Özet
Biyolojik Ajan Kullanan Psoriasis Hastalarında Tedavi Etkinliğinin Psoriasis Alan Şiddet İndeksi ve Yaşam Kalite İndeksleri ile Değerlendirilmesi Amaç: Biyolojik ajanlar yaklaşık 10 yıldır psoriasis tedavisinde kullanılmaktadır. Biyolojik ajanların tedavi etkinliği genellikle yapılan faz çalışmalarına veya retrospektif çalışmalara dayanılarak değerlendirilmektedir. Faz çalışmaları dışında tedavi etkinliğini değerlendiren prospektif çalışma sayısı sınırlıdır. Faz çalışmalarında ise yaşam kalite indeksleri sıklıkla kullanılmamakta ve genellikle PAŞİ kullanılarak tedavi etkinliği değerlendirilmektedir. Bu araştırmanın amacı biyolojik ajanların etkinliğini PAŞİ, VYA ve yaşam kalite indeksleri ile değerlendirmek ve psoriasis hastalarının biyolojik tedavi cevaplarını belirlemektir. Gereç ve Yöntemler: Araştırmaya klinik olarak orta- şiddetli kronik plak tip psoriasis tanısı olan, fotokemoterapi ve konvansiyonel sistemik tedavilere dirençli 11 kadın, 40 erkek toplam 51 hasta dahil edilmiştir. Hastalar prospektif olarak 6 ay boyunca izlenmiş ve tedavi öncesi, tedavinin 12. ve 24.haftasında PAŞİ, VYA, DYKİ, PİKİ değerleri kaydedilmiştir. Bulgular: Ortalama PAŞİ değeri, tedavi öncesi 17.27±7.49, 12.haftada 3.69±4.60 ve 24.haftada 4.01±7.89 olarak bulunmuştur. Tedavi öncesine göre 12. ve 24.haftada PAŞİ'de azalma istatistiksel olarak anlamlıdır. PAŞİ 75 yanıtına 12.haftada hastaların %68.8'inde 24.haftada ise %76.3'ünde ulaşılmıştır. PAŞİ 100 yanıtına ise 12.haftada hastaların %27.1'inde 24.haftada ise %42.1'inde ulaşılmıştır. Hastalar biyolojik naif/ dirençli, psoriatik artrit olan/ olmayan, kadın/ erkek, VKİ <25/ 25≤VKİ <30/ 30 ≤VKİ şeklinde gruplara ayrılarak tedavi yanıtları değerlendirilmiş ve gruplar arasında istatistiksel olarak anlamlı fark saptanmamıştır. Ortalama DYKİ değeri tedavi öncesi 15.09±7.48, 12.haftada 5.12±5.89 ve 24.haftada 3.68±4.50 olarak bulunmuştur. Tedavi öncesine göre 12. ve 24.haftada DYKİ'de azalma istatistiksel olarak anlamlıdır. Tedavi öncesi DYKİ'de etkilenme 10 puan altı ve üstü olanlarda tedavi ile DYKİ'de 5 puan ve üstü iyileşme sağlanan hasta oranları karşılaştırılmıştır. 12.haftada tedavi öncesi DYKİ 10 puanın altında olan hastaların %38.5'inde, tedavi öncesi DYKİ 10 puanın üstünde olan hastaların %91.4'ünde DYKİ'de 5 puan ve üzeri iyileşme 114 sağlanmıştır. 24.haftada da iki grup arasındaki sonuçlar benzer olup istatistiksel değerlendirmede anlamlı fark bulunmuştur. Tedavi öncesi DYKİ 10'un üzerinde olan hastalarda biyolojik tedavi ile DYKİ'de daha fazla iyileşme olmuştur. PAŞİ yanıtlarına göre DYKİ'de iyileşme oranları karşılaştırmasında hastalar PAŞİ<50, 50≤PAŞİ<75, 75≤PAŞİ<90, 90≤PAŞİ<100 ve PAŞİ=100 yanıtlarına göre gruplara ayrılmış ve 12. ve 24.haftada DYKİ düzelme oranları açısından gruplar arasında fark saptanmamıştır. PAŞİ 75 yanıtı elde edilen ve edilemeyen hastalardaki DYKİ düzelme değerleri karşılaştırmasında ise 12. haftada DYKİ'de düzelme PAŞİ 75 yanıtı elde edilenlerde 11.42 puan, PAŞİ 75 yanıtı elde edilemeyenlerde 6.06 puan olarak kaydedilmiştir. Yapılan istatistiksel değerlendirmede iki grup arasında anlamlı fark saptanmış olup PAŞİ 75 yanıtı elde edilenler DYKİ'de daha fazla iyileşme sağlamıştır. Ancak 24.haftada DYKİ'de düzelme PAŞİ 75 yanıtı elde edilenlerde 9.07 puan, PAŞİ 75 yanıtı elde edilemeyenlerde 11.04 puan olarak kaydedilmiştir ve iki grup arasında istatistiksel olarak anlamlı fark saptanmamıştır. Ortalama PİKİ değeri tedavi öncesi 23.58±10.20, 12.haftada 10.41±10.47 ve 24.haftada 8.15±8.58 olarak bulunmuştur. Tedavi öncesi hastalık şiddetini değerlendiren yöntemler arasındaki bağlantının araştırılmasında PAŞİ ve VYA'nın kendi arasında (r= 0.780), DYKİ ve PİKİ'nin de kendi arasında (r= 0.641) korele olduğu görülmüştür. Etanersept, adalimumab ve ustekinumab'ın tedavi yanıtlarının karşılaştırılmasında 12.haftada PAŞİ 75 yanıtları sırası ile %61.5- %57.9- %84.6, 24.haftada PAŞİ 75 yanıtları sırası ile %72.7- %76.9- %90.9 olarak bulunmuş, yapılan istatistiksel değerlendirmede 12. ve 24.haftada ajanlar arasında PAŞİ 75 yanıtları arasında istatistiksel olarak anlamlı fark saptanmamıştır. Sonuçlar: Çalışmamızda biyolojik tedavilerin etkinliği literatürle uyumlu bulunmuştur. Tedavi öncesi PAŞİ ve VYA ile DYKİ ve PİKİ arasında korelasyonun olmaması buna karşın PAŞİ ile VYA ve DYKİ ile PİKİ arasında korelasyonun olması hastalık şiddetini belirlemede yaşam kalite ölçeklerinin önemini göstermektedir. PAŞİ 90 yanıtında daha iyi yaşam kalite değerlerine ulaşılmakla birlikte kısa dönemde PAŞİ 75 yanıtının yaşam kalitesini düzeltmede yeterli olduğu görülmüştür.

