Özet
AMAÇ: Lenfomalarda relaps hastalık; primer tedavi tamamlandıktan sonra tam remisyon elde edilen hastalarda hastalığın ilerlemesidir. Refrakter hastalık ise; primer tedavi sırasında hastalığın ilerlemesi veya tedaviye rağmen değişiklik olmamasıdır. Bu çalışmada relaps/refrakter lenfoma hastalarında, birinci ve ikinci sıra kurtarma kemoterapi rejimlerinin retrospektif değerlendirmesi ve olguların tedavi yanıtlarına, tedavi bağlı gelişen toksisite, progresyonsuz ve genel sağkalım sürelerine bakılması amaçlanmıştır. GEREÇ ve YÖNTEM: Bu çalışmaya Haziran 2005-Haziran 2012 Dokuz Eylül Üniversitesi Tıp Fakültesi Hastanesi Hematoloji Bilim Dalında takip edilen kurtarma kemoterapi rejimleri alan 56 relaps/refrakter lenfoma hastaları çalışmaya alındı. BULGULAR: Hastaların % 58,9 relaps hastalık, % 41,07 refrakter hastalıktı. Hastaların ortalama yaşı 45,5 (17-72) idi. Bu hastalardan tanılar açısından sınıflandırıldığında hastaların %44,6'sı klasik hodgkin lenfoma,%55,4 Non-Hodgkin Lenfoma tanılıydı. Hastaların 19 (%33,9)'u kadın, 37 (%66,1)'u erkekti. Hastaların %3,6'sının evre 1, %12,5'inin evre 2, %25'inin evre 3 ve %58,9'u evre 4 hastalıktı. %58,9'unda B semptomları, %16,1'inde de Bulky hastalık, 17 hasta IPI skoru ≤3 ve 39 hasta IPI skoru ≥3 bulguları görüldü. Olguların %76,8'inde erken relaps olduğu , %23,2'sinde geç relaps olduğu saptandı. Birinci sıra kurtarma tedavileri incelendiğinde hastaların %85,7'sinde ESHAP aldığı görüldü. 44 Hastaya OHKHN planlandı. 24 hastaya (%54,54) OHKHN başarılı uygulandı. OHKHN uygulanan 24 hastanın 6'sı ikinci sıra kurtarma tedavisi aldı, 8 hasta ex oldu, 14 hasta remisyondaydı. Birinci sıra kurtarma rejimlerine toplam yanıt %41,1'idi (TY %16,1 / KY %25). %25 hastada stabil hastalık ve %33,9 hastada da ilerleyen hastalık bulgularının mevcut olduğu görüldü. Genel sağkalım süresi(GSS) 59,9(±9,007)(p<0,0001) ay'dı. 18 hastaya ikinci sıra kurtarma rejimi verildi (%50'si gemsitabin-vinorelbine, %27,8'i IGEV veya IGEV-Bortezomib, %11,1'i Bendamustine, %5,6'sı hyperCVAD, %5,6'sı ESHAP). İkinci sıra kurtarma rejimleri değerlendirildiğinde %27,7 hastada toplam yanıt, %38,9 hastada stabil hastalık ve %33,3 hastada da ilerleyici hastalık bulguları saptandı, 9 (%50) hastada ölüm görüldü. SONUÇ: Retrospektif ve hasta sayısının kısmen az olmasına karşın çalışmamızda literatüre paralel şekilde relaps/ refrakter lenfoma hastalarında kurtarma rejimleri ile önemli oranda hastada yarar görülebildiği ve bu hastaların bir kısmında da önemli bir tedavi alternatifi olarak OKHN'e olanak sağladığı saptanmıştır.

OBJECTIVE: Relapsing disease means recurrence of a disease, after achieving complete remission at the end of the treatment. Therefore progression of a disease during primary treatment is called as a refractory disease. In our study , we aim to investigate, retrospectively, the toxicity, progression-free and overall survival period of relapsed /refractory lymphoma patients who have been treated with first and second line salvage chemotherapy regimes . MATERIALS AND METHODS: Our study included 56 relaps/refractory lymphoma patiens who have been treated with salvage chemoterapy regimens between June 2005-June 2012 in Dokuz Eylul University Faculty of Medicine Hospital Department of Hematology RESULTS: In our study; 58.9 % of study population was patients with relapsed disease, on the other hand 41.07 % of them had refractory disease . Mean age was 45.5 ( 17-72 ), respectively. 44.6 % of the patients were diagnosed as Hodgkin lymphoma , 55.4 % were diagnosed with Non-Hodgkin 's Lymphoma . 19 patients (33.9% ) were female , 37 (66.1% ) were male . 3.6% of study population had stage 1 disease , 12.5% had stage 2, 25% stage 3 and 58.9% had stage 4 disease. In addition 58,9% of patiens had shown B symptoms, and 16.1% of patients has been diagnosed with Bulky disease. In 17 patients IPI score were ≤ 3 and in 39 patients IPI score were ≥ 3. We have found out that the 76.8 % of the total study population showed early relapse and the other 23.2% were diagnosed with late relapse. When we examine the first line salvage treatment, we have found out that 85.7% of patients had been treated with ESHAP. In total; AHSCT was planned for 44 patients, the procees had been successful in 24 patients ( 54.54 % ) . 6 of 24 Patients whom underwent successful AHSCT received second line salvage chemoterapy, 8 of them died and 14 of them went to remission. The total response to the first line recovery regime was 41.1 % (CR 16.1 % / PR 25%). However 25% of patients still had unvarying disease and in 33.9% of patients were diagnosed with progressive disease findings. Overall survival rate was (GSS ) 59.9 ( ± 9.007 ) ( p <0.0001 ) months. The second line was given to 18 patients as recovery regimen (50% of gemcitabine - vinorelbine, 27.8% IGEV or IGEV - Bortezomib, 11.1% Bendamustine, 5.6% of the hyperCVAD, 5.6% of ESHAP). When we evaluated the second line rescue regimens we found out that 27.7 % of total patient population shown total response, in 38.9% were unvarying disease, 33.3 % of the patients showed evidence of progressive disease and in addition 9 of 18 who had received second line recovery chemoterapy (50%) died . CONCLUSION: Although our study was retrospective and we had insufficient number of patients, our study was correlated with the current literature based on relapsed / refractory lymphoma patients. Finally we have shown that salvage chemoterapy regimens were beneficial in relapse/refractory lymphoma patients and give a opportunity for another possible and successful treatment method known as AHSCT .
