Özet
VI-OZET Efüzyonlu orta kulak iltihabı (OME) çocukluk çağındaki işitme kaybının en önemli nedenlerinden biridir. Haemophilus influenzae, Streptococcus pneumoniae ve Moraxella catarrhalis çocuklardaki OME olgularının ana patojenleri olarak bilinirler. Etkili antimikrobiyal tedaviye rağmen bakteriyel menenjit hala yüksek morbidite ve mortaliteye sahip bir hastalıktır. Vakaların çoğundan H. influenzae, S. pneumoniae ve Neisseria meningitidis sorumludur. Hem bakteriyel menenjitteki hem de OME'deki bu mikroorganizmalar konvansiyonel kültürlerde zor ürerler. Bu yüzden, bakteriyel DNA'yı saptamak ve konvansiyonel kültür yöntemini altın standart kabul ederek polimeraz zincir reaksiyonu (PCR) tekniğinin tanısal değerini belirlemek amacıyla beyin omurilik sıvısı (BOS) ve orta kulak efüzyonu örneklerinde PCR uygulandı. Menenjit ve OME şüpheli çocuklardan alınan 53'ü BOS ve 22'si OKE olmak üzere toplam 75 örnek üzerinde çalışıldı. Her bir örneğe konvansiyonel kültür ve PCR testi uygulandı. 53 BOS örneğinin birinde (S. pneumoniae) ve 22 OKE örneğinin üçünde (1 H. influenzae non b ve 2 M. catarrhalis) kültür pozitifliği saptandı. Çalışmadaki total BOS ve OKE örnekleri için duyarlılık, özgüllük, pozitif kestirim değeri, negatif kestirim değeri ve korelasyon oranı sırasıyla %100.0, %87.3, %30.8, %100.0 ve %88.0 olarak bulundu. Sonuç olarak, bu çalışma, PCR tekniğinin, çocuk menenjit ve OME vakalarından alınan örneklerden üç en yaygın patojeni daha fazla ve daha kısa zamanda saptayabildiğini ortaya koymaktadır. PCR tekniğinin de bu alanda rutin kullanıma girmesi ve mikrobiyolog ve klinisyen arasındaki iyi bir diyalogun sağlanması durumunda bu hastalıklardaki etyolojik ve dolayısıyla tedaviye doğru yaklaşım problemlerinin azalabileceği kanaatine varılmıştır. 46

VII-İNGİLİZCE ÖZET (SUMMARY) Comparison of Conventional Culture and PCR Techniques in Detection of Haemophilus influenzae in Cerebrospinal Fluids and Middle Ear Effusions Otitis media with effusion (OME) is one of the major causes of hearing loss in childhood, it is known that H. influenzae, S. pneumoniae, and M.catarrhalis are considered the chief pathogens of OME in children. Bacterial meningitis is still a disease with high morbidity and mortality rates despite the effective antimicrobial therapy. H.infiuenzae, S.pneumoniae, and Neisseria meningitidis are responsible for the most cases. These microorganisms in both bacterial meningitis and OME have a fastidious characteristics in conventional culture, that have not been able to make clear cascade for identification, therefore, PCR has been applied to the detection of bacterial DNA in cerebrospinal fluid (CSF) and middle ear effusion (MEE) specimens, and to evaluate the significant diagnostic value of PCR technique compared to conventional culture methods as“gold standard”. A total of 53 CSF and 22 MEE samples were collected from meningitis and OME suspected childrens respectively, that were tested by both conventional culture and PCR methods. One of 53 CSF {S.pneumoniae) and three of 22 MEE ( 1 H.infiuenzae non b and 2 M.catarrhalis) samples were culture positive. The PCR revealed genomic DNA sequences of 5 (S.pneumoniae) from 53 CSF samples, and 8 ( 3 H.infiuenzae, 3 M.catarrhalis and 2 yielding both S.pneumoniae and M.catarrhalis with each others) from 22 MEE samples. The sensitivity, specificity, positive predictive value, negative predictive value, and the correlation rate for the total study samples from both CSF and MEE were 100 %, 87.3 %, 30.8 %, 100 %, and 88 % respectively. In conclusion, this study showed that the PCR technique might detect the most common three pathogens more and in a shorter time period in the specimens taken from patients with meningitis and OME (in pediatric population). The routine use of PCR technique along with a good cooperation between microbiologists and clinicians, it was concluded that the problems associated with the etiology and proper treatment approaches would be minimized. 47
