Özet

1. ÖZET: Karpal tünel sendromu tanısı çoğu kez belirgin semptom ve bulguları ve konvansiyonel elektrofizyolojik inceleme sonuçlan ile kolaylıkla doğrulanmasına karşın, baza erken vakalarda tamda zorlanılmakta ve elektrofizyoloji laboratuvarının yükünü arttırmaktadır. Çalışmaya Haziran Eylül döneminde Dokuz Eylül üniversitesi Tıp fakültesi Nöroloji Anabilim Dalı'na KTS öntanısı ile başvuran 76 hasta ve 30 sağlıklı gönüllü alındı ve elektrofizyolojik incelemeler 212 elde yapıldı. 76 hastanın 30'unda elektrofizyolojik olarak KTS saptandı. Elektrofizyolojik olarak KTS saptanan ve saptanmayan grupların sübjektif yakınmalarının değerlendiren Boston Karpal Tünel Sendromu skorları arasında anlamlı fark saptanarak (P< 0.05), bu anketin elektrofizyolojik test öncesi uygulanmasıyla hem elektrofizyoloji laboratuvarın yükünün azaltılabileceği hem de ekonomik kazanç sağlanabileceği düşünüldü. KTS tanısında kullanılan sinir ileti çalışmalarının hangisinin daha duyarlı olduğu incelenerek, avuç içi-bilek segmentinde median sinir ileti hızındaki düşüşün KTS tanısındaki sensitivitesi % 100, spesifitesi % 98.3 bulundu. İkinci parmaktan ölçülen median sinir duysal latansındaki artışın tamdaki sensitivitesi % 93.3, spesifitesi % 86.7 bulundu. Bu iki ölçümün birikteliğinin hastalık tanısndaki sensitivite ve spesifitesinin % 100 olduğu görüldü. Motor tutulumu daha erken dönemde saptamak amacıyla yaptığımız akson sayımı sonuçlan, KTS grubu ile kontrol grubu arasında anlamlı farklılık gösterdiğinden ( Max-M alan için P= 0.000, motor unit sayısı için P= 0.002), motor tutulumu olan ve olmayan grubun akson sayılan da kontrol grubundan anlamlı derecede düşük bulunduğundan, noninvaziv olan bu yöntemin KTS tanısında, motor tutulumun erken saptanması için, rutin elektrofizyolojik incelemeler arasında yer alabileceğini düşünüyoruz. Ancak akson sayımı tekniklerinin tekrarlanabilirliğindeki güvensizlikler nedeniyle bu konuda daha fazla sayıda hasta içeren ve en az iki defa tekrarlanarak ortalamaların dikkate alındığı ölçümlerin yapıldığı çalışmalara gereksinim vardır.

2. SUMMARY: Although the diagnosis of CTS is usually confirmed easily with marked symptoms, signs and the results of conventional electrophysiological investigation methods, it is hard to reach diagnosis in early cases, so participation of the electrophysiologic laboratory task increases. 76 patient referred to Norology Deparment of Dokuz Eylül University Medical Faculty with CTS suspect and 30 healthy volunteers were included in our study between June - October 2003. Electrophysiologic investigations were done in 212 hands totally. CTS was detected electrophysiologically in 30 patients of 76. An significant difference (P<0,05) was detected between the BCTSQ scores assessing the subjective complaints of electrophsiologically CTS(+) and CTS (-) groups, so we suggest the density of electrophysiologic laboratory can be reduced in addition to economic profits by applying this questionaire to the patients. During searching the most sensitive nerve conduction method for the diagnosis of CTS, the sensivity of the decrease of the conduction velocity of median nerve at palm to wrist segment was found as 100 % and the specificity as 93,3 % respectively. The sensivity of the increase of the measurement of the sensory median nerve latency in the second digit was found as 93,3 % and the specifity as 86.7 % respectively. Both of the sensivity and the specifity of the association of these two tests were found as % 100. The motor unit number estimation (MUNE) results which we used to determine the motor involvement more earlier revealed significant difference between the CTS (+) group and the control group (P=0,000 for Max-M area, P=0,002 for MUNE) ; and MUNE was found lower in the groups both with/ without motor involvement in comparison to control group, so we suggest that this noninvasive method can be placed among the routine electrophysiologic investigation for the diagnosis of CTS. Because of the lack of the reliability of the reproducibility of MUNE techniques, we need studies done by calculating MUNE at least two different measurements and taking the average of these measurements into consideration in big sample groups.
