Özet
Giriş ve Amaç: Obsesif Kompülsif Bozukluk (OKB), birçok fenomeni barındıran heterojen bir hastalıktır. Hastalığın yapısını anlamak için bu fenomenler arasındaki ilişkilerin aydınlatılması gerekmektedir. Nörobilişsel performans, bu fenomenlerden biridir ve işlevsellik açısından önem taşımaktadır. Literatürde OKB'de nörobilişsel performansta düşüş saptayan birçok çalışma bulunmasına rağmen, sonuçlar heterojen bir dağılım sergilemekte ve tutarsızlık göstermektedir. Üstbilişler, komorbid semptomlar ve OKB semptomları, nörobilişsel performans ile ilişkili olabilecek diğer fenomenlerdir. Çalışmamızda, bu fenomenler arasındaki ilişkiler incelenerek OKB'de nörobilişsel performans değişikliklerinin mekanizmasını aydınlatmak amaçlanmaktadır. Yöntem: Çalışma, 69 OKB tanılı birey ve 70 sağlıklı kontrol grubuyla kesitsel olarak gerçekleştirilmiştir. Katılımcılara Beck Anksiyete Envanteri, Beck Depresyon Envanteri, Üstbiliş Ölçeği-30, Boyutsal Obsesyon Kompülsiyon Ölçeği, Rey Osterrieth Karmaşık Figür Testi, Stroop Testi (TBAG Formu), İz Sürme Testi, Küplerle Desen Testi ve Sayı Dizisi Testi uygulanmıştır. Bulgular: Yapılan istatistiksel analizler sonucunda, OKB grubu sağlıklı kontrollere kıyasla daha düşük nörobilişsel performans sergilemiştir. OKB grubu, kontrol grubuna kıyasla tüm üstbilişsel bileşenlerde daha yüksek skorlarla ilişkili bulunmuştur. OKB grubunda, semptom boyutları ile nörobilişsel performans arasında istatistiksel olarak anlamlı bir ilişki gözlenmemiştir. Üstbilişlerle nörobilişsel performans arasında da istatistiksel olarak anlamlı bir ilişki saptanmamıştır. Buna karşın, komorbid semptomlar nörobilişsel performansta düşüşle ilişkili bulunmuştur. Yapısal eşitlik modelinde, üstbilişlerin nörobilişsel performans üzerindeki doğrudan etkisi istatistiksel olarak anlamlı bulunmazken, yalnızca depresif semptomlar aracılığıyla dolaylı etkisi anlamlı bulunmuştur. Sonuç: Bu çalışmada, OKB grubunun sağlıklı kontrollere kıyasla daha zayıf nörobilişsel performans sergilediği ve üstbilişsel bileşenlerde daha yüksek skorlarla ilişkili olduğu bulunmuştur. Üstbilişlerle nörobilişsel performans arasında doğrudan bir ilişki bulunmazken, depresif semptomlar aracılığıyla dolaylı bir ilişki saptanmıştır. Ayrıca, komorbid semptomların nörobilişsel performansta düşüşle ilişkili olduğu belirlenmiştir.

İntroduction and aims: Obsessive-Compulsive Disorder is a heterogeneous disorder that encompasses various phenomena. Understanding the structure of the disorder requires elucidating the relationships between these phenomena. One such phenomenon is neurocognitive performance, which is crucial for functionality. Although numerous studies in the literature have identified impaired neurocognitive performance in individuals with OCD, the findings are heterogeneous and inconsistent. Metacognition, comorbid symptoms, and OCD symptoms are potential factors associated with neurocognitive performance. This study aims to examine the relationships among these phenomena to clarify the mechanisms underlying neurocognitive performance changes in OCD. Method: The study was conducted cross-sectionally with 69 individuals diagnosed with OCD and 70 healthy controls. The following assessments were administered: Beck Anxiety Inventory, Beck Depression Inventory, Metacognitions Questionnaire-30, Dimensional Obsessive-Compulsive Scale, Rey-Osterrieth Complex Figure Test, Stroop Test TBAG Form, Trail Making Test, Block Design Test, and Digit Span Test. Results: Statistical analyses revealed that the OCD group exhibited poorer neurocognitive performance compared to healthy controls. The OCD group also scored higher on all metacognitive components relative to the control group. However, OCD symptom dimensions were not significantly associated with neurocognitive performance. No significant relationship was found between metacognition and neurocognitive performance in the OCD group. In contrast, comorbid symptoms were associated with declines in neurocognitive performance within the OCD group. In the structural equation model, metacognition did not have a direct significant effect on neurocognition, but its indirect effect through depressive symptoms was found to be statistically significant. Conclusion: In this study, it was found that the OCD group exhibited weaker neurocognitive performance compared to healthy controls and was associated with higher scores in metacognitive components. While no direct relationship was found between metacognition and neurocognition, an indirect effect was observed through depressive symptoms. Additionally, comorbid symptoms were found to be associated with a decline in neurocognitive performance.
