Özet
ÖZET 1980-1984 yılları arasında Atatürk Üniversitesi Tıp fa kültesi Ortopedi ve Travmatoloji Kliniğine başvuranlardan, takip edebildiğimiz 20 plato kırığı olgusu konservatif ve cerrahi yön temlerle tedavi edildi. Plato kırıklarının nedenleri; trafik kazaları, diz üzerine düşmeler, ateşli silah, yaralanmaları, dizin sıkışması sonucu ya ralanmalar idi. Travmadan sonra hastaneye en erken başvuru 1 saat, en geç başvuru 5 günolup, ortalama gecikme zamanı 12.5 saattir. 20 olgumuzdan 13' ü konservatif olarak tedavi edilip; bun lardan 8» i alçı, 5'i traksiyon yöntemi ile idi. Cerrahi olarak tedavi edilen 7 olgudan; hafif deplase kı- rıklı bir olguya açık redüksiyon, deplase kırıklı bin diğer ol guya açık redüksiyon, 2 olguya ikişer adet kompresyon vidası, bir olguya tibial bolt ve kemik grefti, 2 olguya tibial bolt ve birer adet kortikal vida tatbik edildi. Konservatif tedavi edilen 13 olgudan 4' ünde (%30.7) mükem mel, 6'smda (% 49.2) iyi, l'inde (% 4.8) orta, 2' sinde (% 15.3) kötü sonuç alındı. Traksiyon yöntemi uyguladığımız 5 olgunun l'inde (% 20) mükemmel, 4' ünde (%80) iyi sonuç aldık. Kötü olgumuz yoktu. Alçı uyguladığımız 8 olgunun 3' ünde (% 37.5) mükemmel, 2' sinde (%25) iyi, l'inde (%12.5) orta ve 2' sinde (%25) kötü sonuç aldık.Cerrahi olarak tedavi ettiğimiz olgulardan; açık redüksi- yon uyguladığımız 2 olgudan l1 inde (% 50) mükemmel, l1 inde (% 50) orta sonuç, kompresyon vidası ile internal tesbit yapılan 2 ol gudan l*inde (% 50) mükemmel, l1 inde (% 50) kötü sonuç, tibial "bolt ile internal tesbit yapılan ve kemik grefti konulan 1 (% 100) olguda orta sonuç, tibial bolt ve kortikal vida ile internal tes- bit yapılan 2 olgudan l'inde (% 50) mükemmel, 1' inde (% 50) iyi sonuç alındı. Tedavisi tamamlanmış 20 plato kırığından; 7'sinde (% 55) mükemmel, 7'sinde (% 55) iy&, 5'ünde (%15) orta ve 5'ünde (%15) kötü sonuç alındı. SUMMARY Among the patients, who applied to the Orthopedy and Trau matology Clinic of Medical Collage of Atatürk university, between the year of 1980 and 1984j total of 20 patients with the condyle fractures of tibia were taken care and treated conservatively or surgically. The causes of the plateau fractures were traffic accidents, foiling down on to the knee, firearm wounds, sportive injuries and the wounds resulting from knee compaction. After the trauma, the earliest apply to hospital was in an hour, and the ; latest one "was in 5 days, overaging 12,5 hours. Thirteen of the 20 cases were treated conservatively; 8 of them were casted and to the other 5> some kind of braction method were applied. To the surgically treated 7 cases, the following surgical methods; an open reduction to a slightly deplaced one, again an open reduction to another deplaced fracture, to the other two cases for each one two compression screws, to another case tibial bolt and bone craft and to the other two cases tibial bolts and a cortical screw for each one were applied. Among the conservatively treated. 13 cases; in 4 (% JO. 7) cases superior results, in 6 (% 49.2) cases good results, in one (%4.8) case a fair result and in 2 (% 15»3) cases unacceptable results were obtained. In the 5 cases which traction methods were applied; in one (% 20) case a superior result in 4 (% 80) cases good results were obtained. There was no cases resulting unacceptably.Among casted 8 cases; in 5 (% 57 »5) cases superior results, in 2 (% 25) cases good, in one (% 12.5) a fair results and in 2 (% 25) cases unacceptable results were gathered. In the surgically treated cases; among the open reduction applied ores, in one (% 50) of the two cases a superior, in the other one (% 50) a fair results; in one (% 50) of the two cases which were internally fexed by usage of compression screws a su perior and in the otiier one (% 100$ case which is internally fixed whit a tibial bolt and bone craft applied a fair result, in one (% 50) of tine 2 cases which were internally fixed by the tibial bolt and cortical screw a superior and in the other one (% 50) a good result were obtained. In the completely treated 20 patients with the plateau frac tures; in 7 (% 55) cases superior, in the other 7 (% 3p) cases good, in 5 (% 15) cases fair and in the other 5 (% 15) cases un acceptable results were obtained.
