Özet

Amaç: Onkolojik ilkelerin uygulanabilirliğine dair kaygılar nedeniyle rektum cerrahisinde laparoskopinin yeri ve kullanımı halen tüm dünyada tartışmalıdır. Bu çalışmada kliniğimizde 2005-2011 yılları arasında laparoskopik kolorektal cerrahi öğrenim eğrisinin tamamlanmasıyla birlikte gerçekleştirilen laparoskopik ve açık rektum rezeksiyonlarının erken ve geç dönem sonuçlarının karşılaştırılması amaçlanmıştır.Yöntem ve gereç: Çalışmamız 2012/611-1025 sayılı etik kurul onayı alınarak, Ocak 2005 ile Aralık 2011 tarihleri arasında rektum kanseri tanısı ile laparoskopik veya açık cerrahi uygulanan ardışık 587 hasta cinsiyet, yaş, vücut kitle indeksi (VKİ), Amerikan Anestezistler Derneği ameliyat öncesi risk skorlaması (ASA), tümör yerleşimi, uygulanan cerrahi tipi, anastomoz tekniği, ameliyat süresi, ameliyat sonrası erken dönem klinik sonuçlar, histopatolojik veriler, lokal nüks, uzak metastaz ve sağkalım değişkenleri prospektif olarak oluşturulan veri bankasının retrospektif olarak değerlendirilmesi ile oluşturulmuştur.Bulgular: Laparoskopik cerrahi grubunda ki hastaların 250'si (%57,2) erkek, 187'si (%42,8) kadın, açık cerrahi grubunda ki hastaların 89'u (%59,3) erkek, 61'i (%40,7) kadındır (p=0,64). Laparoskopik cerrahi grubunda median yaş 58 (19-91) yıl, açık cerrahi grubunda 57 (20-87) yıl olarak saptanmıştır (p=0,9). VKİ ve ASA değerleri, laparoskopik ve açık cerrahi grubunda istatistiksel olarak benzer bulunmuştur (26,0±3,6 kg/m2, 2,2±0,8 (laparoskopik) ve 26,4±2,5 kg/m2, 2,0±0,9 (açık)) (p=0,98). Her iki grupta tümörlerin çoğunluğu distal yerleşimli olup, laparoskopik cerrahi grubunun %55,1`ini (n=241), açık cerrahi grubun %46,7'sini (n=70) oluşturmaktadır (p=0,08). Hastaların 437'sine (%74,4), laparoskopik cerrahi teknik, kalan hastalara açık cerrahi teknik uygulanmıştır (n=150, %25,6). Sfinkter koruyucu cerrahi oranı laparoskopik cerrahi grubunda %74,6 (n=326), açık cerrahi grubunda %72 (n=108) olarak saptanmıştır (p=0,34). Laparoskopik cerrahi grubunda ortalama ameliyat süresi 154,5±52,1 dk, açık cerrahi grubunda ortalama 172,6±33,3 dk olarak belirlenmiştir (p<0,001). Laparoskopik cerrahi grubunda %7,8 (n=34) oranında açık cerrahi tekniğe dönüş gerçekleşmiştir. Seride ameliyat sonrası erken dönemde sadece 1 (%0,2) mortalite görülmüştür. Laparoskopik cerrahi grubunda genel morbidite oranı %33,6 (n=148) iken bu oran açık cerrahi grubunda %42'dir (n=64) (p<0,001). Laparoskopik cerrahi grubunda gaz pasajı (anal veya stomadan), gaita pasajı (anal veya stomadan), katı gıda başlama ve hastanede kalış zamanı, açık gruba