Özet
Çalışmamızın amacı dar ve derin damak yapısı ile birlikte çift taraflı posterior çapraz kapanışı bulunan bireylerde hızlı üst çene genişletmesinin dentofasiyal yapılar, suturalar ve sfenooksipital sinkondrozis üzerine etkilerini incelemek ve bu konudaki literatür bilgilerine katkıda bulunmaktır. Bu amaçla, daimi dentisyonda maksiller darlıkla beraber çift taraflı posterior çapraz kapanışa sahip ve yaş ortalaması 13.14 olan 20 kız ve 20 erkek toplam 40 birey tedavi grubu olarak, yaş ortalaması 12.88 olan 10 kız ve 10 erkek toplam 20 birey de kontrol grubu olarak araştırma kapsamına alınmıştır. Tedavi grubunda; tedavi öncesi, tedavi sonrası ve ortalama 6 aylık pekiştirme tedavisi sonrası olmak üzere 3 kez, kontrol grubunda ise gözlem periyodu öncesi ve sonrası olmak üzere 2 kez lateral ve frontal sefalometrik filmler, ortodontik modeller ve bilgisayarlı tomografi görüntüleri alınmıştır. Tedavi grubunda alınan materyal üzerinde yapılan ölçümler; tedavi öncesi-tedavi sonrası, tedavi sonrası-pekiştirme sonrası, ve tedavi öncesi-pekiştirme sonrası şeklinde, kontrol grubunda ise gözlem periyodu öncesi ve sonrası şeklinde olmak üzere Eşleştirilmiş t testi ile karşı laştınlmıştır. Tedavi ve kontrol grupları arasındaki farklılıklar ise Student's t testi ile değerlendirilmiştir. Her iki grupta da çok sayıda ölçüm cinsiyet farklılığı gösterdiği için değerlendirmeler her iki cinste ayrı ayrı yapılmıştır. Sonuç olarak; hızlı üst çene genişletmesinin dental ve iskeletsel yapılarda transversal, sagittal ve vertikal yönlerde önemli etkilerinin olduğu gözlenmiştir. Bunun yanısıra bilgisayarlı tomografi incelemelerinde birçok suturada ve sfenooksipital sinkondroziste önemli açılmalar olduğu tespit edilmiştir.

This study aims at evaluating the effects of Rapid Maxillary Expansion (RME) on dentofacial structures, sutures and sphenooccipital synchondrosis in subjects with narrow and deep palate, and bilateral posterior crossbite and to contribute to the literary knowledge in this field of investigation. A total of 40 subjects, 20 girls and 20 boys with the mean age 13.14, with narrow maxilla and bilateral crossbite in permanent dentition were included in the study as the treatment group. Ten girls and 10 boys with the mean age was 12.88, with the same anomaly were chosen as the control group. Lateral and frontal cephalometric radiographs, dental casts and computerized tomography images were taken for each patient thrice, at the pre-treatment, post-treatment and at the end of the 6-month retention period in the treatment group; and twice, at the pre- observation and post-observation period in the control group. The measurements in the material were evaluated by“Paired t test”comparing the pre- treatment - post-treatment, post-treatment - end of the retention period, and pre- treatment - end of the retention periods in the treatment group, pre-observation - post- observation period in the control group. The differences between treatment and control groups were evaluated by means of Student's t test. As a number of measurements in both groups indicated significant sex differences, the assesments were made in both groups seperately. In conclusion; it was observed that RME has important effects on dental and skeletal structures in transversal, sagittal and vertical directions. In addition; the significant openings were determined in great number of sutures and sphenooccipital synchondrosis through computerized tomography images.
