6.ÖZET Konya il Merkezindeki Pediatri Servislerinde Çalışan Pediatri Hemşirelerinin Toplam Kalite Yönetim Sistemi Hakkındaki Bilgi ve Görüşleri Amaç: Bu araştırma 2. ve 3. basamak sağlık kurumlarında pediatri ve pediatrik cerrahi kliniklerinde çalışan hemşirelerin TKY hakkındaki bilgi ve düşüncelerinin değerlendirilmesi amacıyla yapılmıştır. Yöntem: Tanımlayıcı tipteki bu çalışma Konya İl merkezinde bulunan dört büyük hastanede Şubat-Mart 2005 tarihleri arasında yürütülmüştür. 85 pediatri hemşiresi araştırma kapsamına alınmıştır. Hemşirelere TKY hakkındaki bilgi ve düşüncelerini sorgulayan bir anket formu gözlem altında uygulanmıştır. Bulgular: Araştırmaya katılan hemşirelerin yaş ortalaması 27.6±3.75'ti. Çoğu (% 49.4) önlisans mezunu olup, % 58.8"i 48 aydan daha fazla süredir pediatri birimlerinde çalışmaktaydı. Katılımcıların % 52.9'u en az 1-2 saat TKY eğitimi aldığını belirtti. Sağlık Hizmetlerinde TKY uygulamalarına geçiş gerekçesi sorulduğunda, hemşirelerin % 4 1.2 'si uluslar arası kaliteyi yakalamak için, % 34.1' i sağlık hizmetlerinde sürekli iyileştirmeyi sağlamak için, % 16.5'i Avrupa Birliği'ne geçişi kolaylaştırmak için olduğunu belirtti. Hemşirelerin demografik özelliklerinin TKY' ye bakış açısını etkilememiş, alman TKY eğitimleri ve hastanelerin uygulamaya başlamış ya da başlamamış olmaları TKY'ye bakış açısını etkilemiştir. TKY eğitimi alan hemşirelerin TKY felsefelerine daha olumlu baktıkları, belgesiz hastanelerin de uygulamalara daha olumlu baktıkları bulundu. Pediatri hemşireleri TKY felsefelerim benimserken, personel eksikliği, kırtasiye yoğunluğu ve uygulamaların kağıt üzerinde kaldığını düşünmelerinden dolayı TKY uygulamalarına olumsuz baktıkları saptandı. Belgesiz hastanelerin hemşirelerinin TKY uygulamalarında görev almak için daha istekli oldukları bulundu. Sonuç: Kaliteli sağlık hizmet sunumunun gerçekleştirilebilmesi için TKY sürecine girilmesi gerektiği bilinen bir durumdur. Bu süreçte önemli bir rol üstlenen hemşirelerin konuya olumlu yaklaşmaları ve uygulamalarda yer alma istekleri umut vericidir. TKY uygulamalarının istenilen düzeyde olması için personelin bilgi ve düşüncelerinin alınması, motivasyonlarının sağlanması ve yeterli TKY eğitimi almalarının sağlanması etkili olacaktır. 69

SUMMARY Knowledge and Opinion About Total Quality Management Of Nurses Working In Pediatric Service InKonya City Center. Purpose; This research has been conducted in order to assess the views and knowledge of the nurses who work in the pediatric and pediatric surgery clinics in the second and third rank health centers. Method; This descriptive study has been conducted in the period of February to March 2005 in the four big hospitals that are situated in the city of Konya. 85 pediatric nurses have been taken into the scope of the research. The nurses have been carried out a survey that measures their knowledge and views about the total quality management by the method of application under observation. Findings: The average age of the nurses was 27,6 ±,3,75. The majority of the nurses (49.4%) had a vocational education degree and 58.8% of them were working in the pediatric clinics for more than 48 months. The 52.9 of the participants mentioned that they had been taught total quality management at least 1 to 2 hours. When they were asked about the reason to switch to the total quality management practices, the 41.2 % of the nurses replied it has been done so in order to catch up the international quality, 34.1% of them said to provide a permanent improvement in the health services and 16.5 % of the participants mentioned these practices would make it easier for Turkey to be admitted into the European Union. It has been seen that the demographic features of the nurses have not influenced their standpoint to the total quality management but their standpoint has been affected by the total quality management education and whether the hospitals has practiced total quality management or begun to do so. It has been found out that the nurses who have been educated on the total quality management and the hospitals without document have a more positive approach towards the idea of the total quality management. It has been determined that although the pediatric nurses accept the idea of the total quality management, they have a negative approach towards the total quality management practices due to lack of personnel, intensive red tape and the belief that the practices can only be achieved in theory. The nurses of the hospitals without the document have been seen to be more eager to take part in total quality management practices. Conclusion: That the total quality management process must be achieved in order to create a better service in healthcare is an apparent issue. It is promising that the nurses who are of great importance in this process have a positive approach to the issue and they are willing to take part in the practices. It will be pivotal to make the personnel to share their ideas and knowledge, to motivate them, and to get them educated about the total quality management in order to have a desired level of total quality management practices. 70