göre daha kısa bulunmuş olup bu durum istatiksel olarak anlamlıdır (1,9±1,2 gün, 2,6±1,7 gün, 3,0±2,3 gün, 8,9±3,3 gün (laparoskopik), 2,6±1,4 gün, 3,1±1,3 gün, 3,8±1,7 gün, 10,4±5,6 gün (açık)) (p<0,001, p<0,001, p<0,001, p=0,028). Her iki grupta ortalama piyes uzunluğu, diseke edilen lenf nodu sayısı ve distal cerrahi sınır benzer bulunmuştur (264,8±111,9 mm, 24,3±13,2, 32,3±24,6 mm (laparoskopik), 271,5±137,9 mm, 21,9±14,6, 35,8±68,9 mm (açık)), (p=0,17, p=0,065, p=0,54). Distal cerrahi sınır pozitifliği, laparoskopik cerrahi grubunda 5 (%1,1), açık cerrahi grubunda 5 (%3,3) olguda mevcuttur (p=0,12). Çevresel cerrahi sınır (ÇCS) pozitifliği, laparoskopik cerrahi grubunda 69 (%15,8), açık cerrahi grubunda 15 (%10) olguda saptanmıştır (p=0,2). Laparoskopik cerrahi grubunda %73,5 (n=270), açık cerrahi grubunda %66,6 (n=54) oranında tam ve tama yakın mezorektum bütünlüğü saptanmıştır (p=0,2). Çalışmamızda ortalama izlem süresi 45,5±25,9 ay (6-88 ay), takip oranı %94,5 (n=555)'tir. Laparoskopik cerrahi grubunda toplamda 96 (%22) hastada tümör nüksü saptanmış olup; izole sistemik nüks 69 (%15,8), lokal nüks 27 (%6,2), hem lokal hem sistemik nüks 17 (%3,9) hastada görülmüştür. Açık cerrahi grubunda toplamda 52 (%34,6) hastada tümör nüksü saptanmış olup; izole sistemik nüks 35 (%23,3), lokal nüks 17 (%11,3), hem lokal hem sistemik nüks 8 (%5,3) hastada görülmüştür. Lokal nüks olgularının 27'si (%6,1) laparoskopik cerrahi grupta, 17'si (%11,3) açık cerrahi grupta bulunmaktadır (p=0,045). Beş yıllık genel ve hastalıksız sağkalım laparoskopik cerrahi grubu lehine yüksek bulunmuştur (%85, %78,7 (laparoskopik), %71,5, %68,5 (açık)), (p=0,001). Laparoskopik cerrahi grubunda 5 yıllık genel sağkalım evre 0'da %100, evre 1'de %95,9, evre 2'de %89,8, evre 3'de %77,1, evre 4'de %39,6, açık cerrahi grubunda evre 0'da %100, evre 1'de %80, evre 2'de %87,5, evre 3'de %51,8, evre 4'de % 36,4 oranında saptanmıştır (p=0,001).Sonuç: Laparoskopik rektum kanseri cerrahisi yüksek hacimli kurumlarda ve yüksek hacimli cerrahlar tarafından gerçekleştirildiği takdirde erken ve geç klinik sonuçlar, onkolojik veriler ve sağkalım açısından açık cerrahiye göre avantaj sağlamaktadır. Bu nedenle laparoskopik yöntem, rektum kanser cerrahisinde altın standart olma yönünde ilerlemektedir. Rektum kanseri cerrahisinde laparoskopik tekniğin kullanımının rutin güncel pratiğe geçilebilme kararı için halen yürütülmekte ve prospektif, randomize çalışmalar olan Avrupa'dan Color II, Japonya'dan JCOG 0404 ve Amerika'dan ACOSOG Z6051 sonuçlarının beklenmesi gerekmektedir.

