Özet
ÖZET Taşlı K., Alt ekstremite Amputelerinde Esnek ve Sert Soket Uygulamalarının Fonksiyona Etkilerinin Karşılaştırılması, Hacettepe Üniversitesi sağlık Bilimleri Enstitüsü Fizik Tedavi ve Rehabilitasyon Programı Doktora Tezi, Ankara, 2001. Dizüstü amputelerde soket yapımında kullanılan termoplastik materyal ve laminasyon tekniği ile elde edilen iki farklı diz üstü soketinin protezin süspansiyonuna yürüyüş ve fonksiyon üzerine olan etkilerini karşılaştırmak amacı ile yapılan çalışmaya Bio-flt Protez Ortez ve Rehabilitasyon merkezine başvuran 20 unilateral diz üstü ampute alınmıştır. Olgulara önce sert laminasyon soket ile daha sonra esnek soket uygulaması ile protez eğitimi verilmiş ve her iki uygulamayı takiben olgular değerlendirilmişlerdir. Çalışmanın sonucunda esnek soket uygulaması yapılan diz üstü amputelerde herhangi bir yardımcı süspansiyon aracına gereksinim duyulmadığı belirlenerek, sert soket uygulamasına göre negatif basınç ölçüm değerlerinin yüksek olduğu görülmüştür (p<0.05). Esnek soketli protez ile yürüyüşte sert soketli proteze göre yürüme hızının arttığı stabilite temini için genişletilen destek yüzeyinin normal değerlere daha yakın olduğu belirlenmiştir. Esnek soket uygulamasında ayrıca sağlam taraf adım uzunluğunda artış görülmüş, protezli tarafta daha uzun adım alma eğiliminde de azalma olduğu belirlenmiştir (p<0.05). Ambulasyon aktiviteleri her iki soket uygulamasında ayrı ayrı değerlendirilmiş ampütelerin esnek soketle bu aktiviteleri daha kısa sürede yaptıkları saptanmıştır (p<0.05). Esnek soket uygulamasında süspansiyonun daha fazla olması buna bağlı proprioseptif duyunun, denge ve stabilitenin artması hastada güven hissinin artmasına neden olduğundan çift adım uzunlukları değeri sert soket uygulamasına göre daha fazla bulunmuştur (p<0.05). Çalışmamız diz üstü amputelerde soket materyali seçiminin amputenin fonksiyonel aktivitelerine ne oranda katkı sağlayabileceğini göstermenin yanında, uygulama teknikleri bakımından da bu konu da çalışan fizyoterapist ve protez ortez teknikerlerine ışık tutacağı kanısındayız. Konu ile ilgili materyal alanındaki gelişmelerin daha yalandan takip edilmesi ve yeni uygulama tekniklerinin kullanılması ile esnek soket yapımının ülkemizde daha da yaygınlaşacağı düşüncesindeyiz.

VI ABSTRACT Task K., Comparison of the functional effects of flexible and hard sockets on the lower limb amputees, Doctorate theses Health Sciences Institute, Physical Therapy and Rehabilitation programme, Hacettepe University, Ankara, 2001. 20 unilateral above knee amputees were the subjects of the study which was carried out with the purpose of comparing the effects of flexible and hard sockets on the function and suspension. Patients were fitted with hard and flexible sockets respectively, received prosthetic treatment and assessed with functional evaluations after each application. It was found that there was not any requirement of suspension aids in flexible socket applications and the negative pressure measurements were found to be higher in flexible sockets when compared with hard sockets (p<0.05). Walking velocity is increased in flexible sockets and step width reduced to values which are considered to be more normal. Intact side step length showed an increase while prosthetic side step length presented diminution in flexible sockets. (p<0.05) Due to ambulation activity assessments, it was determined that the amputees could perform these activities in a shorter period with flexible sockets. Stride length was also found to be increased in flexible sockets, due to sufficient suspension, improved proprioception, balance, stability and feelings of security (p<0.0.5). Outcome of our study shows that the selection of materials in socket fabrication is very important in the functional state of above knee amputees and will give support to both the technical stuff and physiotherapists who are working in this field. It can be concluded that the new trends in material technology and new fitting designs will lead the flexible socket applications to be more popular in the future.
