Özet

Amaç: Serviks kanserinde lenf nodu metastazı en güçlü prognostik etkenler içinde yer almaktadır. Bu çalışmanın birincil amacı preoperatif F-18 FDG PET/BT görüntüleri üzerinden elde edilen metabolik-volümetrik parametreler ve radyomiks özelliklerin mRMR algoritmasıyla seçilerek beş farklı makine öğrenmesi algoritması ile modelleme sonucunda patolojik lenf nodu metastazı varlığının tahmin edilmesidir. İkincil amaç ise aynı yöntemlerle histolojik alt tipler olan; SCC ile adenokarsinom ayrımının öngörülmesidir. Gereç ve Yöntem: Preoperatif F-18 FDG PET/BT görüntülemesi yapılan, postoperatif lenf nodu patolojsi ve histopatolojik alt tipi kesin olarak bilinen 111 serviks kanseri hastası retrospektif olarak değerlendirildi. IBSI uyumlu LIFEx yazılımı aracılığıyla primer tümörden 163 adet radyomiks özellik oluşturuldu. Üç aşamalı filtrelemenin ardından 31 özellik mRMR algoritmasına sunuldu. Veri seti bazlı rastgele örnekleme ile %71 eğitim (n=79) ve %29 bağımsız (n=32) test seti olarak ayrıldı. Lojistik Regresyon, ElasticNet LR, Random Forest, SVM ve XGBoost olmak üzere beş farklı makine öğrenmesi algoritması uygulandı. Model performansları bağımsız test seti, 1000 iterasyonlu bootstrap validasyonu, DeLong testi, Hosmer-Lemeshow kalibrasyon testi, karar eğrisi analizi ve risk nomogramı ile değerlendirildi. Bulgular: Bütün metabolik-volümetrik parametreler (SUVmax, SUVmean, SUVmin, MTV, TLG), LN pozitif grupta istatistiksel açıdan anlamlı düzeyde daha yüksek saptandı (p<0,001). TLG, LN pozitif grupta LN negatif gruba oranla yaklaşık 5,3 kat yüksek saptandı (294,34 g vs. 55,76). mRMR analizinde lenf nodu metastazı için en güçlü parametre olarak SUV 75. persentil (skor: 0,192) tespit edildi. Makine öğrenmesi modellerinin bağımsız test setinde AUC değerleri 0,804 ile 0,849 arasında olduğu görüldü. En dengeli performans XGBoost modelinde görüldü (AUC=0,849). DCA analizi tüm modellerin yaklaşık %20-70 risk eşiği aralığında net klinik fayda verdiğini gösterdi. Histolojik alt tip ayrımında ise tüm modellerde şans düzeyine yakın ya da altında AUC değerleri görüldü (0,459-0,626). Sonuç: F-18 FDG PET/BT bazlı radyomiks özellikler ile klinik değişkenlerin mRMR algoritması kullanılarak seçimi ve makine öğrenmesi ile modelleme yapılması, serviks kanserinde preoperatif lenf nodu metastaz öngörüsünde klinik açıdan anlamlı sonuçlar vermektedir. XGBoost modelinin dengeli duyarlılık ve özgüllük profili iyi kalibrasyonu ve DCA'da kanıtlanan net klinik faydası bu yaklaşımın non-invaziv preoperatif evreleme sürecine katkıda bulunabileceğini desteklemektedir. Histolojik alt tip ayrımında tüm modellerde başırısızlık görülmesi FDG PET/BT radyomiksinin bu konudaki temel biyolojik kısıtlılığını yansıtmaktadır.

Objective: Lymph node metastasis is among the strongest prognostic factors in cervical cancer. The primary aim of this study is to predict the presence of pathological lymph node metastasis by selecting metabolic-volumetric parameters and radiomixture features obtained from preoperative F-18 FDG PET/CT images using the mRMR algorithm and modeling them with five different machine learning algorithms. The secondary aim is to predict the differentiation between histological subtypes, namely SCC and adenocarcinoma, using the same methods. Materials and Methods: 111 cervical cancer patients who underwent preoperative F-18 FDG PET/CT imaging and whose postoperative lymph node pathology and histopathological subtype were definitively known were retrospectively evaluated. 163 radiomixture features were generated from the primary tumor using IBSI-compatible LIFEx software. After three-stage filtering, 31 features were presented to the mRMR algorithm. Dataset-based random sampling was used to divide 71% into training (n=79) and 29% into independent (n=32) test sets. Five features were selected for lymph node metastasis and four for histological subtype differentiation. Five different machine learning algorithms were applied: Logistic Regression, ElasticNet LR, Random Forest, SVM, and XGBoost. Model performance was evaluated using independent test set, 1000-iteration bootstrap validation, DeLong test, Hosmer-Lemeshow calibration test, decision curve analysis, and risk nomogram. Results: All metabolic-volumetric parameters (SUVmax, SUVmean, SUVmin, MTV, TLG) were found to be statistically significantly higher in the LN- positive group (p<0.001). TLG was approximately 5.3 times higher in the LN-positive group compared to the LN-negative group (294.34 g vs. 55.76 g). In mRMR analysis, the SUV 75th percentile (score: 0.192) was identified as the strongest parameter for lymph node metastasis. The AUC values of the machine learning models in the independent test set ranged from 0.804 to 0.849. The most balanced performance was observed in the XGBoost model (AUC=0.849). DCA analysis showed that all models provided a clear clinical benefit within approximately the 20-70% risk threshold range. In histological subtype differentiation, AUC values close to or below the chance level were observed in all models (0.459-0.626). Conclusion: Selection of clinical variables with F-18 FDG PET/CT-based radiomixture features using the mRMR algorithm and modeling with machine learning yields clinically significant results in predicting preoperative lymph node metastasis in cervical cancer. The balanced sensitivity and specificity profile of the XGBoost model, its good calibration, and the clear clinical benefit demonstrated in DCA support the idea that this approach can contribute to the non-invasive preoperative staging process. The failure of all models to differentiate histological subtypes reflects the fundamental biological limitation of FDG PET/CT radiomixing in this regard.
