Özet

Bu çalışmanın amacı, protez stomatitisinin etyolojisinde predispozisyon yaratabilecek konağa bağlı faktörlerin yanı sıra kandida türlerinin patojenik aktivitesinde rol oynayan faktörlerden fosfolipaz enzimi aktivitesinin olası rolünün ortaya konmasıdır. Alt-üst tam protez kullanan ve sistemik hastalığı olmayan“protez stomatitisi”tanısı konmuş 50, sağlıklı mukozaya sahip 25 hasta çalışmaya dahil edilmiştir. Hastaların yaş, cinsiyet, eğitim durumu; protez yaşı, uyumu, hijyeni, kullanma ve temizleme alışkanlıkları; sigara kullanma alışkanlığı bilgileri kaydedilmiştir. Klinik muayeneden sonra mikolojik ve sitolojik örnekler alınmış, hastaların tükürük akış hızlan ve pH değerleri ölçülmüştür. Kandida kolonilerinin sayımı ve türlerinin tayinini takiben fosfolipaz aktiviteleri belirlenmiştir. Sitolojik örnekler kandida kolonizasyonu açısından değerlendirilmiştir. Klinik tipler arasında protezin uyumu ve hijyeni, gece protez kullanımı, tükürük pH'sı, kandida taşıyıcılığı bakımından istatistiksel olarak anlamlı farklılık bulunmuştur (p<0.05). Yaş, cinsiyet, eğitim durumu, protezin yaşı, protez temizleme sıklığı ve şekli, sigara kullanımı, tükürük akış hızı bakımından ise anlamlı farklılık saptanmamıştır. Klinik tipler arasında fosfolipaz üretimi bakımından istatistiksel olarak anlamlı farklılık saptanmıştır (p<0.05). Fosfolipaz üretimi ile damakta kandida kolonizasyonu arasında ileri düzeyde anlamlı ilişki bulunmuştur (p<0.01). Fosfolipaz üretimi negatif ve pozitif hastaların tükürük akış hızları ve pH düzeyleri arasında ise istatistiksel olarak anlamlı farklılık tespit edilmemiştir. Bu veriler doğrultusunda; Newton Tip I protez stomatitisinin proteze bağlı travma sonucu ortaya çıktığı, Tip III protez stomatitisinin etyolojisinde travmanın ve kandida enfeksiyonunun rol oynadığı belirlenmiştir. En yüksek fosfolipaz üretiminin Tip II protez stomatitisli hastalarda görülmüş olması, enfeksiyonun bu aşamasında kandidaların mukozada invaziv kolonizasyonunun yüksek olduğunu düşündürmüştür. Ekstraselüler fosfolipaz enziminin C. albicans' m dokuda invaziv kolonizasyonunda, dolayısıyla da patojenik potansiyelinde etkili olduğu sonucuna varılmıştır.

Relationship Between Phospholipase Activity of Candida Species and Cytological Changes, Clinical Types, Salivary Flow Rate and pH in Patients with Denture Stomatitis The aim of this study was to determine the possible role of phospholipase enzyme, which has an important role in pathogenity of Candida species, in the aetiology of denture stomatitis. Seventy-five complete denture wearers without systemic diseases - 50 with and 25 without signs of denture stomatitis - were included in the study. Demographic characteristics of subjects; age, fit and hygiene of dentures; overnight denture wearing; hygiene and smoking habits were recorded. After clinical examination, samples were obtained for mycological and cytological examinations, salivary flow rates and pH levels were determined. Following the determination of Candida colony counts and types, phospholipase activities of the isolates were established. Cytological samples were evaluated for presence of candidal colonization. Statistically significant relationships were found between types of denture stomatitis and fit and hygiene of the denture, overnight denture wearing, salivary pH levels, candidal carriage (p<0.05). However no significant relationships were found between types of denture stomatitis and demographic characteristics, age of the denture, denture cleaning habits, smoking, salivary flow rate. Significant differences were observed between phospholipase activity in different types of denture stomatitis (p<0.05). The relationship between phospholipase production and candidal colonization on the palate was found to be highly significant (p<0.01). There were no significant differences between the salivary flow rates and pH levels of phospholipase positive and negative patients.Based on these data; it was detected that Newton Type I denture stomatitis arose from denture-related trauma, and both trauma and candidal infection took part in the aetiology of Newton Type III. Highest phospholipase production was detected in patients with Type II denture stomatitis and it was thought that invasive candidal colonization was high at this stage of the infection. It was concluded that extracellular phospholipase enzyme appeared to be important in invasive colonization, consequently in pathogenic activity of C. albicans.
