Özet
Birey haklarının öncelenmesine yönelik değersel değişim, yaşam süresinin uzaması, sağlık hizmetlerinde maliyet artışları, toplumda yaşanan sosyoekonomik değişimler gibi birçok etken alternatif ve tamamlayıcı olarak profesyonel sunulan evde bakım hizmetlerine olan talebi artırmıştır. Bu çalışmanın amacı; Türkiye'nin hukuki, ekonomik, kültürel yapısı ve sağlık örgütlenme modeline uygun bir evde bakım hizmeti modeli önerisi geliştirmektir. Bu kapsamda evde bakım sistemini bakım sigortası ile bütünleştiren ve Türk kökenli kişilerin Türkiye dışında en fazla yaşadığı ülke olan Almanya örneği incelenmiştir. Araştırma nitel araştırma yapısı ile Almanya ve Türkiye'de evde bakım hizmetleri sunumunun farklı kademelerinde yönetici konumunda yer alan kişilerle yarı yapılandırılmış görüşme formu kullanılarak gerçekleştirilmiştir. Almanya'daki görüşmeciler, evde bakım hizmetlerinin sunumunda etik dışı uygulamalara yol açan yoğun bir rekabetin yaşandığını, bakım planlarının esneklikten uzak ve zaman baskısı ile sıkıştırılmış olduğunu, sigorta şirketlerinin maliyetleri düşürmek odaklı yaklaşım içerisinde olduklarını, denetimlerin daha çok hizmetin uygulanmasına yönelik olmasından öte bürokratik işlemlere odaklandığını belirtmişlerdir. Türkiye'deki görüşmecilerin ifadeleri değerlendirildiğinde, evde bakım hizmetlerinin tanımı, kapsamı gibi birçok konuda benzerlik olmadığı saptanmıştır. Türkiye'de kapsayıcı nitelikte tek bir mevzuatın olmaması, sağlık ve sosyal bakım uygulamalarının farklı kurumlar tarafından yönetilmesi, evde bakım hizmetlerinin özel sektördeki sunumunda yaşanan yasadışı çalışma durumunun sistem içinde haksız rekabete neden olması temel sorunlar olarak belirtilmiştir. Türkiye için önerilen modelde güçler ayrılığı temelinde hizmeti sunan ve yöneten kurumların birbirlerinden farklı olması esas alınmıştır. Evde bakım hizmet sunumunun kamu, özel ve kâr amacı gütmeyen sivil toplum örgütleri tarafından, üçlü bir yapı ile sağlanması önerilmektedir.

Change in approach prioritized the individual rights, increasing the life expectancy, increasing the costs of health services, change in socioeconomic conditions in society leads to demand growth in home care services provided professionally as an alternative and complementary. The aim of this study is to develop proposal for home care service model applicable to legal, cultural, economic structure and health organization model of Turkey. In this extent, Germany as a representative country whose care home care system is integrated with the care insurance and where the people with Turkish roots live mostly in Germany after Turkey was investigated. This study with qualitative research structure was carried through using semi-structured questionnaire forms conducted to the personel working at different levels of management in home care service providers from Turkey and Germany. The interviewers from Germany states that there is very intensive competiton that leads to unethical applications during service provision, the care plans are far away from the flexibility, the insurance institutions only focus on the approaches to decrease in costs of expenditure, the audits on home care institutions are mostly based on the bruocratic procederus rather than how the care services are provided. When considering the statements of interviewers from Turkey, it was found that there is no similarity in most of the issues like definition and scope of home care services. Deficiency in legal regulation with an integrated approach to the home care services, management of health and social care applications by different govermental institutions, illegal working situations in private sector leading to unfair competition during home care sevice provision are stated as the main problems faced with in this sector. Proposed model developed through this study is based on the principle of division of powers, which focus on the fact that the institution of service provider and service manager will be different from each other. It is proposed that home care services should be provided by mainly three structures including public institution, private institution and non-profit organizations
