Özet

Amaç: nmeli hastalarda hemiplejik üst ekstremitede aktif nöromuskülerstimülasyonun motor ve fonksiyonel performans üzerine etkilerini araştırmak.Gereç ve yöntem: Çalışmaya, gönüllü 31 inme hastası alındı. Hastalarrandomize olarak üç gruba ayrıldı: Aktif NMES (n=11), Pasif NMES (n=10) veplasebo-stimülasyon grubu (n=10). Her bir gruba, haftada 5 gün, günde 45dakika olacak şekilde 15 seans tedavi uygulandı. Ek olarak tüm hastalaranörofizyolojik egzersiz programı uygulandı. Fugl Meyer Motor FonksiyonSkalası (FM), Fonksiyonel Bağımsızlık Ölçeği (FIM) kendine bakım kısmı,Motor Activity Log (MAL), el bileği ve metakarpofalangeal eklem ekstansiyonu,Modifiye Ashworth Skalası (MAS), yüzeyel EMG potansiyeli ve kavrama gücüdeğerlendirmeleri tedaviden önce ve tedavi bitiminde tedavi türüne kör olanbir hekim tarafından yapıldı.Bulgular: Aktif NMES ile tedavi edilen hastalarda, kontrol grubuna göreFM motor fonksiyon, FIM ve MAL miktar skorları, aktif eklem hareket açıklığı,kavrama gücü ve yüzeyel EMG potansiyeli artışı istatistiksel olarak dahafazlaydı (p<0.05). Pasif NMES ile tedavi edilen hastalarda konrol grubu ilekarşılaştırıldığında, FM motor fonksiyon, FIM skorları ve eklem hareketaçıklığı artışı istatistiksel olarak fazlaydı (p<0.05). Aktif NMES tedavisi alanhastalar, Pasif NMES grubuna göre kavrama gücü ve yüzeyel EMGpotansiyeli artışı bakımından daha kazançlı bulundu (p<0.05).Sonuç: Hem aktif hem de pasif NMES, inmeli hastalarda üstekstremitede motor ve fonksiyonel performansın arttırılmasında etkilidir. AktifNMES el bileği ekstansörleri yüzeyel EMG potansiyellerinin ve kavramagücünün arttırılmasında pasif NMES'den daha etkindir.

Purpose: The purpose of this study was to assess the efficacy of activeneuromuscular stimulation in enhancing the upper extremity motor andfunctional recovery of stroke survivors.Methods: Thirty-one stroke survivors volunteered to participate in thisstudy. Subjects were randomly assigned to 1 of 3 groups: Active NMES group(n=11), Passive NMES group (n=10), control group (n=10). Each treatmentregimen was applied five times weekly, with a duration of 45 min for 15treatment sessions. In addition, all patients received an exercise programaccording to the neurophysiologic approach. Outcomes were assessed in ablinded manner with the upper extremity component of the Fugl-Meyer MotorAssessment (FMA), the self-care component of the Functional IndependenceMeasure (FIM), Motor Activity Log (MAL), goniometric measurements for wristand metacarpophalangeal joint extension, surface EMG potentials, gripstrength, Modified Ashworth Scale (MAS) at study entry and at discharge.Results: Subjects treated with active NMES exhibited statisticallysignificant greater gains in FMA, FIM, MAL amount scores and theimprovements in active range of motion, grip strength and surface EMGpotentials compared with controls (p<0.05). Subjects treated with passiveNMES exhibited statistically significant greater gains in FMA, FIM scores andthe improvements in wrist active range of motion compared with controls(p<0.05). Subjects treated with active NMES exhibited significantly greatergains grip strength and surface EMG potentials compared with passive NMESgroup (p<0.05).Conclusions: Both active NMES and passive NMES are effective inenhancing the upper extremity motor and functional recovery of strokesurvivors. Active NMES provides more gains in grip strength and surfaceEMG potentials of wrist extensors from passive NMES.
