Özet
67 ÖZET Alveoler Kret Augmentasyonu Üzerine Bir Klinik, Radyolojik ve Stereofotogrametrik Değerlendirme Çalışmamızda; dişsiz, mandibular bilateral aşın kret rezorpsiyonunun görüldüğü 15 hastaya vikril meş tüplemesi yöntemi ile hidroksilapatit augmentasyonu yapılmıştır. Augmente edilen kret alanımn preoperatif, postoperatif 1. ay ve postoperatif 12. aylardaki bulguları klinik, radyolojik ve stereofotogrametrik olarak değerlendirilmiştir. Klinik değerlendirmede; yumuşak dokular değerlendirilerek augmentasyon uygulanan bölgedeki yumuşak dokuda gerilimin mevcut olup olmadığı, mukozanın rüptüre olup olmadığı, greft materyalinde migrasyonun görülüp görülmediği, bölgede lezyon, mukozada biperemi olup olmadığına bakılmıştır. Radyografik değerlendirmede; panoramik radyografilerden yararlanılmıştır. Alınan panoramik radyografilerle yabancı cisim reaksiyonunun olup olmadığı, greft materyalinde migrasyonun, rezorpsiyonun olup olmadığı değerlendirilmiştir. Stereofotogrametrik inceleme için hastalardan preoperatif, postoperatif 1. ay ve postoperatif 12. aylarda irreversible hidrokolloid ölçü maddesi ile ölçüler alınmışta Ölçülerden master modeller elde edilmiştir. Master modellerden stereograflar alınarak, stereofotogrametrik inceleme yapılmıştır. Stereofotogrametrik değerlendirme sonucu elde edilen sonuçlar ' paired t testi' sonuçlarına göre preoperatif- postoperatif 1. ay periodu ile preoperatif- postoperatif 12. ay periodu arasındaki geçişte hacim ve alan boyutsal değişimde anlamlı bir azalma ( p< 0,01) olduğu görülmüştür. Sonuçta; aşın alveoler kret rezorpsiyonu görülen hastalara uygulanan vikril meşli hidroksilapatit augmentasyonun; 1 yıllık takipte augmentasyon alanında boyutsal azalma görülmesine rağmen, tam protez kullanmak zorunda olan hastaların, fonksiyon, fonasyon ve estetiklerini tam olarak iade edebildiği ve diğer preprotetik cerrahi işlemlere göre daha ekonomik bir seçenek olduğu görülmektedir.

68 SUMMARY A Clinical, Radiological and Stereophotogrammetrical Evaluation on Alveolar Ridge Augmentation In this study, hydroxylapatite augmentation with vicryl mesh tubing method was applied to 15 patients who have edentulous mandible with bilateral ridge resorption. Clinical, radiological and stereophotogrammetric considerations were carried out preoperative and postoperative at the first and 12th months. In clinical evaluations, presence of tension in the soft tissue at the augmented part, lacerations in the oral mucosa, migrations in the augmented greft material, lesions and hyperemical areas were considered. Panoramical radiographs were used, in radiological evaluation. In panoramical radiographs; foreign substances, migration and resorptions in the greft materials were evaluated. With irreversible hydrocolloid impression materials, the impressions were taken for stereophotogrametric evaluations. From the impressions, the master models were taken and with help of stereograph which were taken from the master models, the stereophotogrametric evaluations were done. The results of stereophotogrametric evaluations with ' paired t test' method were seen, at the first month and 12th month periods were compared. The significant decrease in volume and decrease in area were observed ( p< 0,01). As a resut of this study, vicryl mesh tubing hydroxylapatite augmentation method was applied to the patients who had alveolar ridge resorption and in the postoperative first year follow- up, a decrease in the site of the alveolar ridge were observed. However patients gained their functional, estetic and fonatic abilities when they use their total prosthesis. Additionally, when this techique is compared to the other preprothetic surgical technique, it is more economical than the others.
