Özet
Serebral palsili çocuklarda ayak deformiteleri, motor fonksiyon, spastisite, ambulasyon ve denge düzeylerini araştırmayı planladığımız bu çalışmaya yaşları 2-18 yıl arasında değişen, 20 hemiplejik, 20 diplejik, 20 paraplejik ve 20 kuadriplejik toplam 80 serebral palsili çocuk alındı. Katılımcıların sosyodemografik verileri, botulinum toksin enjeksiyonu ve cerrahi girişim öyküsü, ayak deformitesi varlığı, dağılımı ve şiddeti kayıt edildi. Motor fonksiyonlar Kaba Motor Fonksiyon Ölçümü (KMFÖ) ve Kaba Motor Fonksiyon Sınıflandırma Sisteminin Genişletilmiş ve Yeniden Düzenlenmiş Şekli (KMFSS) ile, spastisite şiddeti Modifiye Ashworth Skalası (MAS) ile, denge Berg Denge Ölçeği (BDÖ) ve ambulasyon düzeyi Fonksiyonel Ambulasyon Sınıflaması (FAS) ile değerlendirildi. SP tipine göre oluşturulan gruplar arasında; yaş, cinsiyet dağılımı, botulinum toksin enjeksiyonu ve cerrahi girişim öyküsü açısından anlamlı bir fark bulunmadı. (p>0,05). Gruplar 1. MTP eklem sorunları açısından karşılaştırıldığında varus/valgus deformitesi varlığı açısından bir fark bulunmazken (p=0,613) deformite olan hastalar arasında semptom şiddetleri açısından fark bulundu (p<0,05). Pençe parmak deformitesi (p=0,014) ve MTP instabilitesi (p=0,008) kuadriplejik grupta daha fazlaydı. Hastaların %20'sinde pes cavus, %44'ünde pes planus deformitesi tespit edildi ancak gruplar arası dağılımda fark bulunmadı. Arka ayakta varus/valgus deformitesi (p=0,002) ve ayak bileğinde eklem hareket kısıtlılığı (p=0,005) kuadriplejik grupta daha fazlaydı. Gruplar arasında FAS (p=0,001), KMFSS (p=0,001) ve MAS (p=0,030) skorları açısından hemiplejikler lehine anlamlı fark tespit edildi. Tüm gruplarda BDÖ ve FAS skorları arasında istatistiksel olarak anlamlı pozitif korelasyon bulundu (p=0,001). Denge düzeldikçe ambulasyonda bağımsızlık da artmaktaydı. Sonuç olarak bu çalışmada ambulasyon düzeyi düşük kuadriplejik serebral palsili çocuklarda ayak deformiteleri daha sık görülmüştür. Kuadriplejik serebral palsili çocuklarda motor fonksiyonlar, ambulasyon ve denge daha olumsuz etkilenmektedir.

Evaluation of Foot Deformites in Cerebral Palsy Children A total of 80 cerebral palsy children with 20 hemiplegic, 20 diplegic, 20 paraplegic and 20 quadriplegic children aged between 2 and 18 years were included in this study which we planned to investigate foot deformities, motor function, spasticity, ambulance and balance levels in children with CP. Sociodemographic characteristics of participants, history of botulinum toxin injection and surgical intervention, presence, distribution and severity of foot deformities were recorded. Motor functions were evaluated by Gross Motor Function Measurement (GMFM) and Gross Motor Function Classification System (GMFCS), spasticity severity was assessed by Modified Ashworth Scale (MAS), balance by Berg Balance Scale (BBS) and ambulation level by Functional Ambulation Category (FAC). There were no difference among the groups, formed according to CP type, in terms of age, gender distribution, botulinum toxin injection, and surgical intervention (p >0,05). When groups were compared in terms of 1st MTP joint problems, no difference was found regarding varus/valgus deformity (p=0,613) whereas symptom severity was different among patients having a deformity (p<0,05). Claw toe deformity (p = 0.014) and MTP joint instability (p = 0.008) were more frequent in the quadriplegic group. Twenty percent of patients had pes cavus, 44% had pes planus deformity, but there was no difference in distribution among the groups. Varus/valgus deformities in the hindfoot (p=0.002) and joint motion limitation in the ankle (p=0.005) were higher in the quadriplegic group. Significant differences were found among the groups in favor of hemiplegics in terms of FAC (p=0.001), GMFCS (p = 0.001) and MAS (p = 0.030) scores. There was a statistically significant positive correlation between BBS and FAC scores in all groups (p=0.001). In conclusion, in this study, foot deformities were more common in children with quadriplegic CP with low level of ambulation. In children with quadriplegic CP motor functions, ambulation and balance are affected more negatively.
