Özet
IV ÖZET: Kronik idiopatik ürtiker, yıllarca devam eden, tedavisi zor, hem hasta hem de hekim için sıkıntı veren bir hastalıktır. Son yıllarda, daha önce kronik idiopatik ürtiker olarak adlandırılan ürtiker grubunun bir bölümü bugün, histamin salgılatan otoantikorların tespit edilmesi ve otoimmün tiroid hastalıkları ile artmış birlikteliği nedeni ile otoimmün ürtiker olarak kabul edilmektedir. Biz bu çalışmada kronik idiopatik ürtikerli 56 hastanın otolog serum deri testi pozitifliği ile tiroid otoimmünitesi arasında bir ilişki olup olmadığını araştırdık. Böylece kronik idiopatik ürtikerli hastalarda otoimmünitenin yerini değerlendirmeyi amaçladık. Çalışmaya kronik idiopatik ürtiker olarak değerlendirilmiş 56 hasta alındı. Tüm hastalara otolog serum deri testi yapıldı. Hastaların serum anti-TPO ve anti-TG antikor düzeyleri ölçülerek, otolog serum deri testi pozitifliği ile antikor pozitifliğinin ilişkisi değerlendirildi. 56 kronik idiopatik ürtikerli hastanın 24'ünde (%42,9) otolog serum deri testi pozitif bulundu. 56 hastanın 13'ünde (%23,2) anti-TPO ve/veya anti-TG antikoru pozitif bulundu. Bunlardan otolog serum deri testi pozitif olan 24 hastanın 7 'sinde (%29,2) en az bir tiroid otoantikoru pozitifti ve otolog serum deri testi negatif olan 32 hastanın 6'sında (%18,8) en az bir tiroid otoantikoru pozitifti. Otolog serum deri testi pozitif ve negatif olan gruplar arasında tiroid antikor pozitifliği açısından farklar istatistiksel olarak anlamlı değildi. Tiroid otoantikorları 37 kadın hastanın 12'sinde (%32,4) ve 19 erkek hastanın 1 'inde (%5,2) pozitif bulundu. Tiroid otoantikorları kadınlarda erkeklerden anlamlı olarak yüksekti. Kronik idiopatik ürtikerli hastalarda tiroid otoantikorları normal popülasyona göre artmış sıklıkta bulunur. Bununla birlikte kronik idiopatik ürtikerde histamin salgılatan otoantikorların bir göstergesi olan otolog serum deri testi pozitifliği ile tiroid otoimmünitesi arasında bir ilişki yoktur.

SUMMARY Chronic idiopathic urticaria is a disease which goes on through the years. It's hard to treat and it's annoying for both the doctor and the patient. Recently, the urticaria group which is called as chronic idiopathic urticaria before, was accepted as autoimmune urticaria because of the autoantibodies that secrets histamine and their rising unity with autoimmune thyroid illnesses. In this work, we researched whether there is a relationship between the pozitiveness of autologous serum skin test and thyroid autoimmunity in 56 patients. So, we tried to realize the relation of autoimmunity and chronic idiopatic urticaria. The research was made on 56 patients with idiopathic urticaria. Autologous serum test was made to all patients. By comparing the level of serum anti-TPO and anti-TG antibodies, the relation of pozitiveness of autologous serum skin test and pozitiveness of antibody was evaluated. Autologous serum skin test in 24 patients with chronic idiopathic urticaria of 56 was pozitive (%42,9). Anti-TPO and / or anti-TG antibody was found pozitive in 13 patients of 56 (%23,2). 7 patients of 24 that have pozitive autologous serum test (%29,2) had at least one thyroid autoantibody. 6 patients of 32 that have negative autologous serum test ( %18,8) had at least one pozitive thyroid autoantibody. Between the groups with pozitive and negative autologous serum skin test, there were not statistically meaningful differences in point of pozitiveness of thyroid antibody. Thyroid autoantibodies were pozitive in 12 female patients of 37 (%32,4) and in 1 male patient of 19 (%5,2). Thyroid antibodies were higher in females than males. Patients with chronic idiopathic urticaria have thyroid autoantibodies much more than normal population. In addition to this, in chronic idiopathic urticarias, there isn't any relationship between the pozitiveness of autologous serum skin test which is an indicator of autoantibodies that secrets histamine and thyroid autoimmunity.
