9) ÖZET Kliniğimizde açık kalp yöntemi ile değişik pozisyonlarda aynı türden yapay kalp kapağı replasmanı yapılan 20 hastada post-operatif dönemdeki eritrosit zarı Adenozin 5' Trifosfataz enzim aktiviteleri ve yine aynı hastalarda post-operatif dönemde gelişen hemoliz incelendi. Bulunan değerler bilinen bir hastalığı olmayan 10 kişiden alınan değerlerle karşılaştırıldı. Hasta gurubunda pre-operatif dönemde hemoliz olmadığı, post-operatif 4.ayda hafif hemoliz geliştiği, post-operatif 8. ayda da yine hafif hemoliz'in devam ettiği görüldü. Aynı dönemlerde hasta gurubunun ATP az değerleri incelendiğinde Pre operatif dönemde hasta gurubunda hemoliz oluşmamasına rağmen her üç ATPaz enzim aktivitesinde kontrol grubuna göre önemli ölçüde azalma olduğu, post operatif 4.ayda hasta gurubunda hafif hemoliz görülmekle beraber yine ATP az enzim aktivitelerinde azalmanın devam ettiği görüldü. Post-operatif 8 ayda hasta gurubunun hemoliz şiddeti aynı kalırken ATP az enzim aktivitelerinde yine anlamlı azalmanın devam ettiği saptandı. Bulgularımız bize kapak replasmanı yapılan olgularda ATP az enzimlerinin aktivitelerinde azalma olduğunu ancak bu azalmanın doğrudan hemolize bağlı olarak ortaya çıkmadığını, başka faktörlerinde bu aktivite kaybında etkili olabileceğini düşündürdü. 45

SUMMARY Changes of the erythrocyte membrane Adenosine-5'Triphosphatase activities in the prosthetic heart valve replased patients at post-operative period and interrelations of these changes with hemolysis. In this study, erythrocyte membrane ATP ase enzyme activity changes and also hemolysis developed in the post-operative period by replacing the same type prosthetic heart valves were determined in twenty patients. The findings were compared with the values obtained from ten healty persons. The hemolysis was not observed in the pre-operative period of the patient group; but there were mild hemolysis in the fourth and eighth mounths of the post-operative period. Although there were no hemolysis occured en the patient group at pre-operative period, a significant decrease were observed in each of three ATPase enzyme activities as compared with the control. At the fourth mounth of post-operative period, a mild hemolysis was also determined and ATPase enzyme activities were still continuing to decrease. While the degree of hemolysis remained the same at the eighth mounth of post-operative period in the patient group, meaningfull decrease in the ATPase enzyme activities were still being observed. Results of this study were indicated that ATPase enzyme activities were decrease in patients but these decreases were not directly related with hemolysis. Some other factors may contribute for the loss of enzyme activities. 46
