte olduğu belirlendi. Araştırmanın 10. gününde idrar Örneklerinden izole edilen bakterilerin P. mirabilis (1. gruptan 1 köpek), P. vulgaris (1. gruptan 1, 2. gruptan 1 ve 3. gruptan 1 köpek) E. coli (2. gruptan 3 ve 3. gruptan da 4 köpek) ve Staph, aureus ( 2. gruptan 1 köpek ) olduğu saptandı. İkinci grup 5 köpekte oral sulfisaxazol sağaltımının başlatılmasından sonra iştah kaybı, halsizlik ve huzursuzluk düzeldi. Sağalümın 7. gününde idrardaki patolojik bozuklukların düzelmeye başladığı gözlendi. İdrar örneklerinin mikrobiyolojik kontrollerinde 2 köpekte E. coli, 1 köpekte Staph, aureus izole edildi. Sağaltımın 14. gününde ikinci gruptaki tüm köpeklerin klinik olarak sağlıklı göründükleri idrar analiz sonuçlarının bir köpek hariç normal olduğu ve mikrobiyolojik kontrollerde sadece bir köpekte E. coli nin izole edildiği belirlendi. Çalışmanın 24. gününde (Katater 14çıkartılmasından 14 gün sonra) hiçbir sağaltım yapılmayan ikinci grup köpeklerden biri hariç diğerlerinin klinik olarak iyi göründüğü, 4 köpeğin idrar analizlerinde patolojik değişikliklerin bulunduğu saptandı. İdrar örneklerinin mikrobiyolojik kontrollerinde 3 köpekte E. coli ve 1 köpekte P. vulgaris izole edildi. Sonuç olarak, kalıcı üretral kataterizasyonun uygulamanın 5. gününde enfeksiyona neden olduğu, enfeksiyonun sağaltımı için katater çıkartıldıktan sonra oral sulfisaxazol'un 33 mg/kg vücut ağırlığı dozunda 12 saat arayla ve 10 gün süreyle kullanılmasının, enfeksiyonun önlenmesi için aynı ilacın aynı doz ve sıklıkta kataterizasyon süresince kullanılmasına göre daha iyi sonuç verdiği kanısına varıldı. 15

VII. SUMMARY In this study, indwelling urethral catheter was applied to 15 male dogs aging 1 to 3 years old for 10 days. Dogs were divided into three groups. First group of 5 dogs were given oral sulfisaxazol at a dose rate of 33 mg/kg bodyweight 12 hours intervals during the 10 days' catheterization period. Second group of 5 dogs were given oral sulfisaxazol at the same dose and intervals for ten days after removing catheter. Third group of 5 dogs were left as a control group which was not applied any therapy. The physical, chemical, microscopic and microbiologic examinations of the urine specimens obtained via cystocentesis were performed on days 0,5 an 10 in the first group and on days 0,5,10,17 and 24 in the second and third groups of dogs. During the 10 days' period of the trail for the second and third groups of dogs the clinical findings were weakness, uneasiness, loss of appetite and hematuria while the first group of dogs seemed to be clinically healthy. The examination of the urine obtained from all the dogs in the second and third groups and two dogs in the first group revealed increament in urine pH, proteinuria, hematuria, profuse leucocyte and erytrocyte, a few epithels and struvit crystals. On day 5 of the trail microbiologic examination of the urine specimens revealed that the 4 dogs (% 80) in the second group (E. coli in 3 dogs and Staph, aureus in 1 dog) and the 3 dogs (% 60) in the third group (E. coli in 3 dogs) were found infected due to urethral catheterization. On day 10 the isolated bacterias in the urine specimens were P. mirabilis (1 dog from first group), P. vulgaris (1 dog from first group, 1 dog from second group and 1 dog from third group), E. coli (3 dogs from second group and 4 dogs from third group), and Staph aureus (1 dog from second group). 16In the second group of 5 dogs after commencing the oral sulfisaxazol therapy, Appetite loss, weakness and uneasiness improved. On day 7 of the therapy pathologic changes in urine began to improve. In the microbiologic controls of the urine specimens, E. coli was isolated in 2 dog and Staph, aureus was isolated in a dog. On day 14 of the therapy all the dog in the second group seemed to be clinically healthy, urine analyse results were found normal except one dog and in the microbiologic control E. coli was isolated in only one dog. On day 24 of the study, (14 days after removing catheter) in the third group which therapy had not given, clinical state was good except one. Pathologic changes were determined in urine analyses of 4 dogs. In the microbiologic controls of urine specimens E. coli in 3 dogs and P. vulgaris in 1 dog were isolated. As a result, it was stated that indwelling urethral cateterization caused infection on day 5 of the application. Using oral sulfisaxazol at a dose rate of 33 mg/kg body weight 12 hours intervals for 10 days after removing catheter for treating infection gave better results then using the same drug at the same dose and frequency throughout the 10 days' catheterization period for preventing infection. 17