Aim: Due to the concern regarding the feasibility of oncologic principles, the use of minimal invasive techniques in rectum surgery is still a debate through out the world. The aim of this study is to compare the short and long term results of laparoscopic and open rectum resections, which are performed in our clinic between the time period of 2005 and 2011.Method and material: Our study has been approved by ethical committee number 2012/611-1025 and 587 patients, treated with laparoscopic or open surgery with rectum cancer diagnosis. These technics have been compared by retrospective evaluation of the data bank, which includes gender, age, body mass index (BMI), American Anesthetists Association preoperative risk scoring (ASA), tumoral lesion, type of surgery, anastomose technique, operation time, postoperative short term clinical results, histopathological data, local recurrence, distant metastasis and survival variables.Findings: Two hundred and fifty (57,2%) of the patients were male, 187 (42,8%) of the patients were female in laparoscopic surgery group and 89 (59,3%) of the patients were male, 61 (40,7%) of the patients were female in open surgery group (p=0,64).Median age was 58 (range:19?91) in laparoscopic surgery group and 57 (range:20?87) in open surgery group (p=0,9). BMI and ASA were statistically found similar in both laparoscopic and open surgery groups (26,0±3,6 kg/m2, 2,2±0,8 (laparoscopic) and 26,4±2,5 kg/m2, 2,0±0,9 (open)) (p=0,98). In both two group, the majority of the tumors were distal located and constitute 55,1% (n=241) of the laparoscopic surgery group and 46,7% (n=70) of the open surgery group (p=0,08). 437 patients (74,4%) were treated with laparoscopic technique and rest of them were treated with open technique (n=150, 25,6%). Sphincter preserving surgery rate was determined as 74,6% (n=326) in laparoscopic surgery group and as 72% (n=108) in open surgery group (p=0,34). Average operation time was recorded as 154,5±52,1 minutes in laparoscopic surgery group and as 172,6±33,3 minutes in open surgery group (p<0,001). In 7,8% (n=34) of the laparoscopic surgeries, conversion to open surgery was necessery. Just 1 mortality (0,2%) was recorded in postoperative early period. While general morbidity rate was 33,6% (n=148) in laparoscopic surgery group, this rate was 42% (n=64) in open surgery group (p<0,001). In laparoscopic surgery group, the starting time for solid food consuming, gas pass (anal or from stoma), fecal pass (anal or from stoma) and hospitalization time was found to be shorter than the open surgery group and this is statistically significant (1,9±1,2 days, 2,6±1,7 days, 3,0±2,3 days, 8,9±3,3 days (laparoscopic), 2,6±1,4 days, 3,1±1,3 days, 3,8±1,7 days, 10,4±5,6 days (open) respectively) (p<0,001, p<0,001, p<0,001, p=0,028). In both groups, average specimen length, number of harvested lymph nodes and distal surgery limits were found to be similar (264,8±111,9 mm, 24,3±13,2, 32,3±24,6 mm (laparoscopic), 271,5±137,9 mm, 21,9±14,6, 35,8±68,9 mm (open)), (p=0,17, p=0,065, p=0,54 respectively). Distal margine positivity was found in 5 cases (1,1%) in laparoscopic surgery group and in 5 cases (3,3%) in open surgery group (p=0,12). Circumferantial resection margine positivity was recorded in 69 cases (15,8%) in laparoscopic surgery group and in 15 cases (10%) in open surgery group (p=0,2). 73.5% (n=270) total and nearly total mesorectum integrity was determined in laparoscopic surgery group and this rate was 66,6% (n=54) in open surgery group (p=0,2).Average follow-up time is 45,5 ± 25,9 months (6-88 months), follow-up rate is 94,5% (n=555) in our study. Tumor recurrence was determined in 96 patients (22%) in laparoscopic surgery group; isolated systemic recurrence was observed in 69 patients (15,8%), local recurrence was observed in 27 patients (6,2%), both local and systemic recurrence were observed in 17 patients (3,9%). Tumor recurrence was determined in 52 patients (34,6%) in open surgery group; isolated systemic recurrence was observed in 35 patients (23,3%), local recurrence was observed in 17 patients (11,3%), both local and systemic recurrence were observed in 8 patients (5,3%). 27 local recurrence (6,1%) was recorded in laparoscopic group and 17 (11,3%) was recorded in open group (p=0,045). Overall and disease-free survival rates were found to be higher in laparoscopic surgery group (85%, 78,7% (laparoscopic), 71,5%, 68,5% (open)), (p=0,001). Overall survival rates in laparoscopic surgery group was 100 in stage 0, 95,9% in stage 1, 89,8% in stage 2, 77,1% in stage 3, 39,6% in stage 4, in open surgery group, it was 100% in stage 0, 80% in stage 1, 87,5% in stage 2, 51,8% in stage 3 and 36,4% in stage 4 (p=0,001).Result: If it is practiced in high-volume institutions by high-volume surgeons, laparoscopic rectum cancer surgery is more advantageous in terms of short and long term clinical results, oncologic data and survival. Thus, laparoscopic method is on the way to become a golden standard in rectum cancer surgery. In order to put the laparoscopic technique into routine practice in rectum cancer surgery, the results of Color II study from Europe, JCOG 0404 study from Japan and ACOSOG Z6051 prospective study from the USA must be waited.
