Özet

Hepatoselüler karsinom (HCC), kanser kaynaklı ölümler bakımından üçüncü sırada yer almaktadır. HCC tedavisindeki kısıtlılık sebebi ile sıklıkla nüks gözlenmekte ve tedavi direnci gelişmektedir. HCC gelişimi ve ilerleyişinde karaciğer dokusu genelinde oksidatif stres uyarılmakta ve proliferasyon baskılanarak, hücreler durgun faz olan ''kanser dormansi''sine girmektedir. Tedavi direncinin temel mekanizması, hücrelerin ''Kanser dormansi'si ile gelişen stres bariyerine direnç göstermeleridir. Bu süreçte HGF/c-Met, PI3K/Akt ve mTOR gibi yolaklar aktive olmakta ve sonunda Epitelyal-Mezenşimal Dönüşüm (EMT) ile birlikte ve hücreler agresif karakter kazanmaktadır. Tümör baskılayıcı bir gen olarak tanımlanan Tiyoredoksin etkileşimli protein (TXNIP), oksidatif stresin ana regülatörüdür. TXNIP ekspresyonundaki artış oksidatif stresin artışına, hücre döngüsünün duraklatılmasına ve apoptotik yanıtın gelişmesine yol açmaktadır. Ancak TXNIP'in HCC gelişimi ve ilerleyişindeki rolü bilinmemektedir. Yüksek lisans tez projemiz kapsamında TXNIP gen ekspresyonunun agresif HCC hücre dizilerinde c-Met ile benzer şekilde yüksek olduğunu ve c-Met aktivasyonunun TXNIP'i aktive ettiğini gösterdik. Doktora tezimizin birinci kısmında oksidatif stresi hücrelerimizde model olarak oluşturduk. İkinci kısımda TXNIP eksprese etmeyen HuH-7 ve HepG2 hücrelerinde TXNIP'i aşırı eksprese edip, yüksek eksprese eden SNU-449 ve SK-HEP-1 hücrelerinde sessizleştirerek oksidatif stres üzerine etkisini inceledik. Ayrıca hücre canlılığı, proliferasyonu, ölümü, motilite, invazyon gibi biyolojik yanıtlar üzerine etkisi de incelendi. Elde edilen veriler, Zebrabalığı modelinde in-vivo tümör oluşumu ve metastaz analizleri ve normal, siroz ve HCC'li hasta karaciğer dokularındaki ekspresyon düzeyi incelenerek TXNIP'in prognostik belirteç potansiyeli olup olmadığı araştırıldı. Üçüncü kısımda tüm bu biyolojik süreçlerin düzenleyicisi olan c-Met'in TXNIP ekspresyonunu hangi moleküler mekanizmalar ile düzenlediğini araştırdık. TXNIP ekspresyonundaki artış/azalma ile c-Met aktivasyonu/inaktivasyonunun tanımlanan biyolojik yanıtlar üzerine etkisini inceledik. Bizim çalışmamız literatürde ilk kez TXNIP ekspresyonundaki artışın i) hücresel ROS miktarını artırdığını ancak HCC hücrelerinin bu yanıta dirençli olduğunu, ii) TXNIP aracılıklı olarak hücrelerin EMT benzeri fenotip kazandığını ve in-vitro motilite, invazyon ve anjiyogenez yeteneklerinin arttığını, iii) TXNIP ekspresyonunun düzenlenmesinde c-Met aktivasyonunun olduğunu, iv) TXNIP aşırı ekspresyonu ile gözlenen biyolojik yanıtların c-Met aktivasyonu ile daha da arttığını; v) Zebrabalığı modelinde TXNIP'in in-vivo metastaz sürecinde tetikleyici rol oynadığı; vi) HCC'li hasta dokularında TXNIP ekspresyonunun arttığını göstermiştir. Verilerimiz TXNIP'in c-Met hedef genlerinden biri olduğunu ve HCC'de agresif fenotipin gelişmesinde birlikte görev almalarından dolayı birleştirilmiş kemo- ve ilaç temelli terapi yaklaşımları için c-Met ve TXNIP'in iyi hedefler olabileceklerini desteklemektedir.

Hepatocellular carcinom, is the third leading cause in cancer-related deaths. Due to the limited availability of options in HCC treatment, relapses are often observed and resistance to treatment is often acquired. As part of the course of HCC development, oxidative stress in liver tissue is stimulated and cell proliferation is inhibited, forcing cells into ¨cancer dormancy¨. The main mechanism of drug resistance involves cells acquiring resistance to the stress barrier that is involved in cancer dormancy. During this process, signaling pathways such as HGF/c-Met, PI3K/Akt, and mTOR are activated, allowing cells to become more aggressive as a result of the epithelial-mesenchymal transformation (EMT). Thioredoxin-interacting protein (TXNIP), a gene characterized as being tumor suppressant, is the main regulator of oxidative stress. Increase in the expression of TXNIP corresponds to an increase in oxidative stress, arrest of the cell cycle, and the activation of apoptotic responses; however, little is known about the role of TXNIP in HCC tumorigenesis and progression. As part of our doctoral thesis, we have demonstrated that similar to c-Met expression, TXNIP expression is high in aggressive HCC cell lines, and that c-Met activation causes activation of TXNIP. In the first part of this thesis, oxidative stress was used as a model in our cells. In the second part, we have ectopically overexpressed TXNIP in HuH-7 and HepG2 cells, and silenced it in SNU-449 and SK-HEP-1 cells and have then examined the effects of these transformations on oxidative stress. Other conducted assays where the effects of TXNIP expression and silencing were studied include cell survival, proliferation, death, motility, and invasion. The collected data, in vivo tumorigenesis and metastasis analysis in zebrafish models, and TXNIP expression levels in tissue samples from patients with cirrhosis and/or HCC were examined to show whether TXNIP could be a potential biomarker in HCC prognosis. In the third part of the thesis, we have investigated the molecular mechanism by which c-Met, the master-switch for the aforementioned biological processes, regulates TXNIP expression. We have also examined the effects of change in TXNIP expression as a result of c-Met activation or deactivation on the biological responses. Our research, for the first time in literature, has demonstrated that increase in TXNIP expression causes (i) an increase in ROS levels in HCC cells (although HCC cells are resistant to this response), (ii) cells to undergo the EMT due to the effects of TXNIP, accompanied by an increase in vitro motility, invasion, and angiogenesis abilities. Our research has also demonstrated that (iii) c-Met activation plays a role in the regulation of TXNIP overexpression, (iv) c-Met activation further increases the biological responses that TXNIP elicits, (v) in vivo metastasis in zebrafish is, to some extend triggered, by TXNIP and that (vi) TXNIP expression is increased in tissue samples originating from HCC patients. Our data show that c-Met and TXNIP could be effective targets for miRNA chemotherapy (and combinations of it with conventional chemotherapy), as TXNIP is a target gene of c-Met in HCC, and as they act together in causing biological effects.
