HİPERKALSEMİ VE OSTEOPOROZ TEDAVİSİNDE ZOLENDRONİK ASİT KULLANIMININ HİPOKALSEMİ GELİŞME SIKLIĞINA ETKİSİ: KARŞILAŞTIRMALI BİR ÇALIŞMA Bu çalışmanın amacı hiperkalsemi ve osteoporoz sebebiyle zolendronik asit verilen hastalarda hipokalsemi gelişme sıklığını karşılaştırmalı olarak araştırmaktır. Vitamin D eksikliği, böbrek yetmezliği, hipomagnezemi gibi ek risk faktörlerinin varlığında ciddi hipokalsemi olguları tanımlanmıştır. Hiperkalsemi nedeniyle zolendronik asit verilen hastalarda, diğer gruba kıyasla klinik ve laboratuvar olarak hipokalsemi gelişme sıklığının daha fazla olduğu düşünülmektedir. Bu konuda yapılan karşılaştırmalı çalışmalar yetersizdir. Bu çalışma literatürdeki tedavi endikasyonlarından ikisini karşılaştırmalı değerlendirerek bu boşluğu doldurmaya ve güvenli tedavi-izlem protokollerinin oluşturulmasına katkı sağlayacaktır. Retrospektif kesitsel tek merkezli çalışmamıza 1 Ocak 2022-28 Şubat 2026 tarihleri arasında 18 yaş üzeri toplam 129 hasta alındı. Bu hastaların 28'i genel dahiliye servisinde yatarak izlenen hiperkalsemi ve 101'i endokrinoloji ile geriatri polikliniklerinden izlenen ayaktan osteoporoz hastalarıydı. Hastane sistemi üzerinden gerekli klinik ve laboratuvar verilerine eksiksiz şekilde erişilemeyen hastalar ve osteoporoz grubu için tedavi öncesi hiperkalsemi saptanan hastalar çalışmaya dahil edilmedi. Tüm hastalarda PTH, 25(OH)D3, magnezyum bakıldıktan sonra; ZA tedavisi öncesi ve sonrası düzeltilmiş kalsiyum, eGFR, kreatinin, fosfor bakıldı. Çalışma Helsinki Deklarasyonu ilkelerine uygun olarak yapılmış ve İstanbul Medeniyet Üniversitesi Klinik Araştırmalar Etik Kurulu tarafından onaylanmıştır. Çalışmamızda hipokalsemi gelişme oranı hiperkalsemi grubunda %28,6, osteoporoz grubunda %10,9 olarak saptanmış olup bu fark istatistiksel olarak anlamlı bulunmuştur (p=0,007). Hipokalsemi gelişen hastalarda hiperkalsemi grubunun 25(OH)D3 medyan değerleri osteoporoz grubuna kıyasla istatistiksel olarak anlamlı düşük bulunmuştur (10,7'ye karşı 30,2 ng/mL; p=0,009). Benzer şekilde, hipokalsemi gelişen hiperkalsemi grubunda PTH medyan değerleri de osteoporoz grubundakilere göre anlamlı derecede düşük saptanmıştır (9,7'ye karşı 30,5 pg/mL; p=0,027). Bu bulgular, D vitamini eksikliği ve suprese PTH'ın zolendronik asit sonrası hipokalsemi gelişiminde kritik rol oynadığını desteklemektedir. Ayrıca hipokalsemi osteoporoz grubunda ortalama 15, hiperkalsemi grubunda ortalama 10. günde saptanmış; osteoporoz grubunda 10. günden önce kalsiyum kontrol edildiğinde ortalama 5, 10. günden sonra bakılanlarda ortalama 25. günde saptanmıştır. Ayrıca zolendronik asitin hiperkalsemi grubunda osteoporoz grubuna kıyasla daha derin hipokalsemiye yol açtığını ortaya koymuştur. Her iki grupta da tedavi sonrasında kalsiyum ve fosfor düzeylerinde anlamlı değişimler gözlenmiş; bununla birlikte böbrek fonksiyonlarında anlamlı bir bozulma saptanmamıştır. Tedavi öncesi 25(OH)D3 düzeyi, hipokalsemi gelişimi için tek bağımsız belirleyici olarak belirlenmiştir ve 25(OH)D3 düzeyindeki her 1 ng/mL'lik artışın hipokalsemi riskini yaklaşık %4,8 oranında azalttığı görülmüştür. Sonuç olarak bu çalışma, zolendronik asit kullanan hastalarda hipokalsemi riskinin endikasyona göre farklılaştığını; 25(OH)D3 ve PTH düzeyinin bu riski öngörmede güçlü bir belirteç olduğunu ortaya koymuştur. Elde edilen veriler, klinik pratikte hasta bazlı risk değerlendirmesinin önemini bir kez daha vurgulamakta; kalsiyum-vitamin D takibi ve güvenli tedavi izlem protokollerinin oluşturulmasına sağlam bir veri temeli sunmaktadır.

