Background It is thought that genetic predisposition to schizophrenia can also be expressed as liability to non-psychotic dysfunctions including cognitive impairment. Deficits of episodic memory and executive processing in schizophrenia are well documented and appear to manifest themselves also in some of the healthy biological relatives of schizophrenic patients. Method Episodic memory and executive processing were examined in 70 schizophrenic patients, 115 of their healthy first-degree relatives (including 10 presumed obligate carriers) and 66 normal controls. Of these probands, 31 had a manifest family history of schizophrenia (familial schizophrenic patients); the remainder 39 had no such family history (non-familial schizophrenic patients). Episodic memory was assessed by material-specific immediate/delayed recall and percentage retention. Executive functioning was measured by administering tests of planning ability, spatial working memory, strategy formation and shifting mental sets. Results Schizophrenic patients performed significantly worse than controls on nearly all variables considered. Relatives of schizophrenic patients performed significantly worse than controls in episodic memory. Further analyses of the relatives' performance based on family history of schizophrenia showed that those classified as non-familial relatives (relatives of non-familial schizophrenic patients, n = 55) had impairments in episodic memory, planning ability and strategy formation. Of these functions episodic memory was selectively impaired. Of the familial relatives (relatives of non-familial schizophrenic patients, n = 60), those parents who had themselves a parent and/or a sibling as well as an offspring that suffered from schizophrenia (presumed obligate carriers) also showed trends for attenuated performance compared to controls in visual memory and planning ability. Overall, deficits among relatives were subtle, all within one standard deviation of the control group. Conclusion Genetic liability to schizophrenia may be expressed as modality specific episodic memory and executive processing dysfunction.
