Özet
Bu çalışmanın amacı, tam dişsiz hastaların, peri-implant dokularında protetik restorasyon öncesi (implantasyon sonrası 3.ay) değerlendirilen klinik parametreler ile PİOS'larında bulunan biyokimyasal belirteçlerin birbiriyle ilişkisini ve 6 ay sonraki seviyelerini belirlemek ve bu verileri peri-implant mukozitis grubundaki bireylerin klinik ve biyokimyasal verileri ile karşılaştırmaktır. Araştırmada 40 bireyde bulunan 80 implant iki ayrı grupta değerlendirilmiştir. İlk grupta 11 kadın, 9 erkek toplam 20 hasta; ikinci grupta ise 8 kadın, 12 erkek toplam 20 hasta üzerinde çalışılmıştır. Birinci gruptaki hastaların tümüne alt çene kanin dişleri hizasına iki adet implant uygulanmıştır. İmplantasyon sonrası 3. ayda peri-implant oluk sıvısı (PİOS) örnekleri toplanan tüm hastaların modifiye plak indeksi (MPİ), gingival indeks (Gİ), sondlamada kanama (SK), sondlama cep derinliği (SCD) ve keratinize dişeti miktarını (KDM) içeren klinik parametreleri de kaydedilmiştir. PİOS ve klinik parametreler, protetik restorasyondan 6 ay sonra (implantasyon sonrası 9.ay) tekrar değerlendirilmiştir. İkinci grupta bulunan peri-implant mukozitis teşhis edilmiş bireylerden de PİOS örnekleri toplanarak klinik parametreleri kaydedilmiştir. Her iki grupta toplanan PİOS örneklerinde, Enzime Bağlı İmmüno Sorbent Testi (ELISA) kullanılarak İnterlökin-1β (IL-1β), Tümör Nekrozis Faktör-α (TNF-α), Matriks Metalloproteinaz-8 (MMP-8) ve Aspartat Aminotransferaz (AST) seviyeleri değerlendirilmiştir. Elde edilen verilerin istatistik analizlerinin yapılmasında SPSS paket programı kullanılmıştır. Çalışmanın sonuçlarına göre, peri- implant mukozitis grubunda tespit edilen MPİ, Gİ, SK, SCD, PİOS hacmi, IL-1β ve TNF-α seviyeleri sağlıklı gruba göre istatistiksel olarak anlamlı şekilde yüksek bulunmuştur (p<0,05). MMP-8 ve AST değerlerinde iki grup arasında istatistiksel olarak anlamlı fark tespit edilmemiştir (p>0,05). KDM'nin ise peri-implant mukozitis grubunda sağlıklı gruba göre istatistiksel olarak anlamlı şekilde düşük olduğu tespit edilmiştir (p<0,05). Bu çalışmanın sınırları dahilinde, klinik parametrelere ek olarak PİOS içerisinde bulunan biyokimyasal belirteçlerin analizinin, peri-implant doku sağlığını değerlendirmede önemli bir yöntem olduğu ve hastalığın erken teşhisi açısından kullanılabileceği söylenebilir.

The aim of this study was determination of the relation between clinical parameters and biomarkers in peri-implant crevicular fluid (PICF) before (three months after implantation) and sixth months after prosthetic restorations in peri-implant tissues of totally edentulous patients and comparison with peri-implant mucositis patients. In this research 80 implants as two groups in 40 individuals were evaluated. In the first group there were 20 patients as 11 women and 9 men; in the second group there were 20 patients as 8 women and 12 men. In the first group all patients received two implants at the mandibular canine regions. 3 months after implantation, PICF samples were collected. Clinical parameters including modified plaque index (MPI), gingival index (GI), bleeding on probing (BOP), probing depth (PD) and keratinised tissue (KT) were recorded. 6 months after prosthetic restorations (nine months after implantation), PICF samples and clinical parameters were evaluated again. PICF samples and clinical parameters were analyzed in peri-implant mucositis patients too. In PICF samples which were collected from both groups, Interleukin-1β (IL-1β), Tumor Necrosis Factor-α (TNF-α), Matrix Metalloproteinase-8 (MMP-8) and Aspartat Aminotransferase (AST) were evaluated by Enzyme Linked Immuno Sorbent Assay (ELISA). In the statistical analysis, SPSS pocket program was used. Results of this study indicated that MPI, GI, BOP, PD, PICF volume, IL-1β and TNF-α levels were statistically higher in peri-implant mucositis group than the healthy group (p<0,05). There was no statistical difference in MMP-8 and AST values between the groups (p>0,05). Peri-implant mucositis group showed statistically lower KT scores than the healthy group (p<0,05). Within the limitations of this study, the analysis of PICF in addition to the clinical parameters is a useful method for evaluating the peri-implant tissue health and early diagnosis.
