Özet

Amaç ve Kapsam: Öğretmenler için ses birincil iş aracıdır. Öğretmenlerin günlük ders verme sürelerinin, konuşmaları ve ses yolları üzerine etkisi açıklığa kavuşmamıştır. Sese fazla yüklenilmesi ses rahatsızlıklarının oluşmasında ana risk kabul edilir ve yoğun ses kullanımı gerektiren meslekler için önemli bir meseledir. Çalışmamızda; akustik analiz MDVP parametreleri ile objektif, VHI puanları ve anket uygulaması ile subjektif olaraköğretmenlerin sessel davranış analizleri yapılmış ve günlük ses kullanım süreleri ile algılanan sessel kusurlar arasındaki ilişki araştırılmıştır. Yöntemler: Prospektif randomize kesitsel çalışma olarak Ankara'da hizmet veren okullar ve dershanelerde görev yapan öğretmenlerde; anket uygulaması, subjektif ses şikayet sorgulaması Voice Handicap Index ve Elektroglottografi (Laryngograph Micro Processor, EGG-D400, Seri No:10425, Wallington UK) cihazı ve Laryngograph Speech Studio programı kullanılarak hastaların ses ölçümleri yapıldı. Deneklerin K.B.B. hekimi tarafından VLS (Video-laringo-stroboskopi) ile ses tellerinin muayenesi yapıldı. Sesini profesyonel olarak kullanmak zorunda olmayan normal popülasyon ile (eş zamanlı aynı yöntemler kullanılarak yapılan değerlendirmeler) karşılaştırıldı. Örneklem evreni; tam gün (günde 6-12 saat) ders anlatan 30 öğretmen ve yarım gün (1- 4 saat) ders anlatan 30 öğretmenden, kontrol grubu cinsiyet ve yaş eşlenik, diğer meslek grubu çalışanları 60 kişiden oluştu. Çalışmada elde edilen akustik analiz MDPV programının temel frekans (F0), %Jitter ve %Shimmer parametreleri ile VHI puanları ve anket verileri istatistiksel yöntemlerle karşılaştırıldı. Bulgular: Akustik analiz MDPV parametrelerine göre; tam gün ders anlatan kadın ve erkek öğretmenlerin F0 değerleri yarım gün ders anlatan öğretmenlere göre daha yüksek bulundu (p<0,05). Tam ve yarım gün çalışan gruplar arasında kadın öğretmenlerde (p=0,171, p=0,604), ve erkek öğretmenlerde (p=0,152, p=0,89) %jitter ve %shimmer açısından anlamlı fark saptanmadı. Normal popülasyon ile yapılan karşılaştırmalarda ise hem kadın hem de erkek öğretmenlerin F0 değerleri kontrol grubuna kıyasla anlamlı olarak daha yüksek bulundu (p<0,05), %jitter ve %shimmer açısından anlamlı fark saptanmadı. Tam gün eğitim veren kadın ve erkek öğretmenlerin VHI puanları yarım gün derse giren hemcinslerinden anlamlı olarak yüksek bulundu (p<0,05). Hem kadın hemde erkek öğretmenlerin VHI puanları normal popülasyonuna göre anlamlı olarak daha yüksek bulundu (p<0,05).Anket cevaplarına göre; her eğitim grubunda ses problemi yaşanıyor olduğunu, süre bazında yıl olarak daha uzun meslekte olan değil, gün içerisinde saat olarak sesini çok kullanan öğretmenlerin daha yoğun ses sorunu yaşadığı, tam gün ders anlatan kadın ve erkek öğretmenlerin daha sık ve yoğun ses kısıklığı yaşadıkları, öğretmenlerin %95'inin farklı düzeyde ders esnasında yüksek ses kullanmak zorunda kaldıkları, gün sonunda ses yorgunluğu hissettiği %95 güven düzeyinde istatistiksel olarak tespit edilmiştir. Sonuç: Çalışmamızda, öğretmenlerin seslerini çok kullanmaları nedeniyle topluma göre daha çok temel frekansları normal dışı olarak saptanmış ve ses şikayetlerinindaha fazla olduğu görülmüştür ki bu veriler daha önce yapılan birçok çalışmayı desteklemektedir. Araştırmamız da özellikle günlük ders saatleri üzerinde durulmuş ve öğretmenlerin gün içinde çalışma süreleri arttıkça kendi meslek grupları içinde de temel frekanslarında normal dışı değerler ve ses problemlerinde artma olduğu ve süre olarak yıl bazında daha uzun süre çalışanların değil, gün içerisinde saat bazında daha fazla sesini kullananların daha yoğun ses sorunları ile karşılaştıkları görülmüştür.

Purpose and Scope: Voice is the primary means of work for teachers. The effect of teachers' daily lecturing periods on their speaking and vocal tracts is not clear. Using voice intensively is considered as the main risk of vocal disorders and it is an important issue for the professions that require intense voice usage. In this study; teachers' vocal behavioral analyses are done objectively by means of acoustic analysis MDPV parameters and subjectively by means of VHI scores and questionnaires, and the relationship between daily voice usage periods and perceived vocal flaws is investigated. Methods: As prospective randomized cross-sectional study; questionnaire, subjective voice handicap index (VHI), and voice measurements are done on the teachers working in the schools and private teaching institutions in Ankara, by using Electroglottography (Laryngograph, Micro Processor, EGG-D400, Serila Number: 10425, Wallington, UK) device and Laryngograph Speech Studio programme. Subjects' vocal cords are examined by anotorhinolaryngologist with VLS (Video-laryngo-stroboscopy). A comparison is done between the subjects and the population whodo not need to use their voice professionally (assessments that are done using the same methods synchronously). Sample population is composed of 30 teachers lecturing full-time (6-12 hours daily) and 30 teachers lecturing part-time (1-4 hours), and control group is composed of 60 people from other occupational group employees of sex and age matched. Fundamental frequency (F0), % Jitter and % Shimmer parameters of acoustic analysis MDPV programme that are obtained from the studyare compared with VHI scores and questionnaire data using statistical methods. Findings: According to acoustic analysis MDPV parameters; F0 scores of male and female teachers lecturing full-time are found higher than teachers lecturing part-time (p<0,05). Among groups working full-time and part-time, no significant difference is detected in terms of % Jitter and % Shimmer in female (p=0,171, p=0,604) and male teachers (p=0,152, p=0,089). In the comparison done with the normal population, F0 scores of both female and male teachers are found significantly higher compared to that of control group (p<0,05); no significant difference is detected in terms of % Jitter and % Shimmer. VHI scores of full-time lecturing female and male teachers are found significantly higher than their part-time lecturing fellows (p<0,05). According to questionnaire responses; it is determined statistically with %95 reliability that every training group is experiencing voice problems, not the teachers working longer on the basis of year but the teachers using their voice more intensely during the day are having more vocal problems, full-time lecturing male and female teachers are having more often and more intense hoarseness, %95 of the teachers have to speak louder in different levels during lecturing, and they are feeling vocal weakness at the end of the day. Results: In this study, it is determined that fundamental frequencies of teachers are abnormal compared to society and they have more vocal complaints, as they are using their voice more intensely. Those data is verifying many studies done before. In this research, particularly daily lecturing hours are considered and it is observed that the more the daily working hours of teachers increase, the more they have abnormal fundamental frequency scores and vocal problems, and also not the teachers working longer on the basis of year but the teachers using their voice more intensely during the day are having more vocal problems.
