Özet
ÖZET: Antifosfolipid sendromu (AFS) antifosfolipid antikorları (AFA) varlığında ortaya çıkan, hem arter hem de ven sisteminde trombozlar ve tekrarlayan düşükler ile seyreden bir durumdur. AFA oldukça heterojendir; başta endotel aktivasyonu, protein C ve protein S aktivitesinin inhibisyonu, trombositlerin aktivasyonu, doku faktörü ekspresyonunun arttırılması ve fıbrinolitik aktivitenin bozulması gibi çok sayıda mekanizma ile tromboz gelişiminde katkısı olduğu düşünülmektedir. Her ne kadar epidemiyolojik çalışmalarda AFA- pozitif kişilerde tromboz riskinin arttığı gösterilmişse de, herhangi bir trombotik atak veya gebelik kaybı olmayan normal bireylerde de yüksek-pozitif AFA değerleri bulunabilmektedir. Bu nedenle AFS'da tromboz gelişiminde başka edinsel ve kalıtsal risk faktörlerinin etkisi olabileceği düşünülmektedir. Bu çalışmada AFS'da tromboz gelişiminde kalıtsal trombofilik risk faktörlerinin (protein C, protein S, antitrombin eksiklikleri; faktör V Leiden mutasyonu, protrombin gen mutasyonu, faktör XIII Val34Leu polimorfizmi ve P-selektin glikoprotein ligand-1 polimorfizmi) katkısı incelenmiştir. Trombotik komplikasyonu olan 73 AFS hastası; AFA-pozitif olup herhangi bir trombotik atağı olmayan, düşük ve trombositopeni nedeniyle izlenen 29 hasta ve 126 sağlıklı birey çalışmaya dahil edilmiştir. Bu çalışmada trombozlu olgular, tromboz geçirmemiş AFA-pozitif olgular ve sağlıklı bireyler kıyaslandığında protein C, protein S, antitrombin eksiklikleri; protrombin gen mutasyonu ve faktör XIII Val34Leu polimorfizmi açısından anlamlı bir fark olmadığı görülmüştür. Faktör V Leiden mutasyonunun normal sağlıklı bireylere kıyasla trombozlu bireylerde anlamlı oranda artmış olduğu gösterilmiştir. P-selektin glikoprotein ligand-1 polimorfizmi trombozlu olgular ile trombozsuz olgular ve sağlıklı bireyler ile karşılaştırıldığında, AB genotipinin trombozlu olgularda istatistiksel olarak anlamlı oranlarda arttığı (sırasıyla x2, p=0.0015, p=0.04) görülmüştür. Sonuçlarımız faktör V Leiden mutasyonu ve P-selektin glikoprotein ligand-1 AB genotipinin AFS'da tromboz gelişiminde katkısı olan önemli risk faktörleri olduğunu işaret etmektedir. Bu çalışmanın en önemli kısıtlaması trombozsuz AFA-pozitif grubun sayısının az oluşudur. Daha büyük olgu ve kontrol gruplarıyla çalışılması, AFS'da trombofilik mutasyonların gerçek rolünü adınlatacaktır. 42

SUMMARY: Antiphospholipid syndrome (APS) is characterized by both arterial and/or venous thrombosis and pregnancy losses in the presence of antiphospholipid antibodies (APLA). APLA are a heterogenous family of antibodies, which are thought to be responsible for thrombotic complications of APS in various ways including endothelial cell activation, inhibition of protein C and protein S activity, activation of platelets, increased tissue factor expression, and impairment of fibrinolytic activity. Although epidemiological studies showed an increased risk of thrombosis in patients positive for APLA, high-positive antiphospholipid antibody levels may be found in normal individuals without any history of thrombosis and fetal loss. It has been thought that other inherited or acquired thrombogenic risk factors might influence the development of thrombosis in patients with APS. In the present study, we examined the effect of inherited thrombophilic risk factors (inherited protein C, protein S and antithrombin deficiency; factor V Leiden mutation, prothrombin gene mutation, factor XIII Val34Leu polymorphism, and P-selectin glycoprotein ligand-1 polymorphism) in the development of thrombosis in patients with antiphospholipid syndrome. Seventy-three APS patients with arterial and venous thrombosis, 29 antiphospholipid antibody-positive patients with first trimester abortus and/or thrombocytopenia and no history of thrombosis, and 126 healthy controls were included into the study. We found that inherited protein C, protein S, antithrombin deficiencies, prothrombin gene mutation, and FXIII Val polymorphism showed no statistically significant difference between case and control groups. FV Leiden mutation was significantly higher in patients with thrombosis compared to healthy controls. When we compared P- selectin glycoprotein ligand-1 AB genotype between APS patients with thrombosis and both aPLA-positive patients without thrombosis and healthy controls; we found statistically significant difference (x2, p=0.0015, and p=0.04, respectively). Our results suggested that FVL mutation and P- selectin glycoprotein ligand-1 AB polymorphism are important factors in the development of thrombosis in the APS. The low number of patients in the group of APA-positive patients without thrombosis is the main limitation in our study. Further prospective studies in larger cohorts of patients are needed to delineate the exact role of thrombophilic mutations in the development of thrombosis in APS. 43
