Kasnak,G. (2012). Periodontitisli Bireylerin Dişeti Oluğu Sıvısındaki Matriks Metalloproteinaz ve Metalloproteinaz İnhibitor Enzimlerinin Etkisi ve Genetik Polimorfizmleri ile İlişkisi. İstanbul Üniversitesi Sağlık Bilimleri Enstitüsü, Periodontoloji ABD Doktora Tezi. İstanbul.Çalışmamızda, generalize agresif periodontitisli (GAP), lokalize agresif periodontitisli (LAP) ve genç sağlıklı bireylerde (GS) MMP-1 promoter gen polimorfizminin incelenmesi, bu polimorfizmin fenotipe yansıması ve diş eti oluğu sıvısı (DOS) ile genotipin ilişkisinin değerlendirilmesi amaçlanmıştır. Bu amaç çerçevesinde çalışmamıza 28 GAP'li, 30 LAP'li ve 32 GS'lı birey olmak üzere toplam 90 birey katılmıştır. Çalışmamıza katılan bireylerin periferik kan örneklerindengenomik DNA elde edilmiş olup, MMP-1 -519A/G, -1607 1G/2G polimorfizmleri standart PCR-RFLP yöntemi ile incelenmiştir. Bireylerden alınan DOS örneklerinde MMP-1 enzim düzeyleri ELISA yöntemi ile belirlenmiştir.GAP'li ve LAP'li bireylerde göre GS'lı bireylere göre MMP-1 -1607 1G/2G ve MMP-1 -519 A/G promoter gen polimorfizmi görülme sıklığının anlamlı derecede yüksek olduğu, bu anlamlı farktan kaynaklanan DOS sıvısındaki MMP-1 enzim konsantrasyonunun; GAP'li ve LAP'li bireylerin toplamından oluşan agresif periodontitisli (AP) bireylerde, GS'lı bireylere oranla daha yüksek olduğu bulunmuştur.Çalışmamızın verileri değerlendirildiğinde, Türk toplumunda yer alan AP'li bireylerin diş eti oluğu sıvısındaki MMP-1 enzim konsantrasyonunun, GS'lı bireylere oranla daha fazla bulunması, MMP-1 enzmini kodlayan genin -1607 ve -519 lokusunda meydana gelen promoter polimorfizm ile açıklanabilir. Bireylerin AP'e yatkınlığı, DOS'ta ki MMP konsantrasyonlarına bakılarak değerlendirilebilmesi ileride yapılacak kapsamlı çalışmaların desteği ile düşünülebilinir.

İstanbul.The first aim of study was to investigate the MMP-1 promoter gene polymorphsim in generalized aggressive periodontitis (GAgP), localized aggressive periodontitis (LAgP) and young healthy subjects (YHS). The second aim was to investigate the effect of MMP-1 -1607 1G/2G and MMP-1 -519 A/G promoter gene polymorhism on the MMP-1 enzyme levels in gingival crevicular fluid (GCF) of GAgP, LAgP, and YHS. For this purpose 28 GAgP, 30 LAgP in the sum of 58 aggressive periodontitis (AP) and 32 YHS were included to our study. Peripheric blood samples were taken from all the subjects for DNA extraction and by the help of PCR-RFLP technique; MMP-1 -1607 1A/2G and MMP-1 -519 A/G promoter gene polymorphisms were determined. GCF samples were collected by using strile perio papers and analyzed with ELISA.The frequency of MMP-1 -1607 1G/2G and MMP-1 -519 A/G promoter gene polymorphism was significantly higher in the GAgP, LAgP compared to the YHS group. As a result of this, as we expected, MMP-1 enzyme levels in the GCF of AP group were significantly higher than the YHS group.According the data of our study we could suggest that; the higher levels of MMP-1 enzyme in GAgP and LAgP groups might be associated with the MMP-1 -1607 1G/2G and MMP-1 -519 A/G promoter gene polymorphism. By the help of further investigation in the Turkish population, the relationship between the MMP-1 promoter gene polymorphism and AP might be determined.