THE EFFECT OF ZOLENDRONIC ACID USE IN HYPERCALCEMIA TREATMENT VERSUS OSTEOPOROSIS TREATMENT ON THE INCIDENCEOF HYPOCALCEMIA: A COMPARATIVE STUDY The aim of this study was to comparatively investigate the incidence of hypocalcemia in patients receiving zoledronic acid for hypercalcemia versus osteoporosis. Severe hypocalcemia cases have been documented in the presence of additional risk factors such as vitamin D deficiency, renal failure and hypomagnesemia. It is hypothesized that the incidence of clinical and laboratory hypocalcemia is higher in patients receiving zoledronic acid for hypercalcemia compared to the osteoporosis group. Comparative studies on this subject remain limited. By comparatively evaluating two treatment indications documented in the literature, this study aims to address this gap and contribute to the development of safe treatment and monitoring protocols. A total of 129 patients aged 18 years and older were included in this retrospective crosssectional single-center study conducted between January 1, 2022, and February 28, 2026. Of these patients, 28 were hospitalized in the general internal medicine ward with hypercalcemia, and 101 were outpatients with osteoporosis followed at endocrinology and geriatrics outpatient clinics. Patients whose required clinical and laboratory data could not be fully accessed through the hospital information system, and patients in the osteoporosis group in whom hypercalcemia was detected prior to treatment, were excluded from the study. In all patients, PTH, 25(OH)D3 and magnesium levels were measured; corrected calcium, eGFR, creatinine and phosphorus levels were assessed both before and after zoledronic acid administration. The study was conducted in accordance with the principles of the Declaration of Helsinki and was approved by the Clinical Research Ethics Committee of Istanbul Medeniyet University. The rate of hypocalcemia was 28.6% in the hypercalcemia group and 10.9% in the osteoporosis group, a difference that was found to be statistically significant (p = 0.007). Among patients who developed hypocalcemia, the median 25(OH)D₃ levels in the hypercalcemia group were significantly lower compared to the osteoporosis group (10.7 vs. 30.2 ng/mL; p = 0.009). Similarly, median PTH levels in the hypercalcemia group that developed hypocalcemia were significantly lower than those in the osteoporosis group (9.7 vs. 30.5 pg/mL; p = 0.027). These findings support the critical role of vitamin D deficiency and suppressed PTH in the development of hypocalcemia following zoledronic acid administration. Hypocalcemia was detected at a mean of day 15 in the osteoporosis group and day 10 in the hypercalcemia group. Within the osteoporosis group, hypocalcemia was identified at a mean of day 5 in patients whose calcium was measured before day 10, and at a mean of day 25 in those monitored after day 10. The study further demonstrated that zoledronic acid led to more profound hypocalcemia in the hypercalcemia group compared to the osteoporosis group. Significant post-treatment changes in calcium and phosphorus levels were observed in both groups; however, no significant deterioration in renal function was detected. Pretreatment 25(OH)D3 level was identified as the sole independent predictor of hypocalcemia development, with each 1 ng/mL increase in 25(OH)D3 associated with an approximately 4.8% reduction in hypocalcemia risk. In conclusion, this study demonstrates that the risk of hypocalcemia in patients receiving zoledronic acid varies according to the indication, and that 25(OH)D3 and PTH levels serve as strong predictors of this risk. The findings further underscore the importance of individualized risk assessment in clinical practice and provide a solid evidence base for the implementation of calcium and vitamin D monitoring as well as the development of safe treatment and follow-up protocols.
