Özet

ÖZET A. BASILARISIN MİNÖR DALLARININ ANATOMİSİ Beynin posterior dolaşımında oldukça önemli bir arter olan a. basilaris, verdiği dallarla fossa cranii posterior'daki yapıların beslenmesine katılmaktadır. Çalışmamızda a. basilaris'in boyunu ve çapını ölçerek, özellikle anevrizma ameliyatlarında cerrahi yaklaşımın şeklinin belirlenmesinde önemli olan vertebrobasiler bileşke ile bifurcatio basilaris seviyesini değerlendik. A. basilaris'in, dallarından diğerine kıyasla daha büyük çapa sahip olan a. inferior anterior cerebelli, a. superior cerebelli ve a. cerebri posterior'ları major dallar, a. labyrinthi ve aa. pontis'i minor dallar olarak belirledik. Bugüne kadar farklı farklı sınıflandırmalar kullanılan aa. pontis'i perforan dallar ve kranial sinir dalları olarak iki grupta inceledik. Daha sonra perforan dalları da perforasyon bölgesine göre anterior perforan dallar ve posterior perforan dallar olarak ayırdık. Kranial sinir dalları başlığı altında topladığımız trigeminal dallar n. trigeminus'un beslenmesinde, a. labyrinthi ise n. vestibulocochlearis ve n. facialis'in beslenmesinde görev alıyordu. N. trigeminus'un beslenmesini de incelediğimiz çalışmamızda trigeminal dalların, a. inferior anterior cerebelli'nin, a. inferior posterior cerebelli'nin ve a. superior cerebelli'nin sinirin beslenmesine katıldığını gözledik. Ayrıca a. trigeminocerebellaıis'in görülme sıklığı daha önceki araştırmalarda %2,3 verilmişken, çalışmamızda üst trigeminal dalların %10'unun, alt trigeminal dalların %3,3'ünün a. trigeminocerebellaris olarak seyrettiği gözlendi. Çalışmamızda edindiğimiz veriler sonucunda aa. pontis'e dair yaptığımız yeni sınıflandırmanın klinisyenlere, cerrahi sırasında bu dalların korunması ve komplikasyonların engellenmesi konusunda yol göstereceğini düşünmekteyiz.

SUMMARY THE ANATOMY OF THE MINOR BRANCHES OF THE BASILAR ARTERY The basilar artery and its branches, quite important for the posterior circulation of the brain, contributes the blood supply of the structures in the posterior fossa. In our study, we measured the length and the diameter of the basilar artery and especially evaluated the level of the bifurcation of the basilar artery and the vertebrobasiler junction which are important to determine the operation approaches for the aneurysms. We defined the anterior inferior cerebellar artery, the superior cerebellar artery and the posterior cerebral artery which are larger in diameter than the other branches of the basilar artery as the major branches, where as the labyrinthine artery and the pontine branches were defined as the minor branches. We examined the pontine branches as the perforan branches and the cranial nerve branches which were classified variously until now. Then we categorized the perforan branches as the anterior perforan branches and the posterior perforan branches according to their perforation zones. Cranial nerve branches included the trigeminal branches that contributed to the arterial supply of the trigeminal nerve and the labyrinthine artery that contributed to the arterial supply of the facial nerve and the vestibulocochlear nerve. In addition, we evaluated the arterial supply of the trigeminal nerve in our study and observed that the trigeminal branches, anterior inferior cerebellar artery, posterior inferior cerebellar artery and the superior cerebellar artery contributed the supply of the trigeminal nerve. Furthermore, in previous studies the frequency of the trigeminocerebellar artery was given 2.3%, in our study 1 0% of the superior trigeminal branches and 3,3% of the inferior trigeminal branches were seen and noted as trigeminocerebeller artery. As a result of our observations we think that this 61new classification about the pontine branches will guide the clinicians in preserving the branches of the artery during surgery and preventing many complications.
