Özet
Karpal tünel sendromu (KTS) en sık görülen tuzak nöropatidir ve median sinirin el bileğinde karpal tunel içinde kompresyonu sonucu meydana gelir. KTS'li hastalardaki klasik belirti elde median sinir dağılım alanında, özellikle geceleri ortaya çıkan parestezi (uyuşma, yanma, karıncalanma) ve ağrıdır. Hafif-orta düzeyde KTS'nin yönetiminde akupunkturun etkinliği, çelişkili sonuçları olan sınırlı çalışmalarda araştırılmıştır. Bu çalışmanın amacı, hafif-orta düzeyde karpal tünel sendromunun tedavisinde akupunkturun etkilerini değerlendirmektir. Bu çalışmada klinik ve elektromiyografi (EMG) ile hafif-orta düzeyde KTS saptanan hastalara ilk 2 hafta haftada 3 seans, sonraki 2 hafta haftada 2 seans ve her seans 30'ar dakika olmak üzere toplam 10 seans akupuntur uygulaması yapıldı. Akupunktur tedavisisinde pericardium (PC)-3, PC-6, PC-7 noktaları kullanılarak kuru iğneleme yapıldı. Hastalar tedavi öncesinde, tedavi sonrası birinci ve üçüncü ayında elektronörofizyolojik parametreler, parestezik şikayetler (uyuşma, yanma, karıncalanma), ağrı semptom varlığı, Visuel Analog Skala (VAS) ve Boston Skalası (semptom şiddeti ve fonksiyonel kapasite ), Tinnel ve Phalen testi ile değerlendirildi. Katılım kriterlerini karşılayan 40 hastadan, 37'si çalışmayı tamamlayabildi ve sonuç için değerlendirildi. Tedavi sonrası, tedavi öncesine göre uyuşma ve karıncalanma semptomlarında, Phalen test pozitifliğinde istatistiksel olarak anlamlı düzelmeler mevcuttu (p<0,05). Akupunktur tedavi öncesine göre tedavi sonrası VAS skoru ve Boston semptom ölçeğinde 1. ve 3.ayda, Boston fonksiyonel kapasite skalasında ise tedavi sonrası 1.ayda ileri derecede anlamlı olarak iyileşmeler mevcuttu (p<0,01). Elektrofizyolojik parametreler arasında, median sinir distal motor latansı tedavi öncesine göre tedavi sonrası 3.ayda anlamlı şekilde düzelmişti (p=0,01). VAS skoru ve Boston fonksiyonel kapasite skalasında tedavi sonrası 3.ayda, tedavi sonrası 1.aya göre anlamlı olmayan artış saptanmıştı (p>0,05). Sonuç olarak, hafif-orta şiddetli KTS'de PC-3, PC-6, PC-7 noktaları kullanılarak yapılan akupunktur tedavisinin; elektrofizyolojık bulgulara göre klinik bulgulardaki iyileşmede etkisi daha belirgindi.

The Effect of Acupuncture Treatment on the Clinical and Electrophysiological Findings in Carpal Tunnel Syndrome Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy and occurs as a result of the compression of median nerve in wrist. The common symptoms in patients with CTS are pain and paraesthesia (numbness, burning and tingling) in the distribution of the median nerve, especially at nights. In the management of mild to moderate CTS, the effectiveness of acupuncture has been analyzed in limited number of researches having conflicting results. The purpose of this study was to evaluate the effects of acupuncture on the treatment of mild to moderate CTS. In this study, ten sessions (3 sessions in the first two weeks, 2 sessions in the latter two weeks; each lasting 30 minutes) were applied to the patients diagnosed as mild to moderate CTS both clinically and with electromyography (EMG). For acupuncture treatment, dry needling was done by using pericardium (PC)-3, PC-6 and PC-7 points. The patients were evaluated with electrophysiological parameters, paraesthesia complaints (numbness, burning and tingling), the presence of pain symptoms, Visuel Analog Scale (VAS) and Boston Scale (symptom severity and functional capacity), Tinnel and Phalen test before, one and three months after the treatment. Among 40 patients, 37 of whom met the participation criteria, completed the study and were evaluated for the results. In the companion of symptoms before and after treatment, it was seen that there was statistically significant recovery in the Phalen test positivity (p<0.05). In the first and third months, it was observed that there was advanced and significant recovery in Boston functional capacity scale in the first month (p<0.01). It was also determined that median nerve distal motor latency had better outcome in the 3rd month compared to before treatment in terms of electrophysiological parameters (p=0.01). It was determined that there was an insignificant increase in the 3rd month of treatment when compared to the 1st month of after treatment with respect to VAS score and Boston functional capacity scale (p>0.05). As a result, it was determined that the effect of the acupuncture treatment by using PC-3, PC-6 and PC-7 points in mild to moderate CTS was more prominently effective in the clinical findings rather than electrophysiological findings.
