ÖZET HEMİPLEJİK HASTALARDA ÜST EKSTREMİTE FONKSİYONLARINDAKİ GERİ DÖNÜŞÜ ETKİLEYEN FAKTÖRLER Fzt Birgül DÖNMEZ Amaç: Bu çalışmanın amacı, strok (inme) geçirmiş hemiplejik hastalarda üst ekstremite fonksiyonlanndaki geri dönüşü etkileyen faktörleri saptamaktır. Gereç-Yöntem: Strok'tan itibaren en az altı ay geçmiş 112 hemiplejik hasta çalışmaya alınmıştır. Çalışmada kullanılan anketler aracılığıyla strok hastalarına ait demografik bilgiler, sigara, alkol kullanma ve düzenli egzersiz yapma alışkanlığı, hastanın emosyonel durumuna ilişkin bilgiler elde edilmiştir Ayrıca her hastada üst ekstremitenin duyu, algı ve motor fonksiyonları duyusal ve algısal fonksiyon testi, Fugl-Meyer üst ekstremite motor performans testi ve Motricity indeksi ile değerlendirilmiştir. Bulgular: Yaş faktörü ve strok'tan itibaren geçen süreye göre gruplandırılan hastalar arasında geri dönüş açısından anlamlı bir fark olmadığı tespit edilmiştir (p>0.05). Rehabilitasyon programı görmemiş olan hastaların duyu, algı ve motor fonksiyon testlerine ait puanların daha yüksek olduğu ve bu sonuçlar açısından sadece algısal fonksiyona ait puanlar arasında anlamlı fark olduğu tespit edilmiştir (p<0.05). Hastaların algıladıkları genel sağlık durumuna göre emosyonel durumu iyi olanların duyu, algı ve motor fonksiyon testi puanlarının anlamlı derecede daha yüksek olduğu ve gruplar arasında anlamlı fark olduğu tespit edilmiştir (p<0.05). Strok sonrası üst ekstremite motor fonksiyonlanndaki geri dönüş üzerine yaş, emosyonel durum ve etkilenen hemiplejik tarafın, el fonksiyonlanndaki geri dönüş üzerine de hemiplejik taraf ve emosyonel durumun anlamlı bir etkisinin olduğu belirlenmiştir (p<0.05). Sonuç: Çalışmanın sonucunda yaş, hemiplejik taraf ve emosyonel durumun üst ekstremitede geri dönüşü etkileyen en önemli faktörler olduğu bulunmuştur.

SUMMARY THE EFFECTIVE FACTORS TO THE RECOVERY OF UPPER EXTREMITY FUNCTIONS IN HEMIPLEGIC PATEENTS PT. BİRGÜL DÖNMEZ Objective: The purpose of this study was to determine the factors, influences the recovery of the upper extremity functions in the hemiplegic patients. Material and methods: This study was consisted of 112 hemiplegic patients with the condition that the time was left from the onset of the stroke at least 6 months. The patients were interwieved using a questionnaire containing data on demographics, smoking and alcohol habits, exercise habits and emotional status. In addition, the patient's sensorial, perceptual and motor functions of the upper extremity were examined by using the Sensorial and Perceptual Function Tests, the Fugl-Meyer Upper Extremity Performance Test, and the Motricity Index. Results: The patients were grouped according to the age and the time from the onset of the stroke. It was established that there was not a significant difference between the groups concerning the recovery. The patients who were not in a rehabilitation program, had higher scores in the sensorial, perceptual, and motor function tests and significant difference was found only in the scores of the perceptual function test (p<0.05). According to the self-perception of general health status, the patients, whose emotional status was good, had higher scores in the sensorial, perceptual and motor function tests and there was a significant difference between the groups (p<0.05). It was suggested that, the age, hemiplegic side and emotional status, have a significant influence on the motor recovery of the upper extremity functions. On the other hand the hemiplegic side and emotional status, have a significant influence on the recovery of hand functions after stroke (p<0.05). Conclusion: At the end of this study it was suggested.,, that the age, hemiplegic side and emotional status are important factors that effects the recovery of the upper extremity in hemiplegic patients.
