Özet

Amaç: Bu araştırmanın amacı tükürük İnterlökin-6 (IL-6), Nükleer faktör kappa-B reseptör aktivatörü ligandı (RANKL) ve Osteoprotegerin (OPG) biyobelirteçlerin osseointegrasyon süreci ile ilişkisini araştırmak ve farklı yüzey özelliklerinin erken iyileşme döneminde hasta konforu, tükürük biyobelirteçleri, implant stabilitesi ve marjinal kemik kaybı (MBL) üzerine etkilerini değerlendirmektir. Gereç ve Yöntem: Hastalar, uygulanan implantın yüzey özelliğine göre Maxicell ve TiPure Plus olmak üzere iki gruba ayrılmıştır. İmplant stabilitesi, cerrahi işlem sırasında ve üçüncü ay kontrolünde rezonans frekans analizi (RFA) kullanılarak değerlendirilmiştir. MBL üçüncü ayda panoramik radyografiler üzerinden ölçülmüştür. Postoperatif ağrı, görsel analog skala (VAS) ile; analjezik tüketimi ise kullanılan analjezik miktarı esas alınarak yedi gün boyunca değerlendirilmiştir. Tükürük örnekleri preoperatif dönemde ve postoperatif birinci ve üçüncü aylarda toplanmıştır. Bulgular: Çalışmaya yaş ortalaması 36.08±12.19 olan, 6'sı kadın ve 6'sı erkek olmak üzere toplam 12 birey dahil edilmiştir. Primer ve sekonder stabilite ile postoperatif ağrı kesici tüketimi açısından gruplar arasında anlamlı fark bulunmamıştır. MBL açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmamakla birlikte, TiPure Plus grubunda MBL değerlerinin Maxicell grubuna göre daha yüksek olduğu gözlenmiştir. Stabilite değişimi açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmış; Maxicell grubunda stabilite değerlerinde azalma izlenirken, TiPure Plus grubunda artış gözlenmiştir. Tükürük örneklerinde IL-6, RANKL ve OPG düzeylerinin preoperatif, 1. ay ve 3. ay ölçümleri arasında anlamlı farklılık saptanmamıştır. RANKL düzeylerinin preoperatif–3. ay değişimi değerlendirildiğinde, Maxicell grubunda artış, TiPure Plus grubunda ise azalma izlenmiş ve gruplar arasındaki fark istatistiksel olarak anlamlı bulunmuştur. Stabilite değişimi ile IL-6, RANKL ve OPG değişimleri arasında anlamlı korelasyon bulunmamıştır. RANKL/OPG oranı her iki grupta da preoperatif döneme kıyasla 3. ayda artış göstermiş; 3. ayda TiPure Plus grubundaki oranın Maxicell grubuna göre yüksek olduğu görülmüştür. Sonuçlar: Farklı yüzeyler erken iyileşme döneminde hasta konforu, primer ve sekonder stabilite ile MBL açısından benzer sonuçlar göstermiştir. Bununla birlikte, stabilite değişimi ve RANKL yanıtında gruplar arasında gözlenen farklılıklar, implant yüzey özelliklerinin erken dönem osseointegrasyon ve kemik remodeling süreci üzerinde etkili olabileceğini düşündürmektedir. Tükürük IL-6, RANKL ve OPG biyobelirteçlerinin osseointegrasyon süreci ile ilişkisini ortaya koymak için daha geniş örneklemli ve uzun dönem takipli çalışmalara ihtiyaç vardır.

Aim: The aim of this study was to investigate the relationship between salivary interleukin-6 (IL-6), receptor activator of nuclear factor kappa-B ligand (RANKL), and osteoprotegerin (OPG) biomarkers and the osseointegration process, and to evaluate the effects of different implant surface characteristics on patient comfort, implant stability, salivary biomarkers, and marginal bone loss (MBL) during the early healing period. Material and Method: Patients were divided into two groups according to the surface characteristics of the implant used: Maxicell and TiPure Plus. Implant stability was assessed using resonance frequency analysis (RFA) during the surgical procedure and at the third-month follow-up. MBL was measured on panoramic radiographs at the third month. Postoperative pain was evaluated using the visual analog scale (VAS), and analgesic consumption was assessed based on the number of analgesics used over seven days. Saliva samples were collected preoperatively and at the first and third postoperative months. Results: A total of 12 individuals, 6 females and 6 males, with a mean age of 36.08 ± 12.19 years were included in the study. No significant difference was found between the groups in terms of primary and secondary stability or postoperative amalgesic consumption. No statistically significant difference was observed between the groups in terms of MBL, MBL values were higher in the TiPure Plus group than in the Maxicell group. A statistically significant difference was found between the groups in terms of stability change; stability values decreased in the Maxicell group, whereas an increase was observed in the TiPure Plus group. No significant differences were detected in salivary IL-6, RANKL and OPG levels among the preoperative, first-month, and third-month measurements. When the preoperative–third-month change in RANKL levels was evaluated, an increase was observed in the Maxicell group, whereas a decrease was observed in the TiPure Plus group, and the difference between the groups was statistically significant. No significant correlations were found between stability change and changes in IL-6, RANKL, or OPG levels. The RANKL/OPG ratio increased in both groups at the third month compared with the preoperative period, and the third-month ratio was higher in the TiPure Plus group than in the Maxicell group. Conclusions: Different surfaces showed similar outcomes in terms of patient comfort, primary and secondary stability, and MBL during the early healing period. However, the observed differences between the groups in stability change and RANKL response suggest that implant surface characteristics may influence early osseointegration and bone remodeling processes. Further studies with larger sample sizes and long-term follow-up are needed to clarify the relationship between salivary IL-6, RANKL, and OPG biomarkers and the osseointegration process.
