Özet

Amaç Bu çalışmanın amacı, baş-boyun bölgesindeki servikal lenf nodlarının ultrasonografi (USG) görüntüleri üzerinden normal, reaktif ve malign olarak otomatik tespitini ve sınıflandırılmasını sağlayan derin öğrenme modellerinin geliştirilmesidir. Bu sayede, USG muayenesi sırasında kullanıcı tecrübesine bağlı subjektivitenin azaltılması, malign oluşumlar için klinik ön tanı desteği sunulması ve hastaların biyopsi işlemleri öncesi doğru şekilde yönlendirilmesine katkı sağlanması hedeflenmiştir. Yöntem Çalışmada, 227 normal, 350 reaktif, 377 malign ve 50 arka plan olmak üzere toplam 1004 adet USG görüntüsünden oluşan bir veri seti kullanılmıştır. Sınırlayıcı kutu ile nesne tespiti için YOLO11m modeli, piksel tabanlı sınır segmentasyonu için ise YOLO11m-seg modeli üzerinden eğitim gerçekleşmiştir. Performans ölçümü, 5-Katlı Çapraz Doğrulama ve eğitim sürecinden tamamen bağımsız test seti (n=183) üzerinden yapılmıştır. Modellerin istatistiksel güvenilirliği 10.000 tekrarlı Bootstrap analizi ile doğrulanmış; gerçek zamanlı uygulanabilirlik 33 adet USG videosu üzerinden zamansal tutarlılık metrikleriyle ölçülmüştür. Modellerin karar verme mekanizmalarının şeffaflığı ise Grad-CAM ısı haritaları ile değerlendirilmiştir. Bulgular Bağımsız test setinde; segmentasyon modeli (YOLO11m-seg) nesne tespiti modeline (YOLO11m) kıyasla daha üstün performans (mAP@0.50 değerinde 0.853'e karşılık 0.819; F1-Skorunda 0.825'e karşılık 0.792) göstermiştir. Malign lenf nodu sınıfında nesne tespiti modeli %95.9, segmentasyon modeli %94.8 hassasiyet skorları elde etmiştir. Gerçek zamanlı USG videolarında yapılan analizde, modellerin hedefi kaybetmeden izleme başarısını gösteren zamansal stabilite skor ortalamalarında nesne tespiti modeli için %93.9, segmentasyon modeli için ise %93.3 takip skoru ölçülmüştür. Grad-CAM aktivasyon haritası analizleri, yüksek aktivasyon alanlarının tam olarak hedeflenen lenf nodunun merkezine ve ekojenite farkının belirginleştiği dış korteks sınırlarına odaklandığını kanıtlamıştır. Sonuç Geliştirilen YOLO tabanlı derin öğrenme modellerinin analizlerinden elde edilen genel performans metrikleri değerlendirildiğinde, lenf nodunun USG ile teşhisinde piksel düzeyinde analiz yapan sınır segmentasyonu mimarisinin, şekilsel asimetriyi değerlendiren sınırlayıcı kutu yöntemine göre belirgin bir üstünlük sağladığı saptanmıştır. Çalışmamızda elde edilen modellerin yüksek tanısal doğruluk göstermeleri ve prob hareketlerine dirençli dinamik takip yapabilmeleri, bu modellerin baş-boyun bölgesinde USG muayenesi süreçlerinde klinik pratiğe entegre edilme potansiyellerinin yüksek olduğunu göstermektedir.

Aim The aim of this study is to develop deep learning models for the automatic detection and classification of cervical lymph nodes in the head and neck region as normal, reactive, and malignant using ultrasonography (US) images. By doing so, it is aimed to reduce subjectivity related to user experience during US examinations, provide clinical preliminary diagnostic support for malignant formations, and contribute to the appropriate referral of patients prior to biopsy procedures. Method In this study, a dataset consisting of a total of 1004 US images—comprising 227 normal, 350 reactive, 377 malignant, and 50 background images—was utilized. Training was performed using the YOLO11m model for bounding box object detection and the YOLO11m-seg model for pixel-based boundary segmentation. Performance evaluation was conducted using 5-Fold Cross-Validation and an independent test set (n=183) completely excluded from the training process. The statistical reliability of the models was verified through a 10.000-iteration Bootstrap analysis, and real-time applicability was assessed on 33 US videos using temporal consistency metrics. The transparency of the models' decision-making mechanisms was evaluated using Grad-CAM heatmaps. Results On the independent test set, the segmentation model (YOLO11m-seg) demonstrated superior performance compared to the object detection model (YOLO11m) (mAP@0.50 of 0.853 versus 0.819; F1-Score of 0.825 versus 0.792). In the malignant lymph node class, the object detection model achieved 95.9% and the segmentation model achieved 94.8% precision scores. In the analysis conducted on real-time USG videos, the average temporal stability scores, indicating the models' success in tracking the target without losing it, were measured as 93.9% for the object detection model and 93.3% for the segmentation model. Grad-CAM activation map analyses proved that high activation areas focused exactly on the center of the targeted lymph node and the outer cortex borders where the echogenicity difference becomes prominent. Conclusion Considering the overall performance metrics obtained from the analyses of the developed YOLO-based deep learning models, it was determined that the boundary segmentation architecture, which performs pixel-level analysis in the diagnosis of lymph nodes via USG, provides a significant superiority over the bounding box method that evaluates shape asymmetry. The fact that the models obtained in our study show high diagnostic accuracy and can perform dynamic tracking resistant to probe movements indicates that these models have a high potential to be integrated into clinical practice during USG examination processes in the head and neck region.