Evaluation of Treatment Effectiveness by the Psoriasis Area Severity Index (PASI) and the Life Quality Indexes in Psoriasis Patients Treated with Biological Agent Objectives: Biological agents have been used for the treatment of psoriasis for about 10 years. The treatment efficacy of biological agents is usually evaluated on the basis of phase studies or retrospective studies. The number of prospective studies evaluating treatment efficacy is limited except for phase studies. The life quality indexes are not used frequently in phase studies and also treatment efficacy is usually evaluated by the PASI. The purpose of this study is to evaluate the treatment efficacy of biological agents with the PASI, BSA, and LQIs and to determine the response of psoriasis patients to biological treatment. Materials and Methods: Fiftyone patients (11 women and 40 men) who were diagnosed clinically as moderate to severe chronic plaque psoriasis and were resistant to photo-chemotherapy and conventional systemic treatments were included in the study. The patients were followed up prospectively for 6 months, and also the PASI, BSA, DLQI, and PDI values were recorded before treatment and at the 12th and 24th weeks of treatment. Results: The mean PASI scores before treatment, at the 12th and 24th weeks of treatment were 17.27±7.49, 3.69±4.60, 4.01±7.89 respectively. The decrease in the mean PASI scores at the 12th and 24th weeks of treatment was statistically significant compared to before treatment. PASI-75 was achieved in 68.8%of the patients at the 12th week and 76.3% of the patients at the 24th week. PASI-100 was achieved 27.1% of the patients at the 12th week and 42.1% of the patients at the 24th week. The patients were divided into groups as biological naive/resistant, with/without psoriatic arthritis, female/male and BMI<25/25≤BMI<30/30≤BMI. The treatment responses of the patients were evaluated and there was no statistically significant difference between the groups. The mean DLQI scores before treatment, at the 12th, 24th weeks of the treatment were 15.09±7.48, 5.12±5.89, 3.68±4.50 respectively. The decrease in the mean DLQI scores at the 12th and 24th weeks of treatment was statistically significant 116 compared to the baseline scores. The patients with the DLQI score of <10 and the DLQI score of >10 before treatment were compared with the patients with an increase of 5 or more points in the DLQI score after treatment. At the 12th week of treatment, there was an increase of 5 or more points in DLQI score in 38.5% of the patients with the DLQI score of <10 before treatment and in 91.4% of the patients with the DLQI score of >10 before treatment. At the 24th week of treatment, the results were similar between the two groups and also there was a statistically significant difference. It has been shown that DLQI improved significantly in the patients with the DLQI score of >10 before treatment. In the comparison of the improvement in the DLQI score according to the PASI responses, the patients were divided into groups as PASI<50, 50≤PASI<75, 75≤PASI<90, 90≤PASI<100 and PASI=100. There was no significant difference between the groups at the 12th and 24th weeks in terms of the improvement in the DLQI score. In the comparison of the improvement in the DLQI score in the patients who reached PASI-75 and did not reach PASI-75, the improvement in the DLQI score was recorded as 11.42 in those with a PASI-75 response and as 6.06 in those without a PASI-75 response at the 12th week. There was a statistically significant difference between the two groups and also those with a PASI-75 response had more improvement in the DLQI score. However, the improvement in the DLQI score was recorded as 9.07 in those with a PASI-75 response and as 11.04 in those without a PASI-75 response at the 24th week and also there was no statistically significant difference between the two groups. The mean PDI scores before treatment and at the 12th and 24th weeks of the treatment were 23.58±10.20, 10.41±10.47, 8.15±8.58 respectively. To investigate the link between the methods that assess the disease severity before treatment, there was a correlation between the PASI and BSA (r=0.780) and between the DLQI and PDI (r=0.641). In comparing treatment responses of etanercept, adalimumab, and ustekinumab, the PASI-75 response was 61.5%, 57.9% and 84.6% at the 12th week and 72.7%, 76.9% and 90.9% at the 24th week, respectively. There was no statistically significant difference between the agents at the 12th and 24th weeks in terms of the PASI-75 response. Conclusions: In our study, the effectiveness of biological agents was consistent with the current literature. The PASI was correlated with the BSA but was not correlated with the DLQI and PDI before treatment. Moreover, the DLQI was 117 correlated with the PDI but was not correlated with the PASI and BSA before treatment. This suggests that the physical assessment methods (PASI, BSA) and the quality of life indexes should be evaluated separately. Although a better quality of life was achieved in the PASI-90 response, the PASI-75 response was sufficient to improve the quality of life in the short term.
