Özet

Bazı sayrılıkların sağaltımında fototerapi ve fotokemoterapi topikal ajanlarla kombine edilebilmektedir. Kombinasyondaki amaç, iyileşme için gerekli total ultraviolet (UV) dozunun ve seans sayısının düşürülmesi ve UVnin yan etkilerinin azaltılabilmesidir. Bu noktada kombine sağaltımda kullanılan topikal ajanın UV geçişini arttırıcı veya azaltıcı etkisi önem kazanmaktadır. UV geçişini azaltması sağaltım etkinliğinin azalmasına ve süresinin uzamasına, UV geçişini arttırması fotosensitivite reaksiyonlarına neden olabilmektedir. Bu çalışmada bazı sayrılıkların sağaltımında fototerapi ve fotokemoterapi ile kombine edilebilen klobetazol-17-propionat krem, tretinoin krem ve kalsipotriol kremin ince ve kalın olarak UVA'dan hemen önce uygulanmalarının, UVA geçişi üzerine olası etkilerinin araştırılması amaçlanmıştır. Tek kör olarak yapılan çalışmaya 27 olgu alınmıştır. Olgulara hiçbir topikal ajan uygulanmadan, topikal %0.1 tretinoin krem, %0.05klobetazol-17-propionatkrem, 5 mg/gr kalsipotriol kremin ince (0.1 cc/25 cm2) ve kalın tabaka (0.3 cc/25 cm2) ve gün örtüsü uygulanmasından hemen sonra, ardışık sıralarda doz arttırılarak UVA verilmiştir. 72 saat sonra, hangi sıralara hangi topikal ajanın uygulandığını bilmeyen bir hekim tarafından yapılan değerlendirmeyle her sıranın minimal fototoksik dozu (MFD) belirlenmiştir. Belirlenen sonuçların istatistiksel analizi "Wilcoxon signed ranks tesf ile yapılmıştır. Topikal ajan uygulanmadan belirlenen MFD değeri ile tretinoin krem, klobetazol-17- propionat krem ve kalsipotriol kremin ince ve kalın tabaka uygulanmasından hemen sonra UVA verilmesiyle saptanan MFD değerleri arasında istatistiksel olarak anlamlı fark belirlenmemiştir (p>0.0033). Kalsipotriol kremin kalın tabaka uygulanımının ince tabaka uygulanımına göre MFD'u belirgin olarak arttırdığı saptanmış ve aradaki fark istatistiksel olarak anlamlı bulunmuştur (p<0.0033). Gün örtüsünün tüm olgularda tam blokaj yaptığı izlenmiştir. Çalışma sonucunda %0.1 tretinoin krem ve %0.05 klobetazol-17-propionat kremin UVA verilmesinden hemen önce uygulamasının UVA geçişine belirgin etkisinin olmadığı sonucuna varılmıştır. 0.05 mg/gr kalsipotriol kremin UVA verilmesinden hemen önce, ince ve kalın uygulamalarının MFD üzerinde belirgin değişikliğe yol açmadığı, fakat kalın uygulamanın ince uygulamayla karşılaştırıldığında MFD'da artışa neden olduğu belirlenmiştir. Kalın uygulama ile ince uygulama arasında anlamlı derecede fark bulunmasına rağmen, kalın uygulamanın hiçbir topikal ajan uygulanmadan bakılan MFD ile anlamlı farklılık göstermemesi nedeniyle, kalsipotriol kremin UVA geçişi üzerine etkisinin belirlenebilmesi amacıyla daha geniş olgu serilerinde çalışmalar yapılması gerektiği düşünülebilmektedir. Phototherapy and photochemotherapy may be combined with topical agents for the therapy of some skin diseases. The combination therapies are designed to decrease the total dose and the adverse effects of ultraviolet (UV) radiation and the duration of treatment. Topical agents, which may be used in a combination therapy, should be evaluated for their blocking or enhancing effects on UV. Any agent that have a potential to decrease UV entrance to the skin may cause inadequate treatment efficiancy and the prolongation of treatment duration. The drugs which enhances the UV entrance may cause photosensitivity reactions. In the present study, the effects of thin and thick clobetasole-17-propionate, tretinoin and calcipotriol creams, which were applied immediately before UVA, on UVA penetration into skin is investigated. Twenty-seven cases were included in this single blind study. A phototest with UVA was performed to determine the minimal phototoxic dose (MPD) and the test was repeated with thin (0.1 cc/25 cm2) and thick (0.3 cc/25 cm2) 0.05 % clobetasole-17- propionate cream, thin and thick 0.1 % tretinoin cream, thin and thick 5 mg/gr calcipotriol cream and sunscreen. After 72 hours, the effects of each agent on MPD was investigated by another physician who doesn't know the applied topical agent. Wilcoxon signed ranks test was used for statistical evaluation. There are no statistically significant difference between the MPD values after pure UVA and after thin and thick applications of clobetasole-17-propionate, tretinoin and calcipotriol creams (p>0.0033). However thick calcipotriol application increased MPD values more than thin application and there was statistically significant difference between them (p<0.0033). The sunscreen had a blocking effect on UVA entrance in our all cases. Our results show that thin and thick applications of 0.05% clobetasole-17-propionate and 0.1 % tretinoin creams have no significant effect on UVA penetration into the skin. Although thin and thick applications of 0.05 mg/gr calcipotriol cream have no significant effect on MPD values, the comparison of thin applications with thick applications showed that there is a significant increase of MPD values with thick applications. We have also observed that there are no significant differences between the MPD values of only UVA and after thick calcipotriol applications. According to these results, it would be better to investigate the interaction between calcipotriol and UVA penetrance on large series.
